An intravenous dissociation technique for outpatient plastic surgery: tranquility in the office surgical facility.

Intravenous diazepam and ketamine may be used as sole agents, along with local anesthesia, or in combination with preoperative sedation in office plastic surgical procedures. These medications can lower or, in some instances, eliminate the requirements for local anesthesia. They enable procedures traditionally confined to hospital operating rooms (where general anesthesia must be used) to be done in office surgical facilities. True dissociation occurs, and this is not seen without the use of ketamine, which selectively and centrally blocks the effect of painful stimuli without obtunding vital

Sympathetic blockade of isolated limbs by intravenous guanethidine.

As a result of experience at the Montreal General Hospital, it has been found that intravenous guanethidine blockade of extremities has therapeutic, prophylactic and diagnostic value in conditions where the aetiology is a disorder of sympathetic nervous conduction. The properties of guanethidine, namely its selective action on blocking the sympathetic nervous system peripherally, together with its long half time and rapid tissue fixation, render it a very useful drug in techniques where an isolated limb is blocked with guanethidine. Experience on a 18-month basis suggests that the procedure of

Persistent Penile Erection Following Spinal or General Anesthesia During Pediatric Hypospadias Repair: A Case Report.

Penile erection (PE) during anesthesia is a rare yet challenging complication, particularly in pediatric patients undergoing urological surgeries such as hypospadias repair. This case report presents the management of a persistent PE in a 13-year-old male during stage II hypospadias repair under spinal anesthesia, which was subsequently switched to general anesthesia due to the failure of initial interventions. Despite attempts to resolve the erection using intravenous glycopyrrolate, ketamine, cold compresses, blood aspiration, muscle relaxation under general anesthesia, and dorsal penile ner

Sevoflurane-associated torsade de pointes in a patient with congenital long QT syndrome genotype 2.

Although patients with congenital long QT syndrome (c-LQTS) are considered to be at high risk for anesthesia, few reports describe c-LQTS genotype-specific considerations for anesthesia. We describe a case of torsade de pointes (TdP) caused by sevoflurane in a patient with c-LQTS genotype 2 (LQT2). A 39-year-old woman diagnosed with c-LQTS was scheduled for an elective therapeutic abortion. Immediately after starting the operation, the patient developed TdP. Since pulseless ventricular tachycardia was sustained despite intravenous injection of lidocaine, defibrillation was performed. Analysis

General anesthesia in fibrodysplasia ossificans progressive: a case report and clinical review.

Fibrodysplasia ossificans progressiva (FOP) is a rare and devastating genetic disorder of connective tissue characterized by heterotopic bone formation and progressive musculoskeletal disability. Soft-tissue trauma may exacerbate this condition and lead to further ossification. We described the anesthetic and perioperative management of a 23-year-old male with FOP scheduled for dental extraction requiring general anesthesia. Preoperative examination revealed multisystem involvement including cranial and cervical ankylosis and severe restrictive lung disease. Nasal fiber-optic endotracheal intu

Propofol versus thiopental by target controlled infusion in patients undergoing craniotomy.

BACKGROUND: For decades thiopental has been considered as the hypnotic drug of choice for intracranial surgery. However, total intravenous anesthesia performed with thiopental is associated with delayed recovery, whereas early post-operative neurological evaluation is critical. For this reason, target controlled infusion (TCI) of propofol is increasingly used for maintenance of anesthesia. However, a thiopental TCI has never been assessed for this purpose. We tested the hypothesis that a thiopental TCI provides an acceptable way to achieve early recovery compared to a propofol TCI during supra

Activation of CaMKII+ neurons in the paramedian raphe nucleus promotes general anesthesia in male mice.

General anesthesia (GA) is an essential clinical and surgical adjunct, widely recognized as the result of coordinated networks among numerous brain regions. Anesthetic drugs with different characteristics are associated with distinct networks of brain regions involved in anesthesia. Ciprofol, a novel intravenous anesthetic derived from structural modifications of propofol, has shown promise in clinical applications. However, the specific neuronal circuits and brain regions mediating their actions may differ. Moreover, the core brain regions that mediate the common anesthetic effects of these d

Effect of Dexmedetomidine Compared to Remifentanil During Bariatric Surgery on Postoperative Nausea and Vomiting: a Retrospective Study.

PURPOSE: Postoperative nausea and vomiting (PONV) occurs frequently after bariatric surgery and is a major cause of adverse outcomes. This retrospective study investigated whether opioid-restricted total intravenous anesthesia using dexmedetomidine as a substitute for remifentanil can reduce PONV in bariatric surgery. MATERIALS AND METHODS: The electronic medical records of adult patients who underwent laparoscopic bariatric surgery between January and December 2019 were reviewed. The patients were divided into two groups according to the agents used for anesthesia: Group D, propofol and dexm

Anesthetic management of the diaphragmatic pacemaker placement surgery. Our experience in the Institut Guttmann.

BACKGROUND: The diaphragm pacemaker (DP) has proven its utility in replacing mechanical ventilation in patients with chronic spinal cord injury (SCI) and amyotrophic lateral sclerosis (ALS). The DP improves patient quality of life and reduces morbidity and mortality and their associated health care costs. The anesthetic management of these patients and the particularities of the surgical procedure are challenging. The aim of our study is to analyze anesthetic management and intraoperative complications in patients undergoing DP placement in our hospital. METHODS: We performed a chart review o

Intranasal midazolam for rapid sedation of an agitated patient.

Rapidly, establishing a difficult intravenous access in a dangerously agitated patient is a real challenge. Intranasal midazolam has been shown to be effective and safe for rapidly sedating patients before anesthesia, for procedural sedation and for control of seizure. Here, we report a patient in intensive care unit who was on mechanical ventilation and on inotropic support for management of septic shock and who turned out extremely agitated after accidental catheter removal. Intravenous access was successfully established following sedation with intranasal midazolam, using ultrasound guidanc

Dual-route medetomidine-alfaxalone-butorphanol anesthesia: a refined protocol for balanced anesthesia in male rabbits.

Injectable anesthesia is widely used in laboratory animals because of its ease of administration and minimal equipment requirements. However, it necessitates careful monitoring as well as thermal and oxygen support. This study evaluated the efficacy of medetomidine-alfaxalone-butorphanol (MAB) anesthesia in male rabbits using a dual-route administration protocol. The anesthetic doses were as follows: medetomidine, 0.2 mg/kg; alfaxalone, 2.0 mg/kg; and butorphanol, 2.0 mg/kg. MAB anesthesia, administered via intravenous and subcutaneous routes, induced rapid and smooth induction, achieving anes

Propofol injection discomfort recall after monitored anesthesia care or general anesthesia: a prospective observational cohort study.

PURPOSE: Pain during propofol injection is common, yet little is known about how often patients remember this experience after anesthesia or which perioperative factors influence memory formation during induction. METHODS: In this prospective single-center observational cohort study, English-speaking adults (≥ 18 years) receiving propofol under general anesthesia (GA) or monitored anesthesia care (MAC) were enrolled. Perioperative characteristics, intravenous access details, anesthetic medications and clinician-observed injection pain were recorded. On postoperative day 1, patients were inter

[A Case of Postoperative Paraplegia Caused by Idiopathic Spinal Cord Infarction following Hepatectomy under Both General and Epidural Anesthesia].

A 73-year-old woman (height : 155 cm, weight : 55 kg) was scheduled to undergo a laparotomic hepatectomy and radiofrequency ablation for hepatocellular carcinoma. Her medical history did not include any relevant conditions such as cardiovascular or neurological disorders. A thoracic epidural catheter was introduced at T8-9 before the induction of anesthesia with intravenous propofol. General anesthesia was maintained with the inhalation of oxygen, air, and desflurane, and the continuous infusion of remifentanil. Several intraoperative episodes of mild hypotension occurred, each of which was su

Office versus operating room insertion of the bone-anchored hearing aid: a comparative analysis.

BACKGROUND: The Bone-anchored Hearing Aid is indicated for patients with unilateral deafness and/or an irreversible conductive hearing loss. The insertion of this device is usually performed in the operating room under intravenous sedation or general anesthesia. We suggest that the placement of the abutment can occur easily and safely in the clinic setting, reducing time and costs for both the patient and the physician. STUDY DESIGN: Retrospective study. METHODS: Ten patients requiring 11 devices (1 bilateral) were implanted with the Bone-anchored Hearing Aid device in the outpatient clinic
