Intermittent self-catheterisation.

Intermittent self-catheterisation is a way for patients to manage a poorly emptying bladder with less risk of infection than is found with long-term indwelling catheters. Rachel Barton discusses the principles, how to identify suitable candidates and complications.

Prolonged catheterization after vaginal prolapse surgery.

OBJECTIVE: To evaluate the domestic use of a transurethral Foley catheter after discharge from the hospital in patients with urinary retention after vaginal prolapse surgery. METHODS: We reviewed 27 patients who used a Foley catheter at home after prolapse surgery. All patients were interviewed by postal questionnaire regarding their experience of this procedure. RESULTS: Of 263 vaginal prolapse operations 27 (11%) needed a prolonged catheterization period at home. In all but two patients micturition was successfully restored after removal of the catheter at a median of 13 days after dischar

Problems of indwelling Foley catheters.

One hundred patients undergoing urinary bladder catheterization for various reasons were studied. 42 patients received Sewoon catheters of various sizes. 38 received Eschmann catheters of various sizes and 20 received Bard catheters of various sizes. The duration of catheterization ranged from a few hours to 21 days. Various problems associated with catheterization were recorded whenever encountered. Stuck catheter was found to occur in 15 of the 40 patients who received Sewoon catheters. There were no stuck catheters recorded for Eschmann or Bard catheters. Infections occurred in 9 out of 100

Neurogenic bladder pathophysiology, assessment and management after lumbar diseases.

Neurogenic bladder (NB) is a group of bladder and/or urethral dysfunctions caused by neurological lesions, commonly seen in patients with lumbar spine diseases, manifesting as urinary storage and voiding dysfunction, significantly affecting patients' quality of life. Degenerative changes or trauma to the lumbar spine can lead to narrowing of the dural sac, compressing the sacral nerve roots, cauda equina or blood vessels, causing bladder dysfunction and leading to NB. Diagnostic methods for NB include history taking, physical examination and noninvasive and invasive tests, such as urodynamic t

Salmonella prostatitis in a man with spinal cord injury.

CONTEXT: Prostatitis is a very unusual manifestation of Salmonella urinary tract infection and has not been reported in men with spinal cord injury (SCI). FINDINGS: A 57-year-old man with paraplegia and a history of recurrent symptomatic urinary tract infections presented with Salmonella typhimurium prostatitis. Clinical and sonographic examination of the urinary tract, as well as urinalysis including microbiologic examination, revealed no relevant abnormalities. The microbiologic analysis of the ejaculate revealed growth of monophasic Salmonella enterica ssp. enterica serotype 4,12:i:-. A 6-

Correlation of catheterized and clean catch urine protein/creatinine ratios in preeclampsia evaluation.

OBJECTIVE: To examine whether clean catch urine specimens correlate with catheterized specimens for determination of protein/creatinine ratios in pregnant women being evaluated for preeclampsia. METHODS: Sixty pregnant women who were at least at 20 weeks of gestation were enrolled. Patients with ruptured membranes, vaginal bleeding, or urinary tract infections were excluded. Midstream clean catch urine specimens were collected. Catheterized specimens were then collected and used for clinical management. The specimens were analyzed for protein, creatinine, urinalysis, and culture. Based on sam

Techniques to create continence in the failed bladder exstrophy closure patient.

We reviewed retrospectively 315 patients with bladder exstrophy treated at our hospital between July 1976 and April 1992 to assess the outcome of those who failed primary closure of the bladder. Of the patients 47 required reclosure of the bladder, including 28 who have undergone a procedure to restore urinary continence. Methods used to achieve dryness included bladder neck reconstruction in 18 patients, bladder neck reconstruction along with augmentation in 4, augmentation alone in 4, repeat bladder neck reconstruction in 1, and reclosure with creation of a continent stoma and augmentation i

Intermittent Self-catheterization for Bladder Dysfunction After Deep Endometriosis Surgery: Duration and Factors that Might Affect the Recovery Process.

STUDY OBJECTIVE: To assess the duration needed for regaining normal bladder voiding function in patients with postoperative bladder dysfunction requiring intermittent self-catheterization after deep endometriosis surgery and identify risk factors that might affect the recovery process. DESIGN: Retrospective study based on data recorded in a large prospective database. SETTING: Endometriosis referral center. PATIENTS: From September 2018 to June 2022, 1900 patients underwent excision of deep endometriosis in our center; 61 patients were discharged with recommendation for intermittent self-ca

Is It Really the Foley? A Systematic Review of Bladder Management and Infection Risk.

BACKGROUND: The belief that intermittent catheterization results in fewer infections than indwelling catheters is commonly expressed in the spinal cord injury literature. Some practice guidelines strongly recommend intermittent over indwelling catheterization due to concerns about infections and other complications. However, studies on this topic are of low quality. Guidelines from the Consortium for Spinal Cord Medicine suggest the data regarding infection risk are mixed, and they do not recommend one bladder management method over the other. OBJECTIVES: To compare risk of bias in studies re

[Intrauretheral devices and infection : common beliefs and scientific evidence].

Intrauretheral catheters, used for drainage of the upper urinary tract, are associated with mechanical and infectious complications. The risk of infection is associated with the duration of catheterization, the presence of comorbidities, and the clinical context. Microbial colonization of synthetic material, via the formation of a biofilm, is a known and expected phenomenon that is most often clinically insignificant but may be associated with an increased risk of infection, even though a direct causal link between colonization and symptomatic infection has not been established. The management

Drug and Vaccine Development for the Treatment and Prevention of Urinary Tract Infections.

Urinary tract infections (UTI) are among the most common bacterial infections in humans, affecting millions of people every year. UTI cause significant morbidity in women throughout their lifespan, in infant boys, in older men, in individuals with underlying urinary tract abnormalities, and in those that require long-term urethral catheterization, such as patients with spinal cord injuries or incapacitated individuals living in nursing homes. Serious sequelae include frequent recurrences, pyelonephritis with sepsis, renal damage in young children, pre-term birth, and complications of frequent

Parameters affecting urologic complications after major joint replacement surgery.

INTRODUCTION AND OBJECTIVES: Peri-operative bladder management after major arthroplasty procedures remains controversial. The purpose of this study was to assess the risk of urological complications in those patients undergoing hip or knee joint replacement. As well, we identified those factors that may affect the likelihood of developing complications. METHODS: Two hundred and twenty-one consecutive patients receiving a total knee or hip arthroplasty were reviewed. The outcomes measured were prolonged urinary retention, as well as urinary tract infections and the development of a septic pros
