Transfer from pediatric to adult care is a critical component of a high-quality transition experience for adolescents and young adults (AYA) with chronic illness. To examine the current evidence regarding the effect of transition interventions on care transfer, we performed a systematic review of studies that evaluated the effect of transition interventions on the specific health services outcome of transfer. The Medline, CINAHL, and PsycINFO databases were searched for studies that evaluated 1) a discrete transition intervention for AYA, 2) included a comparison group, and 3) reported on the

BACKGROUND: Persons living with dementia (PLWDs) and their care partners (CPs) often face decisions about future supportive care transitions.
METHODS: We conducted in-depth interviews with a purposive sample of healthcare professionals (HCPs) experienced in supporting PLWDs and CPs with transitions in care. Interviews were audio recorded, transcribed, and coded using a hybrid deductive-inductive approach. We used thematic analysis to identify key themes regarding factors influencing supportive care decisions and planning and decision-making facilitators used by HCPs.
RESULTS: Qualitative ana

UNLABELLED: When people with long term health issues transition from illness to health, or move from hospital to home after an exacerbation they feel vulnerable, unsafe, uncertain, lost, and unsupported. Transitions are life experiences that result in change. Telehealth gives easier access to care and increases patient involvement and self-awareness for self-care and improved outcomes. The purpose of our research was to explore how telehealth tools and processes lend themselves to nursing of patients through transitions.
METHODS: A multimethod study with before and after questionnaires consis

BACKGROUND/AIM: Chemotherapy administered near death is a recognized quality indicator of end-of-life cancer care intensity. However, simple clinical signals to prompt treatment reassessment and coordinate planned end-of-life care remain unclear in advanced gastric cancer. This study evaluated the interval from last chemotherapy to death and explored factors associated with near-end-of-life chemotherapy and failure to transition to a planned end-of-life care setting.
PATIENTS AND METHODS: This single-center retrospective study included 53 patients with advanced or recurrent gastric cancer who

BACKGROUND: Psychiatric disorders often emerge during adolescence or young adulthood, leading to significant disability among youth. The transition from Child and Adolescent Mental Health Services (CAMHS) to Adult Mental Health Services (AMHS) is critical for individuals experiencing emerging psychopathology, with delayed access to care negatively impacting long-term outcomes. Accessing mental health services for adolescents and young adults is often complex and delayed due to challenges in service visibility, accessibility and appropriateness.
METHODS: This study examines the care trajectori

BACKGROUND: Disease activity in juvenile idiopathic arthritis (JIA) commonly persists into adulthood. Transfer of JIA patients to adult healthcare services can be challenging, with prior studies showing poor rates of success.
AIMS: This audit sought to examine characteristics of patients undergoing transfer of care within the rheumatology unit at the Royal Children's Hospital in Melbourne, with the aim of identifying areas for improvement. Specifically, we sought to determine the rate at which confirmation of established care with an adult service (confirmed transfer of care) was documented i

Spinal Muscular Atrophy (SMA) is a severe complex disorder involving different aspects of care and professionals. Helping individuals to achieve their best possible quality of life is an essential part of health care. A multidisciplinary approach to management across the range of actors improves the function, quality of life and longevity of patients with SMA. Multidisciplinary management should be designed to address the psychosocial challenges of patients with prolonged survival and novel therapies. In this part, we focus on multidisciplinary management of SMA, pluridisciplinary consultation

BACKGROUND: There has been little prior investigation of the general health of young people with intellectual disabilities across transition, nor separately for youth with intellectual disabilities with or without Down syndrome, despite general health being a strong predictor of subsequent health service use, hospital admissions and mortality in the general population. We aimed to investigate general health status in youth with intellectual disabilities with and without Down syndrome over the transitional period and quantify the extent to which personal characteristics, parental relationship a

Advances in newborn screening, molecular diagnostics, and targeted therapies have markedly improved survival for individuals with inborn errors of metabolism (IEM), allowing many to live well into adulthood. While some may remain asymptomatic, their underlying metabolic vulnerabilities persist and can be unmasked by illness, stress, or other triggers. As a result, a growing population of adults with IEM now faces unique and evolving challenges related to long-term disease management, continuity of care, and quality of life. Despite these shifting demographics, there remains limited literature

Background: Many people want to die at home, but it is often not possible because they do not share their wishes with family members. This study was conducted to find out the extent to which patients with advanced urological malignancies had wishes regarding their final stage of life, made arrangements accordingly, and communicated their wishes to relatives and health care professionals. Methods: We conducted a survey among advanced urological tumor patients during their clinic visit at a German university hospital using a 31-item questionnaire. Inclusion criteria were metastatic or irresectab

OBJECTIVES: To describe (1) the content of a transition programme for young people with juvenile idiopathic arthritis (JIA) designed as a brief intervention, (2) the rationale and design of a mixed-methods study evaluating the clinical impact of this transition programme and (3) to provide baseline data of the intervention group.
DESIGN: An 'embedded experimental' design is used for the evaluation of the transition programme. A 'one-group pretest-posttest, with a non-equivalent posttest-only comparison group design' is used to quantitatively evaluate the impact of the transition programme, ap

IMPORTANCE: A growing body of evidence suggests that adverse pregnancy outcomes (APOs), including hypertensive disorders of pregnancy, gestational diabetes (GD), preterm birth, and intrauterine growth restriction, are associated with increased risk of cardiometabolic disease and cardiovascular disease (CVD) later in life. Adverse pregnancy outcomes may therefore represent an opportunity to intervene to prevent or delay onset of CVD. The objective of this review was to summarize the current evidence for targeted postpartum interventions and strategies to reduce CVD risk in women with a history

BACKGROUND: An adolescent with a chronic condition must prepare for transition from the pediatric to the adult health care system. Ideally, transition is a purposeful and coordinated process between the two systems. We sought to evaluate a pediatric rheumatology transition clinic from the perspective of the young adults who attended the clinic.
METHODS: Young adults who attended the IWK Health Centre Pediatric Rheumatology Transition Clinic in Halifax, Nova Scotia, Canada were asked to complete a mail questionnaire. In this clinic an adult rheumatologist joins the pediatric team for the patie
