We present a patient with a history of lung transplantation who subsequently underwent dual heart-kidney transplantation for nonischemic cardiomyopathy and chronic kidney disease, becoming one of the rare cases of triple-organ transplantation. This case underscores the evolving challenges and successes in managing complex transplant recipients.

Mature T-cell non-Hodgkin lymphomas (T-cell NHL) are a heterogeneous group of lymphoid malignancies including NK/T-cell lymphomas. Hematopoietic cell transplantation (HCT) is an important component of the management of T-cell NHL; however, the optimal timing and type of transplant for each different subtype is an ongoing debate. For the purpose of this chapter, PTCL will be classified as (1) systemic PTCL that includes nodal as well as non-nodal histologies in PTCL (2) CTCL-or cutaneous T-cell lymphomas that arise primarily in the skin and (3) NK/T-cell lymphomas both nasal and extranasal type

BACKGROUND: Iron overload, indicated by elevated ferritin, is a potentially modifiable risk factor in allogeneic hematopoietic cell transplantation (allo-HCT); however, its prognostic value remains uncertain.
OBJECTIVES: We evaluated whether pre-transplant ferritin (pre-TF) levels are associated with allo-HCT outcomes, including overall survival (OS), disease-free survival (DFS), relapse incidence, and GVHD incidence.
STUDY DESIGN: We retrospectively analyzed 682 allo-HCT recipients transplanted between 2019 and 2022, with a median follow-up of 37 months. Pre-TF was measured at a median of 1

CAR-T cell therapy revolutioned the therapeutic landscape of the relapsed/refractory B-cell acute lymphoblastic leukemia (ALL-R/R), improving relapse-free survival and overall survival of patients. Optimal management of late complications, such as hematological toxicity as well as allogeneic transplantation role in the therapeutic sequence, are specific topics currently being debated. We report a case of a young woman with ALL-B R/R extramedullary relapsed after a first allogeneic transplant and second-line immunotherapy, successfully treated with brexu-cel obtaining a complete remission statu

In this review, we describe the major milestones in the development of organ transplantation with a specific focus on hepatic transplantation. For many years, the barriers preventing successful organ transplantation in humans seemed insurmountable. Although advances in surgical technique provided the technical ability to perform organ transplantation, limited understanding of immunology prevented successful organ transplantation. The breakthrough to success was the result of several significant discoveries between 1950 and 1980 involving improved surgical techniques, the development of effecti

BACKGROUND: Chronic kidney disease among prior heart transplant recipients is a growing problem that is likely to place an increased demand on a limited supply of kidney allografts. Allocation of the limited resource of kidneys for transplantation requires consideration of the demands of fair distribution and optimizing patient and graft survival. The aim of this study was to compare the kidney transplant outcomes among recipients of kidney after prior heart transplantation (KAH, n=456) with kidney transplantation in other clinical settings.
METHODS: A retrospective cohort study using United

Biliary cirrhosis complicates some adults with cystic fibrosis (CF) and may require transplantation. Cardio-respiratory disease severity varies such that patients may require liver transplantation, heart/lung/liver (triple) grafts or may be too ill for any procedure. A 15-year experience of adults with CF-related liver disease referred for liver transplantation is presented with patient survival as outcome. Twelve patients were listed for triple grafting. Four died of respiratory disease after prolonged waits (4-171 weeks). Eight underwent transplantation (median wait 62 weeks); 5-year actuari

Post-transplant lymphoproliferative disease (PTLD) is a well recognized complication of solid organ transplantation and therapeutic immunosuppression, first reported in 1968. PTLD incorporates a spectrum of abnormalities ranging from a benign infectious mononucleosis-like illness to non-Hodgkin's lymphoma with nodal and extranodal site involvement. The first liver transplant was performed at our institution in January 1982. This retrospective study examined the incidence of PTLD, reason for the original transplants, presenting symptoms, radiological findings, immunosuppression regimens and out…

Cardiopulmonary transplantation (CPT) is indicated for patients eligible for heart transplantation (HT) or lung transplantation (LT) who have severe concomitant lung or heart disease. Only 2 groups perform CPT in Spain. We report our experience with 18 CPTs representing 8.2% and 5% compared with LT (220) and HT (362), respectively, from February 13, 1990 to October 15, 2002. The mean time on a waiting list was 138 days. The current number of surviving patients is 7 (39%), with a mean follow-up of 602 days (range, 3 to 4627 days). They all remain asymptomatic with normal respiratory function in

The authors give a short overview about the background of the organisation of the liver transplantation and about the process of the "liver alarm". One of the most important work of the coordinator is to increase the number of organ donation. The other part of the preparation of liver transplantation is to create the recipient waiting list. The organisation of the "liver alarm" needs the work of two coordinators. The donor coordinator keeps contact with the donor hospital, harmonizes the travel of different transplant teams and organizes the organ explantation. In the same time, the recipient

Kidney transplantation offers individuals with end stage kidney disease the best chance for long-term survival without dialysis and at the lowest cost to the health care system. However, kidney transplantation comes with risks and complications. These complications can be categorized as short-term, long-term, infectious, and noninfectious. Short-term complications can occur immediately after transplant and for up to six months post-transplant. These complications can involve delayed graft function, infection, rejection, and technical surgical causes. Long-term complications occur six months an

BACKGROUND: Cadaveric organ splitting emerged from an improved understanding of the surgical anatomy of the liver as a possible mechanism to expand the organ pool. In this study, we have reported our first series of split liver transplantations (SLT).
MATERIALS AND METHODS: From June 2006 to June 2008, we performed 17 pairs of SLT: 70.6% ex situ and 29.4% in situ. The mean age of the donors (32 males, 2 females) was 23.15 +/- 9 years. All of them had been stable at the time of harvest according to vital signs, liver function tests, electrolytes, and urine output. The decision on splitting was

BACKGROUND: The potential of Tocilizumab (TCZ) in preventing the cytokine storm caused by COVID-19 infection has been observed, while the survival benefits were inconclusive in solid-organ transplant recipients. We aimed to explore whether the timing of TCZ administration holds significance in the clinical course of COVID-19 infection and identify predicative factors of TCZ efficacy.
METHODS: We conducted a prospective cohort study between December 2022, and January 2023. Early TCZ use referred to administration within 6 days after symptoms onset, while late TCZ use indicated administration a

Antiparkinsonian carotid body (CB) cell therapy has been proven to be effective in rodent and nonhuman primate models of Parkinson's disease … ts induced by in vitro-expanded CB cells were similar to those previously reported by the use of CB transplants. This action could be explained because stem-cell-derived CB neurospheres produce similar amounts of GDNF compared to native CB tissue. This study provides the first evidence that in vitro-expanded CB cells could be a clinical option for cell therap …
