Predictors for avascular necrosis of the femoral head in closed reduction following traction for developmental dysplasia of the hip.

Avascular necrosis (AVN) is a serious complication associated with the treatment of developmental dysplasia of the hip (DDH). In this study, we analyzed 241 affected hips from 232 patients who underwent closed reduction following traction (CR/TR) between 1995 and 2021 to identify risk factors for AVN and establish clinical strategies to reduce its incidence. All patients received four weeks of traction (horizontal, vertical, and gradual abduction) followed by closed reduction. The severity of dislocation was assessed using the International Hip Dysplasia Institute (IHDI) classification and Yam…

A useful technique using a traction device and a rotatable sphincterotome for difficult biliary cannulation of an intradiverticular papilla.

An intradiverticular papilla can make biliary cannulation challenging during endoscopic retrograde cholangiopancreatography. With an accompanying video, Asano and colleagues describe their technique using a traction device to expose the papilla and a rotatable sphincterotome to align with the bile duct axis, achieving successful biliary cannulation without adverse events.

Efficacy of posterior scleral reinforcement in the management of myopic traction maculopathy: a single-arm retrospective study.

BACKGROUND: Myopic traction maculopathy (MTM) is a condition characterized by retinal complications due to excessive axial elongation in myopic eyes. Posterior scleral reinforcement (PSR) stabilizes the posterior pole by mechanically supporting the thinned sclera, limiting further axial elongation, and reducing macular traction. This study evaluates the efficacy of PSR in patients diagnosed with MTM. METHODS: This study included patients with myopic traction maculopathy (MTM) who underwent posterior scleral reinforcement (PSR) between September 2020 and February 2023. Inclusion criteria were …

Hip Rotation and Femoral Anteversion and Its Influence on Traction Force of the Pulled Limb in Hip Arthroscopy.

BACKGROUND: Several variables may affect the traction force during hip arthroscopy. Specifically, the degree of hip joint rotation may influence the magnitude of traction force during hip arthroscopy. However, there is currently limited research available on this particular issue. PURPOSE: To quantify the traction force applied to the pulled limb in various traction states and rotational positions. Additionally, the study aimed to investigate potential correlations between femoral anteversion, BMI, anesthesia methods, and the traction force required for hip dislocation. HYPOTHESIS: It was hy…

Effect of cervical traction on cervicogenic headache in patients with cervical radiculopathy: a preliminary randomized controlled trial.

BACKGROUND: Cervical radiculopathy (CR) is a common condition, often associated with cervicogenic headache (CGH), a secondary headache arising from cervical spine disorders. Mechanical intermittent cervical traction (MICT) is frequently prescribed to treat CR symptoms. The purpose of the study was to make a preliminary estimate of efficacy of adding MICT to conventional rehabilitation on CGH in patients with cervical radiculopathy. METHODS: A total of 36 CR patients with CGH were randomly allocated to 3 equally sized groups (A, B and C). The treatment consisted of twelve sessions of conventio…

Shoulder Traction as a Possible Risk Factor for C5 Palsy in Anterior Cervical Surgery: A Cadaveric Study.

Background and Objectives: Many risk factors for postoperative C5 palsy (PC5P) have been reported regarding a "cord shift" after a posterior approach. However, there are few reports about shoulder traction as a possible risk factor of anterior cervical surgery. Therefore, we assessed the stretched nerve roots when shoulder traction was applied on cadavers. Materials and Methods: Eight cadavers were employed in this study, available based on age and the presence of foramen stenosis. After dissecting the sternocleidomastoid muscle of the cadaver, the shoulder joint was pulled with a force of 2, …

Association of Treatment Type and Loss to Follow-up With Tractional Retinal Detachment in Proliferative Diabetic Retinopathy.

IMPORTANCE: The association of proliferative diabetic retinopathy (PDR) interventions of panretinal photocoagulation (PRP) and intravitreal injections (IVIs) with tractional retinal detachment (TRD) is unclear. OBJECTIVES: To determine whether different treatment types or a 6-month or longer period of loss to follow-up (LTFU) is associated with TRD. DESIGN, SETTING, AND PARTICIPANTS: This nested case-control study included data from January 1, 2000, to June 30, 2021, of patients with PDR. Those who progressed to TRD were matched to non-TRD controls up to a 5:1 ratio. Exclusion criteria inclu…

Minimal surgery for tractional retinal detachment secondary to branch retinal vein occlusion: a case report.

BACKGROUND: Tractional retinal detachment secondary to retinal vein occlusion is a complex entity that can be extremely difficult to manage due to an intricate association of the retinal tissue with the fibrovascular proliferation, making vitreous dissection an extraordinarily difficult procedure. Minimal surgery without endo-tamponade can reduce recovery time and avoid complications of surgery, which in some cases can lead to blindness and even phthisis. CASE PRESENTATION: A 64-year-old Indian woman presented with progressive worsening of vision (right eye) due to fovea involving tractional …

Spontaneous resolution of subretinal fluid postvitreoretinal surgery for tractional retinal detachment in proliferative diabetic retinopathy.

A man in his 60s with proliferative diabetic retinopathy presented with vision loss in both eyes in the past 1 month. On presentation, the best corrected visual acuity was 20/60 and 20/125 for the right and left eyes, respectively. On examination, he had total tractional retinal detachment (TRD) in the left eye and macula threatening TRD in the right eye. In both eyes, the patient had pars plana vitrectomy, membrane peeling, endolaser and silicone oil injection. Though the left eye's postoperative recovery was uneventful, the subretinal fluid (SRF) in the right eye macula persisted. There was …

The role of magnetic anchoring and traction technique in thoracoscopic lymphadenectomy along the left recurrent laryngeal nerve.

BACKGROUND: Dissecting lymph nodes along the left recurrent laryngeal nerve (LRLN) is the most challenging step in thoracoscopic-assisted esophagectomy. To retract the proximal esophagus in the existing lymphadenectomy methods, either a special trocar is required to insert and take out endoscopic instruments or thoracic punctures are needed to externally retract the esophageal loop. Therefore, advanced skills for esophageal traction are important to facilitate the LRLN lymphadenectomy and to reduce the incidence of trauma to the chest wall. Herein, we present the magnetic anchoring and tractio…
