A 65-year-old male presented with a chief complaint of back pain. Chest computed tomography (CT) demonstrated partially calcified enlargements of the anterior mediastinal lymph nodes and pleural effusion in both lungs. His spondylous lesion rapidly progressed and developed into quadriparesis approximately 2 months after he noticed the back pain. We could not determine whether the lesion was an invasion of the spinal cord by a carcinoma of unknown primary or malignant lymphomas. Therefore, we performed a lymph node biopsy using a thoracoscope for a definitive diagnosis. Histopathology showed tu

The authors report a case of pulmonary tuberculosis appearing after an endobronchial fistula from a Pott's abscess. Such a clinical presentation is rare even in a developing country (PVD), where there is experience of more than 200 cases of tuberculous spondylodiscitis seen over a period of six years. The frequency of paravertebral abscess during the course of Pott's disease is around 52%, in the thoracic lesion their exteriorization by endobronchial fistula is around 4% (5 out of 114). The outcome is generally more simple and here a cure was achieved after six months of chemotherapy.

The authors make an analysis of their many-years experience with the surgical treatment of tuberculosis of bones and joints (1380 operations in children aged from 8 months to 14 years with different manifestations of bone--joint tuberculosis). The context and principles of the surgery of tuberculosis of bones and joints in children as an independent scientific branch are determined. The priority of surgical methods in the treatment of the disease is confirmed and further trends of its development are shown.

Currently the differential diagnosis of anaplastic meningiomas often is quite complex. An interpretation of the data of radiological studies does not allow the exclusion of diseases with similar clinical course and character of a lesion, such as primary lymphoma and tuberculosis of the central nervous system, neurosarcoidosis and others. The article presents a clinical case, representing a rare and difficult to diagnose variant of anaplastic meningioma with extremely aggressive nature of the current and widespread involvement of the brain and spinal cord.

OBJECTIVE: The aim of this study was to investigate the occurrence of paradoxical reaction (PR) in HIV-negative tuberculous meningitis (TBM) patients with spinal involvement, as well as its possible risk factors.
METHODS: Fifty TBM patients with spinal involvement were studied retrospectively and divided into a PR group and a non-PR group according to the presence of PR. Their demographic, clinical, radiological, and laboratory data, and status at follow-up were collected and compared.
RESULTS: PR developed in 26 patients (52%), with the median time to the development of PR being 30days (ran

Disseminated osteoarticular TB is uncommon, and the occurrence of multidrug-resistant (MDR) strains impacting many skeletal locations in an immuno-competent individual is very rare. A 25-year-old male from a TB-endemic rural region is presented, having a three-month progression of worsening low back pain, subsequently accompanied by lower limb weakness. GeneXpert from both sites detected Mycobacterium tuberculosis that was resistant to rifampicin. The line probe assay detected resistance to rifampicin, high-level resistance to isoniazid, and resistance to ethambutol. After surgical procedures

Tuberculosis (TB) is a preventable and usually curable disease but remains a major health problem worldwide, particularly in developing countries. TB of the lumbosacral junction is rare and occurs in only 1-2% of all cases of spinal TB. Moreover, isolated sacrococcygeal TB is extremely rare. We present a case of a 64-year-old patient with a history of diabetes who presented with chronic back pain and cough. Physical examinations revealed a perianal fistula and left elbow joint arthritis. The patient is diagnosed with disseminated TB with coccygeal involvement. Diagnosis was achieved non-invasi

A 56-year-old woman was admitted to our hospital, on October 10, 1999 because of tingling numbness of the Th3 area and paraparesis. Her medical history included the diagnosis of pulmonary sarcoidosis which had responded to steroid therapy. On radiographic examination, a Th2 compression fracture and gadolinium enhanced areas were shown in MRI. We diagnosed vertebral sarcoidosis and restarted steroid therapy. Next day, she had no paresis and she was discharged after 20 days. However, the patient returned to our hospital with recurrence of the same pain and paraparesis on January 1, 2000, and spi

Imaging as an essential tool for diagnosis and follow-up of pulmonary tuberculosis. Standard radiography is of prime importance, while in a large number of cases computed tomography (CT) and in selected exceptional cases with spinal involvement, magnetic resonance imaging (MRI) offer further precisions. CT is particularly important for diagnosis because of a better spatial visualization of the lesions and a better evaluation of their structure, both in the parenchyma and in the mediastinum; this indication is especially important in black Africans and in AIDS patients. CT can also be used to g

STUDY DESIGN: Case report and review of the literature.
OBJECTIVE: To report 2 patients, with a history of thoracic tuberculosis, in whom ossification of ligamentum flavum (OLF) was found adjacent to progressive kyphosis and resulted in incomplete paralysis. The relevant literature was also to be reviewed.
SUMMARY OF BACKGROUND DATA: Progressive kyphosis is the most common cause of late onset paralysis after spinal tuberculosis. However, OLF related to tuberculous kyphosis is rarely observed and this makes surgical treatment become very difficult.
METHODS: Clinical history, laboratory exami

We report a rare case of a rigid spinal deformity with severe pelvic obliquity (PO) resulting from hip ankylosis caused by childhood tuberculosis (TB). A 66-year-old woman presented with left knee pain, chronic low back pain, and fatigability during walking. She presented with leg length discrepancy (LLD) due to an ankylosed right hip joint, severe PO, and secondary lumbar scoliosis. Total hip arthroplasty (THA) and adductor tendonectomy were performed prior to spine surgery, and posterior spinal correction and fusion were performed from T10 to the pelvis. Prior to spinal correction surgery, w

An immunocompetent man in his 40s presented with 3 months of mid-thoracic back pain which progressed to include progressive paraesthesias and lower extremity weakness. Investigations revealed thoracic spine osteomyelitis with signs of cord compression. He underwent neurosurgical intervention, including laminectomy, spinal cord decompression and partial resection of an epidural mass. Initial intraoperative biopsy and surgical pathology results were concerning for an acid-fast bacillus as the causative pathogen, and the patient was given empiric therapy for presumed Mycobacterium tuberculosis Ho

Musculoskeletal tuberculosis is known for its ability to present in various forms and guises at different sites. Tubercular spinal epidural abscess (SEA) is an uncommon infectious entity. Its presence without associated osseous involvement may be considered an extremely rare scenario. We present a rare case of tubercular SEA in an immune-competent 35-year-old male patient. The patient presented with acute cauda equina syndrome and was shown to have multisegmental SEA extending from D5 to S2 vertebral level without any evidence of vertebral involvement on MRI. The patient made an uneventful rec
