An accurate assessment of the foot and ankle problem in elite athletes is the foundation of a treatment plan and prognosis. The special pressures of professional sports, where managers, agents, and lawyers may be involved, makes a thorough assessment especially critical for sound decision-making. Evaluation includes taking a history of the acute and chronic condition, including mechanism, physical sensation at injury, compensatory stresses, and general medical review. The athlete is assessed physically in several different ways, including comprehensive focal examination and alignment in static…

Playing sports barefoot has been contested since the very beginnings of athletic competition. Even today, some data suggest that shoes may limit the adaptive pronation that occurs after footstrike during running gait. This pronation likely protects runners from injury. Boardsport participants who perform their sports barefoot on the water seem to be at risk for foot and ankle injuries. The high-impact forces in gymnastics place participants at risk for foot and ankle injuries, as well. Swimming and diving have a low rate of foot and ankle injuries. The risk of ankle sprain in beach volleyball,…

If confronted with a neuropathic patient with an acute foot-ankle trauma, including fracture, this patient is by definition Eichenholtz stage 0. This is of major importance for the treatment. Whereas the surgical handling of fractures does not differ from ordinary fracture treatment, the subsequent treatment of sprains and fractures with immobilisation and a non-weight bearing period is significantly prolonged, frequently doubled, in diabetic patients. Follow-up includes frequent ambulatory controls including X-rays. If the neuroarthropathy progresses, the period of treatment is adjusted accor…

Athletes sustain ankle syndesmosis injuries far less frequently than they do lateral ankle sprains; however, syndesmosis injuries are more challenging to detect and treat. Grade II injuries, which are occultly unstable, may be overlooked or treated too conservatively (nonsurgically), leading to latent diastasis, chronic instability, further injury, arthritic changes, chronic pain, osteochondral lesions, and other sequelae. Surgical intervention for chronic syndesmosis injuries produces mixed results and creates an uncertain future for athletes who desire to return to their sport. Optimal treat…

Traumatic knee injuries in automobile accidents and sports often lead to damage of the peroneal nerve. A lack of control of muscles innervated by the peroneal nerve due to this damage, results in the inability to dorsiflex and evert the foot and to extend the toes. This condition is commonly known as foot drop. Foot drop reduces the stability in the body while walking and running and may also cause injury due to lack of foot clearance during the swing phase of the gait. Traditionally, an ankle foot orthosis (AFO), comprised of a moulded sheet of plastic that conforms around the posterior calf …

BACKGROUND: Isolated involvement of bone in tuberculous infection is uncommon, and the variable clinical and radiological features may mimic pyogenic osteomyelitis, bone tumor or other inflammatory and neoplastic processes of the synovium. We have reported our experiences with the diagnosis of tuberculosis infection in the ankle and foot with the hope of providing sufficient information about these cases to lead to early diagnosis.
MATERIALS AND METHODS: We treated 15 patients with tuberculosis involving the foot and ankle between 1995 and 2005. They were followed for a minimum of 24 months, …

Charcot arthropathy of the rearfoot and ankle is a complex disorder. To date there are no evidence-based, universally agreed upon treatment protocols. As the number of patients who have these deformities continues to increase, surgeons' skill levels and experience grow as well. With increased technical skill, knowledge, and advances in fixation, these deformities are becoming more manageable. In the future this experience should afford the general community with evidenced-based protocols. This article discusses basic techniques in deformity planning and current uses of internal and external fi…

MR arthrography has become an important tool for the assessment of a variety of ankle disorders. MR arthrography permits more sensitive imaging of suspected intra-articular pathology in cases in which conventional MR imaging is either insufficient or inadequate for diagnosis or treatment planning. The main indications for MR arthrography are the evaluation of ligamentous injuries, impingement syndromes, cartilage lesions, osteochondral lesions of the talus, loose bodies, and several synovial joint disorders. Indirect MR arthrography can be a useful adjunct to conventional MR imaging and may be…

Joint bleeding, or haemarthrosis, is the most common type of bleeding episode experienced by individuals with haemophilia A and B. This leads to changes within the joints, including synovial proliferation, which results in further bleeding and chronic synovitis. Blood in the joint can also directly damage the cartilage, and with repeated bleeding, there is progressive destruction of both cartilage and bone. The end result is known as haemophilic arthropathy. The joints most commonly affected are the knees, elbows and ankles, although any synovial joint may be involved. In the ankle, both the t…

We report a case of synovial chondromatosis of the ankle joint, which has been successfully treated with arthroscopic removal of loose bodies and synovectomy, with the patient immediately returning to school and activities of daily living. Recovery after arthroscopic debridement and loose body removal is much shorter in comparison with arthrotomy, and there is no need of immobilization postoperatively.

Ligament injuries of the ankle are common and troublesome. Management may seem easy, but residual symptoms ae common. Grade III injuries still generate controversy in terms of the best management available, and more studies are needed when it comes to early mobilization, cast immobilization, or surgery. Even the three Cohrane reviews published to date are not conclusive.

Athletes engaged in repetitive activity with recent increases in training or competition are at risk for stress fractures of the foot and ankle. Physical and radiographic findings are often unremarkable and diagnosis may be difficult. Although many of these injuries may be managed symptomatically, certain types of stress fractures are at higher risk for delayed union, nonunion, or refracture without aggressive immobilization or surgical fixation.

PURPOSE OF REVIEW: To evaluate the increase in overuse injuries in the adolescent athlete, specifically sports-related injuries to the foot and ankle of the adolescent runner. Factors affecting these injuries include anatomic considerations, gender, rate of development, growth, training errors, shoe wear, and running surface.
RECENT FINDINGS: Most injuries in adolescent runners are overuse injuries rather than sudden traumatic events. Adolescent athletes are as prone as adults to poor form, poor habits, and poor training patterns. In addition, anatomic variants in the hindfoot, midfoot, and f…

THE PROBLEM: Bilateral congenital absence of the fibula in a 10-year-old boy. A marked valgus malalignment at the left ankle and a foot with three rays caused pain during standing and walking. Ortheses did not help. Therefore, various treatment options were considered such as amputation of the foot, a supramalleolar correction osteotomy, and a tibiotalar arthrodesis.
THE SOLUTION: Correction of malalignment and ankle arthrodesis stabilized with an external mini-fixator while sparing the distal tibial physis.
SURGICAL TECHNIQUE: Two skin incisions: one on the medial side visualizing the flexo…

The development of arthroscopic techniques began in the early 1900s. Today, there are several techniques available for treating athletic injuries of the foot and ankle. This article describes arthroscopic techniques for the great toe, subtalar arthroscopy, anterior ankle impingement exostoses (footballer's ankle), posterior ankle arthroscopy, endoscopic calcaneoplasty, and osteochondral lesions of the talus. New techniques for arthroscopic repair of chronic ankle instability are reviewed, and the authors present their results with thermal-assisted capsular modification for chronic lateral ankl…

Despite appropriate acute treatment, many foot and ankle injuries result in posttraumatic arthritis. Arthrodesis remains the mainstay of treatment of end-stage arthritis of the foot and ankle. An understanding of the biomechanics of the foot and ankle, particularly which joints are most responsible for optimal function of the foot, can help guide reconstructive efforts. A careful history and physical examination, appropriate radiographs, and, when necessary, differential selective anesthetic blocks help limit fusion to only those joints that are causing pain. Compression fixation, when possibl…

External fixation as a modality is a versatile and minimally invasive stabilization option that should not be forgotten even as new and specially designed implants for nearly each bone become more fashionable. As a temporary traction device, the external fixator can help condition the soft tissues and, if closed reduction was sufficient, it can stay in place for definitive treatment. The external fixator can be used as an intraoperative distraction device and as a helpful tool for reduction, which can stay in place as a temporary immobilization. External fixation instead of plaster cast allows

BACKGROUND: Syme ankle disarticulation is an amputation level that minimizes disability and preserves function, but it has been used sparingly in patients with diabetes mellitus. Surgeons have avoided this level because of the perceived high risk for wound failure, wound infection, or migration of the heel pad, which makes prosthesis use difficult.
METHODS: Ninety-seven adult patients with diabetes mellitus who underwent Syme ankle disarticulation because of a neuropathic foot with an infection or gangrene, or both, during an eleven-year period were studied retrospectively. Selection of the a…
