A 61-year-old woman was admitted to our hospital with progressive dyspnea on exertion. Chest X-rays and computed tomography (CT) showed elevation of the left diaphragm and atelectasis of the left lung. Eventration of the diaphragm was performed. BACKGROUND: Intersphincter resection (ISR) is considered to be a superior technique offering sphincter preservation in patients with ultralow rectal cancer.1 Because high-definition laparoscopy offers wider and clearer vision into the narrow pelvic cavity and intersphincteric space, ISR has been further refined.2 However, functional outcome after ISR has not been optimal. Foreign bodies located in the upper aerodigestive tract, if not diagnosed and taken out speedily, may be the cause of great morbimortality. We present a female patient who arrived at the emergency department reporting she had swallowed a fish bone; it was finally located in the pre-vertebral muscle at the level of C3-C4, where it had entered from the oesophagus at 2 cm from the superior oesophageal sphincter. We used a cervicotomy assisted by endoscopy, showing a new application for videoendoscopy.

Pneumocephalus is a very rare complication of video-assisted thoracoscopic surgery (VATS). A 66-year-old man developed pneumocephalus secondary to thoracoscopic excision of a neurogenic tumor in the posterior mediastinum. Pneumocephalus was diagnosed by brain computed tomography. The authors present their initial experience with minimally invasive surgery of the oesophagus. Two emblematic cases of benign and malignant oesophageal pathology, treated by laparoscopic transhiatal oesophagectomy, are reported. The surgical procedure is described in detail and compared with other techniques reported in the literature. Its advantages in terms of postoperative pain and morbidity are stressed. The role of the procedure in neoplastic diseases of the oesophagus is still debated, above all with regard to the accuracy of the mediastinal node dissection.

We report the rare case of a multicystic mesothelial cyst of the mediastinum. A 43-year-old man was referred to our department after a routine chest X-ray showed an anterior mediastinum mass. The lesion was resected by thoracoscopy, which revealed multicystic, thin-walled tense masses arising in the anterior mediastinum. The cysts were not adherent to the pericardium or to the thymus of the right lobe.
