Darkness alters maturation of visual cortex and promotes fast recovery from monocular deprivation.

The existence of heightened brain plasticity during critical periods in early postnatal life is a central tenet of developmental sensory neuroscience and helps explain the enduring deficits induced by early abnormal sensory exposure. The human visual disorder amblyopia has been linked to unbalanced visual input to the two eyes in early postnatal visual cortical development and has been modeled in animals by depriving them of patterned visual input to one eye, a procedure known as monocular deprivation (MD). We investigated the possibility that a period of darkness might reset the central visua…

Refractive error blindness.

Recent data suggest that a large number of people are blind in different parts of the world due to high refractive error because they are not using appropriate refractive correction. Refractive error as a cause of blindness has been recognized only recently with the increasing use of presenting visual acuity for defining blindness. In addition to blindness due to naturally occurring high refractive error, inadequate refractive correction of aphakia after cataract surgery is also a significant cause of blindness in developing countries. Blindness due to refractive error in any population sugges

A concise overview of using the P300 component of the event-related potential for objective estimation of visual acuity.

Visual acuity (VA) assessment traditionally relies on patient cooperation, which can be unreliable in pediatric cases, cognitive impairment, or suspected malingering. While the visual evoked potential (VEP) provides an objective alternative, it primarily reflects the integrity of the early visual pathways and may overestimate VA in conditions like amblyopia. This concise review explores the P300 event-related potential as a promising cognitively-driven tool for objective VA estimation. A literature search was conducted to identify studies utilizing the P300-a marker of high-level cognitive pro…

Ocular findings in 22q11.2 deletion syndrome: A systematic literature review and results of a Dutch multicenter study.

The 22q11.2 deletion syndrome (22q11.2DS) is a multisystem disorder with an estimated prevalence of 1:3000 live births. Manifestations show … uosity (32%-78%), posterior embryotoxon (22%-50%), eye lid hooding (20%-67%), strabismus (12%-36%), amblyopia (2%-11%), ptosis (4%-6%), and refractive errors, of which hyperopia (6%-48%) and astigmatism (3%-23%) were most common. Visual acuity was (near) normal in most patients (91%-94%). Refractive errors, strabismus, and amblyopia are treatable conditions …

The most common cause of moderate or poor visual outcome was amblyopia (80%). Overall, 13 patients (68%) had postoperative visual acuity better than the preoperative vision. CONCLUSIONS: Penetrating keratoplasty can yield good anatomic results in children with acquired, nontraumatic causes of corneal scarring, but amb …

Upper eyelid taping to stimulate visual development in amblyogenic congenital ptosis.

Amblyogenic congenital ptosis in a young child generally requires amblyopia therapy and, if that proves ineffective, prompt surgical intervention. However, early surgery for severe congenital ptosis is often associated with poor cosmesis and a higher reoperation rate than surgery performed near school age. We report the case of a child with amblyogenic congenital ptosis successfully managed with upper eyelid taping through 5 years of age.

The differential diagnosis of visual disorders in patients presenting with marked symptoms but with no observable ocular abnormality.

A differential diagnosis of nine conditions which sometimes produce distressing symptoms without overt signs is presented. Sophisticated tests, especially electrodiagnostic ones, can aid the optometrist in making the correct decision. The selected procedures and the expected results are summarized in a table. The conditions reviewed are Stargardt's macular degeneration, Leber's congenital amaurosis, albinism, foveal hypoplasia, retinitis pigmentosa, retrobulbar neuritis, amblyopia, and problems of psychogenic origin.

Generalized fibrosis of the extraocular muscles.

We studied four patients with the general fibrosis syndrome. One patient had bilateral inguinal hernias and unilateral cryptorchism; the other patients had no other congenital abnormalities. The patients developed normally both neurologically and metally. We successfully treated amblyopia and achieved good functional and cosmetic results with strabismus and blepharoptosis surgery. Histopathologic study revealed fibrous infiltration of extrinsic eye muscle and Tenon's capsule without inflammatory changes.

Evaluation of Choroidal Vascular Index in Amblyopic Patients.

OBJECTIVES: To compare subfoveal choroidal thickness (SFCT) and choroidal vascular index (CVI) in patients with hyperopic refractive and strabismic amblyopia and healthy subjects. MATERIALS AND METHODS: The study included 17 patients with strabismic amblyopia (Group 1), 29 patients with hyperopic refractive amblyopia (Group 2), and 16 eyes of 16 healthy volunteers (Group 3). Best corrected visual acuity was noted in all patients and volunteers. In addition to detailed anterior and posterior segment examinations, macular images were obtained by enhanced-depth imaging mode of optical coherence …

Long-term Monocular Deprivation during Juvenile Critical Period Disrupts Binocular Integration in Mouse Visual Thalamus.

It is widely thought that the neural deficits underlying amblyopia begin in the circuits of primary visual cortex. [Brown syndrome. General considerations. Case report].

This is an overview of a few general aspects of Brown's syndrome followed by a case report of a three year old girl suffering from congenital Brown's syndrome, astigmatism and amblyopia, which was detected by an ophthalmological screening at her kindergarten. The main differential diagnosis is ipsilateral inferior oblique paresis/paralysis. Since this was a mild-degree syndrome, it didn't require surgery only optical treatement and follow-up.

Re-examining the tarsal kink syndrome: considerations of its etiology and treatment.

A severe corneal ulceration that is evident shortly after birth and persists in spite of adequate medical therapy, may be secondary to a congenital entropion. A vertically kinked tarsus is an unusual and frequently overlooked form of congenital entropion. Prompt recognition of this entity is essential to avoid severe corneal injury and amblyopia. We present a technique of tarsal stabilization which corrected this upper lid deformity in one case.

Long-term ocular follow-up in a case with hereditary mucoepithelial dysplasia.

Hereditary mucoepithelial dysplasia is a rare desmosomal gap junction abnormality known to produce keratitis and cataracts in addition to multiple systemic problems. It generally carries a poor visual prognosis due to corneal scarring, amblyopia, and side effects of the treatment. The authors present a sporadic case of hereditary mucoepithelial dysplasia with an unusually good visual outcome and suggest that timely and appropriate intervention can improve the visual prognosis of this rare disorder.

Visual conversion reaction in children.

Visual conversion reaction (VCR) is a psychosomatic illness that will occasionally be seen by optometrists and ophthalmologists. This paper discusses some of the possible aetiologies and manifestations of the condition. Investigation and management of children with suspected VCR is discussed. Four case histories are presented in which signs of VCR included amblyopia, reduced accommodation and a deterioration in handwriting. The visual field defects presented include a static quantitative anomaly. Such a defect has not previously been reported.

Corneal involvement in autosomal dominant coloboma/microphthalmos.

Peripheral corneal opacification in the line of closure of the embryonic fissure associated with hyperopic astigmatism and anisometropic amblyopia was identified in two eyes of two members of a family with isolated autosomal dominant coloboma-microphthalmos. A review of the literature disclosed no previously reported cases. It would appear that the gene for isolated coloboma-microphthalmos can affect the growth and differentiation of mesenchymal cells of neural crest origin, as well as the neuroectodermal tissues of the embryonic fissure.
