BACKGROUND: The posterior inferior cerebellar arteries (PICA) arise from the intracranial segments of the vertebral artery (VA). We report a case where a nondominant isolated vertebral artery, which terminated in PICA, was stenotic. This resulted in brainstem-lower cerebellar ischemia, corrected with balloon angioplasty.
CASE DESCRIPTION: A 62-year-old male presented primarily with transient vertigo, syncope, and dizziness and was diagnosed with transient ischemic attack. Angiography of the left vertebral artery (VA) demonstrated a small-caliber vessel terminating in PICA with a 90% stenosis …

The benign paroxysmal positional vertigo (BPPV) is a common vestibular end organ disorder. BPPV may be idiopathic or secondary to head traumas, viral labyrinthitis, otology surgery and many others conditions. We report two cases of BPPV secondary to surgical maneuvers on superior maxillary. We have not found this etiology in scientific literature but, the increase of this kind of surgery, can produce an increase of its frequency. For this reason the otoneurologists and oral surgeons must bear in mind this possibility to make a right diagnostic and treatment of the syndrome.

We report a case of sudden bilateral hearing loss associated with an occlusion of the right vertebral artery. A 66-year-old man was admitted to hospital suffering from sudden onset vertigo, deafness, and vomiting. He could not walk due to truncal ataxia. There was positional nystagmus to the right; spontaneous and gaze-evoked nystagmus were absent. He had no facial nerve palsy, dysarthria, pyramidal tract signs, limb ataxia, and sensory impairment. Pure tone audiometry demonstrated a profound sensorineural deafness of both ears. A tentative diagnosis of sudden deafness was made. He was treated…

We assessed the efficacy of the canalith repositioning maneuver by comparing it with no treatment in a population of patients with benign paroxysmal positional vertigo (BPPV). In this randomized, controlled, 6-month efficacy trial, outcomes were measured subjectively by patients' reports of symptom status and objectively by Hallpike testing. During the first month of the study, the treated group experienced significantly better outcomes than did the control group, but this trend was not sustained at 3 and 6 months.

The theory of canalolithiasis assumes that benign paroxysmal vertigo and nystagmus is due to particles within the semicircular canal which move freely in the endolymph under gravitational forces, thereby deflecting its cupula. According to this theory the particles could be removed from the posterior canal by manoeuvres applying a succession of different head positions. We describe the pathophysiological mechanism of benign paroxysmal vertigo and our experience with one of these manoeuvres, the Semont manoeuvre. The success rate is impressive and the therapy is highly rewarding in a vertigo sy…

Current theoretical issues central to the understanding of pathological disorientation (vertigo) are addressed through a critical review of research into perceptual disorientation in healthy subjects (motion sickness). Investigations inspired by the 'sensory conflict' model of orientation perception typically paid insufficient attention to higher-order meaningful properties of the environment, purposive activity, and individual differences in sensorimotor experience and skill. These factors are incorporated into an alternative 'active perception' approach, which characterizes perception of ori…

Positional vertigo and nystagmus may occur when inputs from the semicircular canals and otolithic organs are mismatched, and when this mismatch originates from central dysfunction it is called central positional vertigo and nystagmus (CPVN). Therefore, the velocity-storage (VS) circuit, which computes composite estimates of rotational velocity, gravity orientation, and inertial acceleration from peripheral vestibular signals, underlies the generation of CPVN. In this review, we begin by detailing the VS circuit via a deterministic model that demonstrates how it estimates rotational velocity, g…

OBJECTIVE: Benign paroxysmal positional vertigo (BPPV) is commonly attributed to displaced otoconia. These have been shown to have biomineralization close to that of bone, and vitamin D deficiency has been associated with BPPV. We aim to systematically review the available literature on vitamin D supplementation and BPPV intensity and recurrence in adults.
DATABASES REVIEWED: PubMed, MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials (CENTRAL), Current Controlled Trials, and ClinicalTrials.gov.
METHODS: We systematically reviewed the available literature from 1947 to April 2020.…

Distinguishing between central and peripheral causes of vertigo can be challenging not only in an acute setting but also in chronic settings. A thorough review of systems and physical exam can assist providers in differentiating central versus peripheral etiologies and the need for urgent imaging. In this case, a 47-year-old man presented with vertigo, right-sided hearing loss, right-sided headache, and right-sided facial pain that began 4 weeks before while the patient was on a cruise ship. His physical exam findings were notable for anisocoria with right pupil 3.5 and left pupil 4.5, mild pt…
