We reviewed 49 cases of flexor to extensor tendon transfer following injury to the radial nerve (22) or brachial plexus (27). Post-operative follow-up average 5.6 years (0.5-12.5). Function was improved in 84% (41) of patients. 16% (8) reported no improvement; of these, four (50%) had associated vascular injuries. In those with improvement, impaired coordination and dexterity were reported by 79% (15 of 19) of the plexus injuries and in 64% (14 of 22) of the radial nerve palsies. Premature fatigue was noted by 89% (17 of 19) of plexus injuries and in 82% (18 of 22) of radial nerve palsies. The

Rupture of the Achilles tendon is relatively uncommon, but most primary care physicians are occasionally faced with evaluation and treatment of such an injury. Diagnosis is not difficult and is based almost solely on the history and physical findings. To avoid misdiagnosis, the Thompson test should be performed in any evaluation of ankle pain. The three management options are casting, open surgical repair, and percutaneous surgical repair. Incidence of rerupture is slightly higher with casting, but the greater costs and higher risk of complications with surgical treatment seem to outweigh the

Rheumatoid arthritis is basically a disease of the synovium and involves the synovium-lined sheaths that surround many of the tendons in the hand and wrist. Proliferative synovitis affects the tendons, infiltrates the tendons, causes formation of nodules, changes their ultrastructure, and eventually leads to spontaneous rupture. The three common sites of tendon sheath involvement are the dorsal and palmar aspect of the wrist, and the palmar aspect of the digits. Early tenosynovectomy can prevent tendon ruptures and should therefore be the cornerstone of treatment. Once spontaneous rupture has …

The purpose of this study was to review patellar tendon ruptures in athletes treated with surgical repair. Six patellar tendon ruptures in five athletic patients were identified at our institution between 1980 and 1990. All patients had operative debridement and primary repair of the ruptured patellar tendon using nonabsorbable sutures passed through patellar drill holes at an average of 2.5 days after injury. Personal interview, physical examination, roentgenographic evaluation, and isokinetic strength testing were performed at an average of 6 years after surgery. At final followup, no patien

Chronic rupture of the posterior tibial tendon is a common disorder causing a progressive flat foot deformity. MR imaging is an excellent modality for assessing dysfunction of the tendon. It can easily and accurately detect and grade the tears. A variety of secondary soft tissue and bony abnormalities are associated with the tear and can be useful in confirming the diagnosis.

The case presented is a 22-year-old man with previously treated synovial chondromatosis of the shoulder. He subsequently presented with pain and tenderness above the biceps tendon. Magnetic resonance imaging and radiographs revealed recurrent synovial chondromatosis in the biceps tendon sheath. The patient underwent an open procedure to remove the loose bodies and partial tenosynovectomy. The patient has subsequently done well and resumed normal activities. To our knowledge there are no reported cases of synovial chondromatosis involving the biceps tendon sheath in the literature.

Posterior tibial tendon problems are uncommon sources of disability in athletes. Awareness of the condition and early diagnosis are important to prevent disability and prolonged time away from sports. Appropriate imaging studies will aid in diagnosis. Tendon tears in younger patients are different than the typical ruptures seen in middle-aged and older adults. Treatment must be individualized based on the type of tear and degree of deformity present. Other conditions such as dislocation of the posterior tibial tendon, accessory navicular and exertional compartment syndrome can involve the tibi

Author reports on a case of ectopic bone formation in the Achilles-tendons and reviews the literature. The pathologic change was caused probably by the chronic mechanical irritation suffered during the conservative treatment of the club feet. The continuity of the ossified mass was interrupted similarly to a fracture; the function was restored after an "osteosynthesis".

Extensor tendon injuries are conventionally treated by immobilizing the involved digit in an extended position for 4-6 weeks, followed by active mobilization. This regimen is compared with early controlled active mobilization with dynamic splintage. The improved results are demonstrated.

S-shaped force transducers were developed for measurement of the forces along intact tendons. After calibration, the transducers were applied to the flexor pollicis longus and flexor digitorum superficialis and profundus tendons of the index finger in five patients operated on for treatment of carpal tunnel syndrome. The tendon forces generated during passive and active motion of the wrist and fingers were recorded. For pinch function, the amount of the applied load was measured with a special pinch meter. Tendon forces in the range of 0.1 to 0.6 kgf were measured during passive mobilization o…

Twenty-three patients with zone V flexor tendon lacerations rehabilitated by the Kleinert protocol were studied at an average of 46 months after trauma. Hand function was subjectively normal in only eight. Of fourteen patients who were employed at the time of injury, eight returned to their original occupations. Eight others were working at other jobs, and we considered six more capable of employment. Only one had a poor functional result that precluded occupational use of the hand. Pinch/grip strengths recovered to 85%/79% of the uninvolved side. Independent flexor digitorum superficialis/fle

Tendinitis and overuse syndromes plague athletes and performing artists. Although rarely career ending, both may impair performance for significant periods of time. This article emphasizes the distinction between these two entities and discusses proper diagnosis and treatment.

We have reviewed 50 consecutive children treated during the last five years of injuries of nerves and tendons of the hand, wrist and forearm. Seventy six per cent of these injuries affected the hand and/or fingers. The rest were located in the wrist or forearm.

Flexor tendon ruptures in rheumatoid arthritis are caused by either attrition on bone spurs or by direct invasion of the tendon by hypertrop … rticular involvement from the rheumatoid disease, and to a lesser extent, by the number of ruptured tendons. In general, isolated or double ruptures within the carpal canal due to attrition have a better prognosis than those caused by invasive tenosynovitis since the condition of the tendons is more favorable for reconstruction; however, as the number of …

Extensor tendon injuries are commonly encountered and are frequently treated with less regard than flexor tendon injuries. However, several factors make them as difficult to treat and more prone to suture line rupture than their palmar counterparts. A technique is described to protect a tendon juncture on the back of the hand following tendon disruption. This allows early finger motion to prevent joint stiffness while preventing suture line disruption.

We evaluated the surgical results of 42 consecutive patients with spontaneous rupture of the Achilles tendon treated from 1973 to 1984 to determine the causes of rupture and to evaluate our treatment methods. Patients were divided into early and late repair groups and their charts reviewed to determine common clinical features. A new method of repair with early functional range of motion exercises before casting was used and compared to other techniques in common use. Thirty-one patients were contacted for long-term followup (average, 4.7 years). Twenty-three of these patients returned for int
