Severe form of type 2 reaction in patients of Hansen's disease after withdrawal of thalidomide: case reports.

Thalidomide, a racemic glutamic acid analogue, was first developed in 1954 and subsequently marketed in Europe, Australia and Canada as a sedative and anti-emetic. It was approved by the Food and Drug Administration (FDA) in the USA in 1998 for the treatment of the cutaneous manifestations of moderate to severe erythema nodosum leprosum (ENL) and suppression of the cutaneous manifestations of ENL recurrences. It is a good choice for management in patients who are dependent on corticosteroids. Common side effects of thalidomide are teratogenicity, peripheral neuropathy, sedation and constipatio

Systemic lupus erythematosus-associated cutaneous cryptococcosis treated successfully with an intermittent 3-day on/off cycle of itraconazole.

We describe a case of systemic lupus erythematosus (SLE)-associated cutaneous cryptococcosis. A 32-year-old woman with SLE and lupus nephritis presented with the erythematous maculae on the chest and the extremities, in which encapsulated yeasts were revealed, and was diagnosed with secondary cutaneous cryptococcosis. We administered fluconazole (FLCZ) and then itraconazole (ITCZ) instead of amphotericin B (AMPH-B) to avoid the risk of renal toxicity of AMPH-B in the patient. While treatment with FLCZ was not particularly effective, repeated intermittent administration of ITCZ on a "3-day on/o

Emerging therapeutic options for periorbital and orbital cutaneous basal and squamous cell carcinomas.

PURPOSE: Recently, several new therapies have emerged to address locally advanced cutaneous basal cell and squamous cell carcinomas. Given the constraints of the ocular adnexa and orbit, this review was designed to discuss the role of these modalities in this region. METHODS: A PubMed search was carried out to analyze the utility of United States Food and Drug Administration-approved therapies to address these malignancies. The data presented in the identified investigations were analyzed and abstracted. RESULTS: Multiple novel interventions may be useful in the management of periocular cuta

Cutaneous leishmaniasis successfully treated with miltefosine.

Leishmaniasis is a neglected tropical disease with notable worldwide burden and increasing prevalence in the United States due to globalization. We describe 2 cases of cutaneous leishmaniasis in New England, United States, both caused by the New World subgenus Viannia, in adults returning from Central America. Both patients underutilized preventive measures against bites from phlebotomine sand flies while abroad. They were successfully treated with oral miltefosine, which was well tolerated. Avoidance of vector transmission is the most important preventive measure. Prompt identification and tr

A case of glyburide-induced leukocytoclastic vasculitis.

INTRODUCTION: Medication-induced leukocytoclastic vasculitis is a small-vessel vasculitis that most commonly manifests with palpable purpuric lesions on gravity dependent areas. Development of the vasculitis occurs within weeks after the initial administration of the medication, with clearance upon withdrawal of the medication. Glyburide, a sulfonylurea medication, is used to treat non-insulin dependent diabetes mellitus. We report a rare case of glyburide-associated leukocytoclastic vasculitis. OBSERVATION: We report a 71-year-old man with type 2 diabetes mellitus who presented with palpable

[Subacute cutaneous lupus erythematosus induced by terbinafine].

BACKGROUND: The onset of subacute cutaneous lupus erythematosus (SCLE) may be associated with the administration of a variety of drugs. Terbinafine, an oral antifungal agent, which rarely causes cutaneous eruptions, has been implicated as the cause or exacerbation of cutaneous lupus erythematosus in several patients. CASE: A 59-year-old man had received oral terbinafine for onychomycosis. The patient had a history of SCLE, which was in complete remission during the last 5 years. A month after initiating treatment with terbinafine the patient developed SCLE, which was diagnosed clinically and

Increased metastasis and mortality from cutaneous squamous cell carcinoma in patients with chronic lymphocytic leukemia.

In patients with chronic lymphocytic leukemia, squamous cell carcinoma behaves aggressively. Our purpose was to compare squamous cell carcinoma metastasis and mortality between patients with chronic lymphocytic leukemia and control subjects. Medical records were assessed retrospectively for 28 patients with chronic lymphocytic leukemia who underwent surgical excision of cutaneous squamous cell carcinoma and for 56 matched control subjects. The rate of metastasis and mortality from cutaneous squamous cell carcinoma were determined on a per-patient basis. Three of 28 patients with chronic lympho

The spectrum of cutaneous reactions associated with calcium antagonists: a review of the literature and the possible etiopathogenic mechanisms.

Calcium antagonists (CAs) or calcium-channel blockers, are a common group of antihypertensive medications. These drugs have the property of blocking the calcium channels of the vascular and cardiac smooth-muscle fibers. They have been associated with cutaneous reactions ranging from exanthems to severe adverse events. The frequency of these reactions may be as high as 48 percent. The most common are ankle or pedal edema (up to 30 %), gingival hyperplasia (up to 21 %), and flushing (up to 10 %). Less common are facial or truncal telangiectasia, photosensitivity reactions, new-onset psoriasis (a

Comparison of nalbuphine and naloxone for local cutaneous nociceptive blockade in rats.

This study investigated the local inhibition of a reflexive cutaneous nociceptive response produced by nalbuphine and naloxone after subcutaneous administration and compared their pharmacological profiles with those of lidocaine. At the single equimolar dose tested (39.3 μmol/kg), nalbuphine produced significantly greater local cutaneous nociceptive blockade than naloxone. Dose-response analyses were subsequently performed for nalbuphine and lidocaine. Both nalbuphine and lidocaine produced dose-dependent local cutaneous nociceptive blockade, and no significant difference in ED50 (50% effectiv

Treatment of cutaneous leishmaniasis with thermotherapy in Brazil: an efficacy and safety study.

BACKGROUND: Pentavalent antimonials remain as the standard drugs in the treatment of cutaneous leishmaniosis. The high cost, difficult administration, long treatment time, toxicity and increasing morbidity are factors that limit the use of these drugs. OBJECTIVES: To describe the response to radiofrequency thermotherapy in the treatment of localized cutaneous leishmaniasis in Brazil, and to evaluate its safety and tolerability. METHODS: We conducted a non-comparative open trial with a total of 15 patients confirmed to have cutaneous leishmaniasis on parasitological examination. A single radi

Global epidemiology of cutaneous zygomycosis.

The large majority of cases reported worldwide as zygomycosis are infections caused by fungi belonging to the order Mucorales. These infections are invasive, often lethal, and they primarily affect immunocompromised patients. Cutaneous zygomycosis is the third most common clinical presentation, after sinusitis and pulmonary disease. Most patients with cutaneous zygomycosis have underlying diseases, such as hematological malignancies and diabetes mellitus, or have received solid organ transplantation, but a large proportion of these patients are immunocompetent. Trauma is an important mode of a

Manifestation of subacute cutaneous lupus erythematosus during treatment with anti-PD-1 antibody cemiplimab - a case report.

INTRODUCTION: The anti-programmed cell death protein 1 (PD-1) antibody cemiplimab has shown promising results in the treatment of unresectable or metastatic squamous cell carcinoma, however, frequently leads to immune-related adverse events limiting therapy efficacy. Although cutaneous side effects are common, only very few cases of cutaneous lupus erythematosus have been reported under anti-PD-1 immunotherapy. So far, no case of cutaneous lupus has been described under treatment with cemiplimab. CASE REPORT: For the first time, we report the case of a patient with advanced squamous cell carc

Gefitinib-associated lichen planus pigmentosus-like eruption.

The epidermal growth factor receptor (EGFR) signaling pathway is one of the oncogenic pathways in non-small cell lung cancer. Gefitinib is classified as a first-generation EGFR-tyrosine kinase inhibitor (TKI). A variety of cutaneous adverse effects related to the drug has been reported. Cutaneous hyperpigmentation is a rare side effect of EGFR inhibitor (EGFRi). Herein, we report a 62-year-old woman with non-small cell lung carcinoma who presented with symmetrical, slate-gray-to-brownish-black macular pigmentation on sun-exposed and non-sun-exposed areas after eight months of gefitinib adminis

Permethrin in companion animals: Mechanistic neurotoxicity, dermal pharmacokinetics, and secondary exposure pathways.

Permethrin remains one of the most widely used pyrethroids in companion animal ectoparasite control, yet its toxicological profile reflects a complex interplay between mechanism of action, dermal delivery, formulation behaviour and secondary exposure. Its primary activity is mediated through state-dependent modification of voltage-gated sodium channels, resulting in delayed channel deactivation/inactivation, repetitive firing and excitatory neurotoxicity. While this mechanism underpins insecticidal and repellent efficacy, it also explains adverse effects in non-target species. Following topica

Release of rapamycin from core-multishell nanocarriers topically applied on ex vivo human skin probed by scanning transmission X-ray microscopy.

Delivery of rapamycin loaded on core-multishell nanocarriers with different architectures supporting redox-induced drug release is investigated on human skin ex vivo by scanning transmission X-ray microscopy (STXM) near the O 1s-absorption edge (520-565 eV). The local concentration of the drug and nanocarriers in skin sections is obtained from linear combination modeling using reference spectra of the drug, nanocarriers, and untreated skin. This allows us to probe selectively rapamycin and nanocarriers as a function of exposure time reaching up to 5 days. Additionally, skin samples were treate

Isopropyl Alcohol-Induced Peripheral Neuropathy After Transdermal Exposure: A Case Report.

Peripheral neuropathy is a common neurological condition with diverse etiologies, including metabolic, infectious, and toxic exposures. While isopropyl alcohol (IPA) toxicity is well-documented in ingestion cases, transdermal exposure leading to neuropathy is rare. IPA is readily absorbed through intact skin and metabolized to acetone, which may exert neurotoxic effects. Chronic dermal exposure can lead to peripheral nerve damage via oxidative stress, mitochondrial dysfunction, and local inflammation. We report an 80-year-old male who developed left lower extremity numbness following topical a

INDIVIDUAL ARTICLE: Real World Use of Topical Tranexamic Acid Serum in Patients With Hyperpigmentation.

Hyperpigmentation and pigmentary alterations are common dermatologic concerns that remain difficult to treat. The modernized topical tranexamic acid serum (TNXA-SHOT) is a novel, patent-pending serum that contains 5% tranexamic acid, 1% amino acid, and 3% endophytol, which have been shown to mediate various biological pathways of the mechanism of action involved in the pigment production process. These three active ingredients aim to fight key hyperpigmentation triggers: inflammation, hormonal imbalance, and environmental pollution. The following real-world case series aims to demonstrate the

Topical Sodium Thiosulfate for the Treatment of Calciphylaxis: Case Report.

Calciphylaxis is a rare syndrome of microvascular calcification and skin infarction, occurring most commonly in patients with kidney failure. Calciphylaxis carries a poor prognosis, and current treatment involves mitigating risk factors, wound care, and medical therapies such as intravenous sodium thiosulfate for calcium chelation. We present an innovative treatment approach of topical sodium thiosulfate soaked gauze successfully used to treat calciphylaxis of a 58-year-old female with renal transplant. She presented with a tender bruise over the posterior right calf, in the context of good gr

Transdermal Delivery of Ruxolitinib Through CO2 Ablative Laser -Assisted Silk Nanovehicle System for Improved Melanocyte Recovery in Vitiligo.

Vitiligo, an autoimmune skin disorder, is characterized by the loss of melanocytes, resulting in significant skin depigmentation. Ruxolitinib (RUX), which could target the JAK-STAT signaling pathway, plays a crucial role in vitiligo treatment but faces great challenges in transdermal delivery. Here, we developed RUX-laden silk fibroin nanoparticles to achieve transdermal delivery with the assistance of a CO2 ablative laser. In vitro assays demonstrated that the RUX-laden silk nanoparticles significantly inhibited the secretion of CXCL9 and CXCL10, and the phosphorylation of AKT2 and STAT3, ind

Ionic Liquid-Mediated Sulfur Activation and Dermal Delivery for Psoriasis Treatment.

Reactive sulfur species, including supersulfides and hydrogen sulfide (H2S), have attracted attention for their anti-inflammatory and redox-modulating properties. Therefore, these sulfur species have the potential to treat inflammatory and oxidative stress-related diseases, such as psoriasis. However, their clinical application remains limited owing to instability, rapid diffusion, and difficulty in maintaining effective local concentrations at inflamed sites. In this study, we developed a thiol-functionalized ionic liquid as a redox-active delivery platform for elemental sulfur S8 to enable l
