Lessons learned from isolated blunt major pancreatic injury: Surgical experience in one trauma centre.

INTRODUCTION: The aim of this study was to present our surgical experience of isolated blunt major pancreatic injury (IBMPI), and to compare its characteristic outcomes with that of multi-organ injury. MATERIALS AND METHODS: From 1994-2015, 31 patients with IBMPI and 54 patients with multi-organ injury, who underwent surgery, were retrospectively studied. RESULTS: Of the 31 patients with IBMPI, 22 were male and 9 were female. The median age was 30 years (interquartile range, 20-38). Twenty-one patients were classified as the American Association for the Surgery of Trauma-Organ Injury Scale G

Clinical criteria for obtaining maxillofacial computed tomographic scans in trauma patients.

BACKGROUND: Over 150,000 patients present with maxillofacial trauma annually to emergency rooms in the United States. Although maxillofacial computed tomography is a sensitive screening tool for identifying facial fractures, indiscriminate use leads to unnecessary radiation exposure and substantial costs. A decision instrument is needed to ensure computed tomographic evaluation of patients at high risk for facial fracture and limit computed tomography use in low-risk patients. METHODS: A retrospective review was conducted of all patients evaluated at a Level I trauma center over a 3-year peri

Outcome of cervical near-hanging injuries.

BACKGROUND: Cervical near-hangings are not rare, but have received little attention in the trauma literature. Increasing numbers of patients received from our local jail and detention centers prompted this study. METHODS: Seventeen-year review of a level I Trauma Center Registry identified 67 patients with cervical strangulation for study. Data were analyzed using the Mann-Whitney test to evaluate continuous predictors, and Fisher's exact test for categorical predictors. RESULTS: Ten of 67 patients died (14.9% mortality). Patients having a lower Glasgow Coma Score (GCS) at the scene (3.5 +/-

[Fatal outcome after multiple trauma. The thoracic injury as the decisive factor].

In patients suffering from multiple injury, chest trauma is often the main cause of fatality. A case report is given and the literature reviewed.A 49 years old motorcyclist hit a car frontally in a road accident. After primary stabilization and first clinical care, he was transferred to our trauma centre because of severe chest injury, suspected pericardial effusion and lesion of the thoracic aorta. Initial diagnostics (plain radiographs, CT scan of thorax, abdominal ultrasound, echocardiography) showed left-sided serial rib fractures, a fracture of the left scapula, a hematopneumothorax left-

Airbag protection versus compartment intrusion effect determines the pattern of injuries in multiple trauma motor vehicle crashes.

OBJECTIVE: A prospective study of the interaction between airbag (AB) and seat-belt (Bt) protection versus vehicular compartment (VC) intrusion effects on injury patterns in motor vehicle crash (MVC) trauma patients. METHODS: Two hundred MVC patients, nonejected drivers or front seat passengers with multiple trauma or severe lower extremity (LE) trauma admitted to two Level I trauma centers. RESULTS: In frontal crashes, airbags (AB) more than Bt reduced Glasgow Coma Scale severity in brain injury, face fracture, shock, and the need for MVC extrication (all p < 0.05). Frontal AB also had a pr…

Outcomes for Initially Hemodynamically Stable Pediatric Patients With Thoracic Trauma Undergoing Emergency Department Thoracotomy.

BACKGROUND: Resuscitative thoracotomy, performed in only 10%-15% of pediatric thoracic trauma cases, yields a dismally low survival rate of 3.4%. As such, an investigation into the mechanisms of such high mortality associated with this emergent procedure is warranted. OBJECTIVE: This study aims to evaluate the clinical outcomes of initially hemodynamically stable pediatric patients requiring an emergency department thoracotomy (EDT) at pediatric trauma centers (PTC), combined adult/PTCs (CTC), and adult-only trauma centers (ATC). METHODS: The American College of Surgeons-Trauma Quality Impro…

Obesity is Associated With Mortality and Complications After Trauma: A State-Wide Cohort Study.

BACKGROUND: With rising obesity rates in the United States, knowledge of obesity's impact on trauma outcomes is essential to providing high-quality care. The interaction between body mass and outcomes is unclear, with existing literature demonstrating conflicting results. We hypothesized that in a broad cohort of trauma patients, obesity would be associated with in-hospital mortality. MATERIALS AND METHODS: We conducted a retrospective cohort study using the 2014-2015 Pennsylvania Trauma Outcomes Study (PTOS) registry, a state-wide registry to which all accredited Pennsylvania trauma centers …

An evidence-based approach to antibiotic use in maxillofacial trauma.

A patient with craniomaxillofacial (CMF) trauma often receives consultations from multiple medical and surgical services. Antibiotics are sometimes prescribed without consistent evidence. This article reviews current literature regarding antibiotic prophylaxis of patients with CMF fractures. This article also presents the guidelines for the appropriate use of antibiotics for patients with CMF fractures at 2 level I trauma centers.

A contemporary analysis of the management of the mangled lower extremity.

BACKGROUND: The management of a mangled lower extremity is complex and requires consideration of a patient's injury pattern, medical history, social context, and preference. The Lower Extremity Assessment Project provides the highest level of evidence guiding management; however, the Lower Extremity Assessment Project cohort was recruited 15 years ago and was restricted to Level I trauma centers. Furthermore, as our ability to salvage limbs has improved, the decision to amputate in the early period following injury remains particularly challenging. Given these considerations, our primary objec…

Geriatric Skiers: Active But Still at Risk, a National Trauma Data Bank Study.

BACKGROUND: Downhill skiing accounts for a large portion of geriatric sport-related trauma. We assessed the national burden of geriatric versus nongeriatric ski trauma. MATERIALS AND METHODS: Adults presenting to level 1/2 trauma centers after ski-associated injuries from 2011 to 2015 were identified from the National Trauma Data Bank by ICD-9 code. We compared demographics, injury patterns, and outcomes between geriatric (age ≥65 y) and nongeriatric adult skiers (age 18-64 y). A multiple regression analysis assessed for risk factors associated with severe injury (Injury Severity Score >15). …

Geo-Mapping Using In-Hospital Massive Transfusion Data as a Method for Prehospital Blood Management for Trauma Patients.

BACKGROUND: Prehospital whole blood (PHWB) transfusion improves outcomes in trauma patients, but blood products are a scarce and costly resource. We hypothesized that massive transfusion protocol (MTP) activation could be an indicator for trauma patients who might benefit from PHWB, and we used geo-mapping to identify high need zones. STUDY DESIGN: We retrospectively analyzed trauma registry data from 5 trauma centers in Omaha and Lincoln, Nebraska, including all patients who had MTP activated in the trauma bay from June 1, 2019, to March 31, 2025. Assault and motor vehicle crash data were co…

Hypovolemic shock: an overview.

Resuscitation of major trauma victims suffering from shock remains a challenge for trauma systems and trauma centers. Rapid identification, and ensuring correct, aggressive treatment, are necessary for patient survival. This article discusses shock encountered in trauma victims: hypovolemic, cardiogenic, obstructive, and distributive shock. Emphasis is placed on hypovolemic shock and its sequelae. The critical care nurse plays an important role as part of the team involved in the resuscitation and ongoing care of these patients. Understanding the underlying pathophysiology, recognizing signs a…

Validation of an ICD-9-CM-Based Monitoring Tool for Regional Trauma Systems: The PaTraME Study in Pavia Province, Italy.

Background/Objectives: Continuous trauma-system monitoring is limited by the lack of scalable, low-cost tools. The Pavia Trauma Management Epidemiology (PaTraME) project uses routinely collected ICD-9-CM discharge data (SDO) and the Trauma Mortality Probability Model (TMPM) to derive Injury Severity Score (XISS) and probability of death (TMPM-POD), creating a cost-free surveillance framework for regional trauma networks. Methods: We conducted a retrospective study of all major-trauma admissions (XISS > 15) in Pavia Province from 2014 to 2021. Anonymized SDO records were linked with emergency d

Temporary intravascular shunts: when are we really using them according to the NTDB?

Temporary intravascular shunts (TIVS) are synthetic intraluminal conduits that maintain arterial and/or venous blood flow. This technique ca … n while surgeons assess/fixate the limb. TIVS are placed relatively uncommonly by a large number of trauma centers with a few hospitals using them much more frequently for penetrating injuries.

Trauma patients warrant upper and lower extremity venous duplex ultrasound surveillance.

BACKGROUND: Due to the high incidence of thromboembolic events (deep venous thrombosis [DVT] and pulmonary embolus [PE]) after injury, many trauma centers perform lower extremity surveillance duplex ultrasounds. We hypothesize that trauma patients are at a higher risk of upper extremity DVTs (UEDVTs) than lower extremity DVTs (LEDVTs), and therefore, all extremities should be evaluated. MATERIALS AND METHODS: A retrospective chart and trauma registry review of Intensive Care Unit trauma patients with upper and LEDVTs detected on surveillance duplex ultrasound from January 2010 to December 201…

Universal screening for intimate partner and sexual violence in trauma patients-What about the men? An Eastern Association for the Surgery of Trauma Multicenter Trial.

BACKGROUND: A recent Eastern Association for the Surgery of Trauma-supported multicenter trial demonstrated a similar rate of intimate partner and sexual violence (IPSV) between male and female trauma patients, regardless of mechanism. Our objective was to perform a subgroup analysis of our affected male cohort because this remains an understudied group in the trauma literature. METHODS: We conducted a recent Eastern Association for the Surgery of Trauma-supported, cross-sectional, multicenter trial over one year (March 2015 to April 2016) involving four Level I trauma centers throughout the …
