Airway management in a 980-lb patient: use of the Aintree intubation catheter.

We report a 22-year-old, 980-lb (445 kg) man with a body mass index of 163 kg/m(2), who needed intubation for tracheotomy surgery, as he was profoundly hypercarbic and reliant on a tight-fitting continuous positive airway pressure mask. Attempts at oral and nasal fiberoptic intubation during topical anesthesia were unsuccessful because of poor patient cooperation and epistaxis. Thus, after awake placement of a size 5 Laryngeal Mask Airway ProSeal LMA; (LMA North America, San Diego, CA), we induced anesthesia using sevoflurane. Then we placed an Aintree stylet (Cook Critical Care, Bloomington, …

Respiratory outcome of midface advancement with distraction: a comparison between Le Fort III and frontofacial monobloc.

Upper airway stenosis in patients with faciocraniosynostosis is very common and often severe. Midface advancement, either with a Le Fort III or concomitantly to a monobloc frontofacial advancement, may prevent a tracheotomy or result in its ablation. The amelioration of respiratory function appears to be much better if the midface advancement is combined with distraction osteogenesis, although large studies with long-term follow-up are rare. In this study, we reviewed the respiratory outcome between Le Fort III with distraction and monobloc advancement with distraction in 54 faciocraniosynosto…

Inflammatory myofibroblastic tumor in the larynx: clinicopathologic features and histogenesis.

Inflammatory myofibroblastic tumor is extremely rare in the larynx and can mimic a malignant process. We present the case of a 62-year-old male who required tracheotomy due to rapidly progressive stridor. Laryngoscopy showed an exophytic, occlusive tumor located in the right true vocal cord. CT showed an expansive mass measuring 2 x2 x1.3 cm3 and occupying the anterior commissure, with glottic progression to the right true vocal cord. The tumor was completely resected with a CO2 laser via a transoral approach. Histologic examination demonstrated extensive ulceration with the presence of granul…

Circumferential resection of crico-tracheal stenosis with primary end-to-end anastomosis.

Postintubation injury of the upper airway commonly results in stenotic lesions of the larynx, subglottis, and adjacent trachea. The traditio … nts with crico-tracheal stenosis. The cause of the stenosis was related to intubation and/or failed tracheotomy in 87% of the patients. Two to 7 tracheal rings were resected and a tension-free anastomosis was achieved with mobilization techniques that were limited to peritracheal dissection. The anastomosis success rate was 96%. Therapeutic concepts in upper airway obstruction.

In children, congenital or acquired upper airway obstruction can present as an acute, potentially life-threatening condition or a chronic process. The latter may be progressive and lead to chronic respiratory failure or episodes of acute airway obstruction. Therapeutic options have been greatly expanded recently, and the single approach of tracheotomy has been replaced by a complex and invidualised approach in many cases. Conservative treatment ranges from topical application or local injection to systemic administration of pharmacological substances such as steroids, adrenalin and antibiotics…

[Surgical therapy of sleep-related respiratory disorders].

Before the invention of CPAP therapy, the only effective surgical treatment for obstructive sleep apnea syndrome was tracheotomy. Now, surgical approaches mainly focus on two anatomical sites. Each procedure influences either the retropalatal or retrolingual portion of the pharynx. They might be applied individually, synchronously with other procedures, or sequentially with other therapeutic devices. Common methods of velar surgery include uvulopalatopharyngoplasty (UPPP), Laser-assisted uvuloplasty (LAUP), radiofrequency or uvulaflap. These techniques are indicated mainly in patients with mil…

Acute epiglottitis in northern Finland today--still a potential emergency situation.

To examine patient characteristics related to airway intervention in acute epiglottitis and how the intervention affected the further course of the disease, 46 pediatric and 49 adult cases were retrospectively evaluated. All three airway management approaches, namely observation, nasotracheal intubation and tracheotomy were needed in both the children and adults. Five out of every six children needed an artificial airway, while observation was sufficient in three out of every five adults. H influenzae type b bacteremia in children, a short duration of the symptoms in adults and most important,…

[Removal of foreign bodies from the tracheobronchial system in childhood].

We have reviewed 103 cases of rigid tracheobronchoscopies that were performed in pediatric patients at the ENT University Hospital in Tübing … bronchoscopy. There were no serious complications such as bleeding or airway obstruction requiring tracheotomy. At the University Hospital rigid tracheobronchoscopy was found to be an effective and safe procedure. In particular, the procedure should be carried out if the history or clinical findings suggest foreign body aspiration.

[Therapy of cystic lymphangioma in childhood. Report of 4 cases with manifestations in the area of the head-neck].

BACKGROUND: Lymphangiomas are localized in the head and neck area in about 75% of cases. About 75% of these cases are children less than one year old. Treating lymphangiomas with installation therapy to obliterate the cysts has been discussed, but surgery remains the treatment of choice. PATIENTS: Two lymphangiomas in newborns required resection due to the size of the lesions. The surgery involved monitoring of the facial and hypoglossal nerves. A third child had a large lesion with infiltration into the supraglottic space and the tongue requiring a tracheotomy. In a four-and-one-half-year-ol…

The solution in functional rehabilitation after destruction of the larynx: the tracheostoma closed.

A bayonet-shaped airway allowing free respiration and undisturbed deglutition was reconstructed in 2 young women who had undergone tracheotomy and radiotherapy for thyroid carcinoma. The laryngeal vestibule, glottis, and subglottis had been destroyed, and the air passages were closed. An external gutter and an anterior tracheal wall were created in a multistage procedure. In 1 patient, the hypopharynx and upper esophageal segment were reconstructed with bilateral pectoralis major musculocutaneous flaps, which disfigured the breast. The two displaced nipple-areola complexes were properly reposi…

[Minimally invasive treatment of postintubation stenosis by use of Ultra Dream Pulse Laser and steroid-mitomycin in a 4-year-old girl].

Postintubation stenosis is a frequent complication of long-term endotracheal anesthesia. In the last few decades, its incidence showed an increasing tendency particularly among children and premature infants. It mostly affects the subglottic area and avoidance of a tracheotomy could lead to better life quality of the patient. We present the treatment of a glotto-subglottic stenosis in a 4-year-old girl. Ultra Dream Pulse Laser surgery was performed with mometason (Elocom) and mitomycin (Mitomycin-C) submucosal injections to prevent refibrosis. Minimally invasive operations play a key role in t…
