Surgical treatment of subaortic stenosis after biventricular repair of double-outlet right ventricle.

Out of 180 patients who underwent biventricular repair of double-outlet right ventricle between 1980 and 1995, 9 (5%) required reoperation because of subaortic stenosis. Two other patients who initially underwent operation elsewhere underwent reoperation at our institution because of subaortic stenosis. The median age at biventricular repair was 4 months. Repair consisted of tunnel construction from the left ventricle to the aorta in nine patients; the remaining two patients received an arterial switch operation with ventricular septal defect closure. Subaortic stenosis developed with time: th

Incidental Murmurs and the Elective Noncardiac Surgical Patient: A Case Report.

We present a case of left ventricular outflow tract (LVOT) obstruction detected by limited bedside transthoracic echocardiography (TTE). This involved a young and otherwise healthy patient presenting for elective hand surgery with a previously undetected cardiac murmur. It highlights the utility of bedside TTE as an assessment tool and shows the importance of anesthesiologists as perioperative physicians.

A rare case of biventricular non-compaction associated with ventricular septal defect and descendent aortic stenosis in a young man.

Non-compaction of the ventricular myocardium is a cardiomyopathy caused by the arrest of normal embryogenesis of the ventricles. It is classified in isolated non-compaction of the ventricles (most frequently of the left one) and in ventricular non-compaction associated with other congenital anomalies of the endocardium and myocardium, such as obstruction of the right or left ventricular outflow tracts, complex cyanotic congenital heart disease, and coronary artery anomalies. There are controversies regarding the right ventricle non-compaction due to the normally trabeculated shape of its walls

Assessment of myocardial function in obstructive hypertrophic cardiomyopathy cats with and without response to medical treatment by carvedilol.

BACKGROUND: Inconsistency of treatment response in cats with obstructive hypertrophic cardiomyopathy is well recognized. We hypothesized that the difference in response to beta-blockers may be caused by myocardial functional abnormalities. This study was designed to compare myocardial function in cats with obstructive hypertrophic cardiomyopathy with and without response to beta-blockers. Twenty-one, client-owned, hypertrophic cardiomyopathy cats treated with carvedilol were analyzed. After carvedilol treatment, cats with decreased left ventricular outflow tract velocity were categorized as re

Pulmonary stenosis in infants and young children.

Twenty-four patients less than 3 years old underwent operation for pulmonary stenosis. Pulmonary dysplasia was diagnosed preoperatively in only 4 patients; in 20 patients the lesion was categorized simply as pulmonary stenosis. At operation, more severe valve deformities were often present in patients less than 2 years of age. Preoperative evaluation did not reveal the extent of the deformity in 7 additional patients. The deformities included not only valvular dysplasia (thickened redundant valve cusps) but also supravalvular and annular abnormalities. Relief of obstruction was obtained only w

Use of three-dimensional echocardiography for analysis of outflow obstruction in congenital heart disease.

To evaluate the feasibility and accuracy of 3-dimensional (3D) echocardiography in analysis of left and right ventricular outflow tract (LVOT and RVOT) obstruction, 3D echocardiography was performed in 28 patients (age 4 months to 36 years) with outflow tract pathology. Type of lesion and relation to valves were assessed. Length and degree of obstruction were measured. Three-D data sets were adequate for reconstruction in 25 of 28 patients; 47 reconstructions were made. In 13 patients with LVOT obstruction, 3D echocardiography was used to study subvalvular details in 8, valvular in 13, and sup

Echocardiographic Brockenbrough-Braunwald-Morrow sign.

The Brockenbrough-Braunwald-Morrow sign is the paradoxical decrease in pulse pressure during the post-extrasystole beat seen in patients with hypertrophic obstructive cardiomyopathy. We present a case of intermittent left ventricular outflow tract obstruction and secondary mitral regurgitation resulting from post-extrasystolic potentiation following a premature atrial beat, demonstrating using echocardiography the mechanism behind this sign.

Operative treatment of congenital aortic stenosis.

The operative treatment of 131 patients with congenital aortic stenosis is reviewed. Of the 131 patients, 77% had left ventricular outflow tract (LVOT) obstruction at a single level and 23%, major obstruction at more than one level. There were 3 operative deaths (2.3%) and 10 late deaths (7.8%). Twenty of the 128 discharged patients have undergone a second procedure and 6 a third procedure for recurrent or residual LVOT obstruction. The 26 reoperations included 7 aortic valve replacements, 4 left ventricular apical-abdominal aortic (LV-AA) valved conduits, and 15 extensive aortic valvotomies w

Discrete membranous subaortic stenosis. Report of 31 patients, review of the literature, and delineation of management.

The presentation, management, and follow-up of 31 patients with discrete membranous subaortic stenosis (DMSS) is presented. DMSS comprised 16% of 185 patients with congenital left ventricular (LV) obstruction. Only one patient was older than 40 years. The rarity of DMSS in older patients in both our population and in the literature is noted, and possible explanations are discussed. One-quarter of these patients had dyspnea, chest pain, or syncope combined with electrocardiographic left ventricular hypertrophy (LVH) and strain, and these all had peak LV outflow gradients (PSG) greater than 85 m

Pacing in obstructive hypertrophic cardiomyopathy: a therapeutic option?

Hypertrophic cardiomyopathy (HCM) is a heterogeneous disease of cardiac muscle which can present with myriad functional and clinical manifestations. When symptoms and left ventricular outflow gradients are present, it is primarily treated with pharmacologic agents. For refractory patients, dual chamber pacing has been proposed; by altering timing and site of cardiac electrical activation, the hemodynamic abnormalities in HCM may be modified. Results of non-randomized and randomized trials have shown an average gradient reduction of 50%. However, pressure gradient reduction within the left vent

Mavacamten-A Targeted Therapy for Hypertrophic Cardiomyopathy.

The pathophysiology of hypertrophic cardiomyopathy is primarily comprised of dynamic left ventricular outflow tract obstruction, mitral regurgitation, and diastolic dysfunction. Symptoms such as dyspnea, angina, or syncope can occur because of left ventricular (LV) hypertrophy and reduced LV cavity size. Currently, focus on symptom relief through optimizing LV preload and reducing inotropy is the mainstay of therapy through the use of β-blockers, nondihydropyridine calcium channel blockers, and disopyramide. Mavacamten is a novel cardiac myosin inhibitor recently approved by the Food and Drug

Negative right ventricular diastolic pressure after operation for pulmonary valve stenosis: the phenomenon and its relation to volume load and conal contraction.

The phenomenon of negative right ventricular diastolic pressure immediately after operation for relief of isolated pulmonary valve stenosis was investigated in 11 patients. Pressures in the right ventricle and pulmonary artery were measured with a catheter tip micromanometer. One patient had a negative right ventricular diastolic pressure before operation. At the end of operation right ventricular diastolic pressure was negative in all 11 patients. The greater the right ventricular hypertrophy, the lower were these diastolic pressures. Negative right ventricular diastolic pressure is thus comm

Permanent Left Ventricular Assist Devices: Long-Term Management and Outcomes.

This article outlines the historical development, physiologic principles, and contemporary clinical management of durable left ventricular assist devices, with a primary focus on the HeartMate 3 era. An understanding of centrifugal pump physiology, pulsatility index, power, and flow estimation is essential for effective long-term patient management. While major complications such as pump thrombosis and stroke have declined, gastrointestinal bleeding, right ventricular failure, infection, arrhythmias, and outflow graft obstruction remain a significant clinical challenge. Nonetheless, left ventr

Cor triatriatum: an unusual cause of elevated pulmonary capillary wedge pressure in a child with tetralogy of Fallot.

The coexistence of cor triatriatum and tetralogy of Fallot (TOF) is rare. Preoperative identification of cor triatriatum may be difficult owing to reduced pulmonary blood flow in patients with TOF. However, it is imperative to identify this rare combination as failure to identify obstruction to pulmonary venous egress may result in persistent pulmonary venous hypertension postoperatively. The authors discuss hemodynamic aspects of this rare coexistence in a 14-month-old child in whom pulmonary capillary wedge pressure was elevated despite right ventricular outflow obstruction.

[Hypertrophic cardiomyopathy treated by interventional technique of septal reduction].

A 75-year old woman with obstructive hypertrophic cardiomyopathy and class IV dyspnea refractory to medical management had relative contraindications for both pacemaker implantation and surgical approach. Percutaneous intervention was devised to perform the selective injection of absolute alcohol in the first septal branch of the left anterior descending coronary artery. This led to a limited septal infarction documented by ST elevation, enzymatic elevation and the appearance of complete right bundle branch block on the electrocardiogram. A baseline left ventricular outflow tract pressure grad

Results of right ventricular outflow tract stenting as a primary palliation in symptomatic children having Fallot type of lesion.

BACKGROUND: In children with tetralogy of Fallot and associated right ventricular outflow tract obstruction, right ventricular outflow tract stenting has emerged as a viable alternative to surgical shunting, particularly in high-risk patients who are unsuitable for early definitive repair. METHODS: This prospective and retrospective observational study involved 55 symptomatic children with Fallot-type physiology who underwent right ventricular outflow tract stenting over a 30-month period at a tertiary-care centre in India. Data from pre-procedural imaging, intraoperative parameters, and post
