Laser-assisted uvulopalatoplasty: evaluation of postoperative discomfort and the effectiveness of this procedure.

BACKGROUND AND OBJECTIVE: The Beckman Laser Institute and Medical Clinic of the University of California at Irvine began to offer laser-assisted uvulopalatoplasty to treat snoring in October 1993. Current literature at that time advised that patients should expect mild or minimal pain after each treatment. Soon, it became apparent that those instructions needed modifying as most of the patients complained of severe pain. STUDY DESIGN/MATERIALS AND METHODS: Patient surveys were developed to evaluate postoperative pain, pain management, the rate of improvement after each treatment, and the succ

Uvulectomy in the office setting.

Snoring is a common problem which affects 50% of male and 30% of female patients. In a small number of patients, partial uvulopalatoplasty in the office setting has been found to ameliorate loud snoring. In our study, 28 patients had uvulectomy for snoring, 17 of whom responded to our survey. The procedure was somewhat beneficial (rated 3.2 on a scale of 10, with 10 representing no snoring) and had little impact on quality of life (rated 2.7 on a scale of 10, with 10 representing great improvement). After the procedure patients had considerable pain (rated 7.4 on a scale of 10, with 10 represe

Clinical characteristics of acute uvulitis.

Previous literature has identified uvulitis as a condition with serious implications. This report details a series of 15 patients with uvulitis whose clinical features were remarkable for a relatively benign course. This is the largest series of patients with uvulitis in the literature. The predominant symptoms were related to sore throat and pain or difficulty on swallowing. No patient had significant airway or infectious complications attributable to the uvulitis. The only patient admitted to the hospital required management of an associated peritonsillar abscess. Of the 14 patients discharg

Management of total nasopharyngeal stenosis following UPPP.

UPPP has emerged over the past decade as a surgical procedure to improve or possibly eliminate Obstructive Sleep Apnea. The procedure itself is not uniformly successful and the technique continues to undergo evolution. Most complications occur in the acute post-operative period and are not unusual. Late complications are seen much less frequently: less than 100 cases of severe nasopharyngeal stenosis have been reported. Several authors have previously identified this problem and have published "avoidance strategies" and we recommend that the UPPP surgeon be well aware of these pitfalls and the

Effect of uvulopalatopharyngoplasty on upper airway collapsibility in obstructive sleep apnea.

Previous investigators have demonstrated variable responses to uvulopalatopharyngoplasty (UPP) in patients with obstructive sleep apnea. We hypothesized that this variability is due to either (1) differences in baseline pharyngeal collapsibility preoperatively or (2) differences in magnitude of the decrease in pharyngeal collapsibility resulting from surgery. To determine the relationship between changes in collapsibility and the response to UPP surgery, we measured the upper airway critical pressure (Pcrit) before and after UPP in 13 patients with obstructive sleep apnea. During non-REM sleep

Response to CPAP and UPPP in apnea.

Ninety-two consecutive patients with obstructive sleep apnea syndrome (OSAS) were studied before and six weeks after treatment with either nasal continuous positive airway pressure (CPAP) or uvulopalatopharyngoplasty (UPPP) (n = 46 per group). Assignment of patients to treatment was based on clinical considerations and patient preference. Patients were assessed by nocturnal polysomnography and performance on the Multiple Sleep Latency Test (MSLT) the following day. Before treatment, the CPAP and UPPP groups did not differ in sleep-related respiratory disturbance, oxygenation during sleep, frag

Laser CO2 for snoring. Preliminary results.

CO2 Laser Vaporizations of the PalatoPharynx (LVPP), is a new technique, safe and effective, ambulant, by local anesthetic spray, performed for the treatment of snoring. The LVPP is based on the progressive enlargement of the airspace in the oropharynx, to eliminate or reduce obstructions that occurs during sleep, by successive "vaporizations" (or laser-strip carbonization) of the vibrating soft palate, wide posterior tonsil pillars and redundant posterior pharyngeal mucosa. From December 1988 to July 1990, 31 snorers have been treated by LVPP and have been cured or, at least, improved in thei

Localization of upper airway collapse during sleep in patients with obstructive sleep apnea.

The present study was conducted to determine the effects of body position and sleep state, as well as the effect of uvulopalatopharyngoplasty (UPPP) on the regions over which the upper airway (UA) collapses during sleep. To accomplish this goal, 18 male patients with obstructive sleep apnea (OSA) underwent overnight polysomnography with simultaneous monitoring of pressures in the posterior nasopharynx, oropharynx, hypopharynx, and esophagus. From the profile of pressures recorded in the UA and esophagus, the regions over which the UA collapses during apneas could be determined. The patients we

[Treatment of obstructive sleep apnea with methods of orthognathic surgery].

The only surgical means available up to now for treating obstructive sleep apnea syndrome (OSAS) consisted in uvulopalatopharyngoplasty (UPPP), which failed to produce satisfactory results. The ultima ratio frequently resorted to was tracheotomy with all its medical and social drawbacks. Complete advancement of the maxillomandibular complex opens up new avenues for the treatment of upper airways obstruction. The present report is on 13 patients operated for polysomnographically established OSA. The maxillomandibular complex was brought as far forward as possible and esthetically justifiable. T

Somnofluoroscopy: its role in the selection of candidates for uvulopalatopharyngoplasty.

The purpose of this study was to evaluate the efficacy of somnofluoroscopy in the selection of candidates for uvulopalatopharyngoplasty (UPPP). Somnofluoroscopy is a lateral fluoroscopic examination of the upper airway with synchronous polysomnography that provides information about the dynamic function of the airway and the level of stenosis or occlusion during sleep. Somnofluoroscopies were performed preoperatively in 26 consecutive patients undergoing UPPP. Preoperative and postoperative polysomnographic data were obtained on each patient. On review of the somnofluoroscopic recordings, two

Estimation of the Young's moduli of fresh human oropharyngeal soft tissues using indentation testing.

Finite element (FE)-based biomechanical simulations of the upper airway are promising computational tools to study abnormal upper airway deformations under obstructive sleep apnea (OSA) conditions and to help guide minimally invasive surgical interventions in case of upper airway collapse. To this end, passive biomechanical properties of the upper airway tissues, especially oropharyngeal soft tissues, are indispensable. This research aimed at characterizing the linear elastic mechanical properties of the oropharyngeal soft tissues including palatine tonsil, soft palate, uvula, and tongue base.…
