[Use of videolaparoscopy in treatment of peritonitis of appendicular origin].

Videolaparoscopic appendectomy was carried out in 108 patients with acute destructive appendicitis complicated by peritonitis. Local peritonitis was noted in 65 patients, diffuse peritonitis in 33 patients, periappendicular abscess in 10 patients. Postoperative complications were observed in 6 (5.6%) patients. Nobody died. A comparison of results of treatment with a group of 143 patients who underwent appendectomy by laparotomic approach has shown that after endovideosurgical treatment there was no such a serious complication as early adhesive intestinal obstruction. The number of wound compli…

Intussusception of the appendiceal stump.

Intussusception of the appendiceal stump is a rare complication of appendectomy. A patient is described in detail and the eighteen previously reported instances are reviewed. Symptoms associated with this entity were abdominal pain (95%), vomiting (47%), blood per rectum (26%), and a palpable abdominal mass (68%). The onset of symptoms occurred within two weeks following appendectomy in 84% of the patients. Barium enema examination was diagnostic in 87.5% of patients in whom it was performed. The diagnosis of intussusception of the appendiceal stump in the postoperative period is difficult bec…

A rare clinical entity: stump appendicitis. Case report and complete review of literature.

BACKGROUND: Stump appendicitis is a rare delayed complication post-appendectomy and it represents a diagnostic problem as clinicians are often not very familiar with this pathology. MATERIALS AND METHODS: One case of stump appendicitis has been reported in a 54 year old woman, whose acute phase was conservatively treated in our Department. A review of Medline literature was also carried out, from 1945 to 2015, showing 111 cases of stump appendicitis. RESULTS: Stump appendicitis has been reported after either open or laparoscopic appendectomy, in a range between days to several years from the…

Laparoscopy in complicated pediatric appendicitis.

BACKGROUND: Complicated appendicitis (gangrenous or perforated) has been associated with increased risk for postoperative complications, especially intraabdominal abscess. Caution has been advised when attempting laparoscopic appendectomy for complicated appendicitis in children. The objective of our study was to assess the incidence of intraabdominal abscess formation after laparoscopic appendectomy in pediatric patients presenting with complicated appendicitis. METHODS: This is a retrospective review of 52 pediatric patients presenting with acute appendicitis at a single teaching institutio…

Stump appendicitis: a myth that can become reality.

Stump appendicitis is a rare etiology of acute lower right quadrant abdominal pain often forgotten in the emergency room (ER). The Mac Burney scar or a previous laparoscopic appendectomy always rule out the eventuality of appendicitis and mislead management. Advanced imaging tools are more than compulsory to help correct the diagnosis. Computed tomography (CT) scan is the option of choice that may be replaced if unavailable by simple ultrasound examination. The treatment is mainly surgical. We report the case of a stump appendicitis occurring 12 years after laparoscopic appendectomy in an 18-y…

Endometriosis of the vermiform appendix: a rare cause of acute abdomen in women - case report and review of literature.

INTRODUCTION AND IMPORTANCE: Appendicular endometriosis (AE) mimicking appendicitis is very rare. It has been reported only 44 times in the literature from 1975 to 2021. It is one of the causes of acute and chronic recurrent pelvic pain, associated with reduced quality of life, which improves after appendectomy. CASE PRESENTATION: We present a rare case of isolated AE in a 19-year-old female, proven by histopathological examination, together with a short review of the literature. DISCUSSION: We present a rare case of isolated AE. The definitive diagnosis is usually established following hist…

Controversies and Future Directions in Management of Acute Appendicitis: An Updated Comprehensive Review.

Globally, acute appendicitis has an estimated lifetime risk of 7-8%. However, there are numerous controversies surrounding the management of acute appendicitis, and the best treatment approach depends on patient characteristics. Non-operative management (NOM), which involves the utilization of antibiotics and aggressive intravenous hydration, and surgical appendectomy are valid treatment options for healthy adults. NOM is also ideal for poor surgical candidates. Another important consideration is the timing of surgery, i.e., the role of interval appendectomy (IA) and the possibility of delayin…

Recurrent appendicitis after conservative management of perityphlitic abscess: report of two cases.

We report on two cases of recurrent appendicitis after conservative management consisting of antibiotics and percutaneous drainage of perityphlitic abscess. The first patient presented 13 years after acute appendicitis with a perityphlitic abscess that was treated operatively. This patient refused to undergo interval appendectomy after the initial event. The second patient with appendicitis and perityphlitic abscess was equally managed conservatively initially but presented with reperforated appendicitis and diffuse peritonitis 1 week before scheduled interval appendectomy and also had to unde…

Clinical outcomes and optimal indications for nonoperative management of acute appendicitis in adult patients: a comprehensive literature review.

Appendectomy as the standard treatment for acute appendicitis has been challenged by accumulating evidence supporting nonoperative management with antibiotics as a potential primary treatment. This review aimed to summarize the clinical outcomes and the optimal indications for nonoperative management of acute appendicitis in adults. Current evidence suggests that uncomplicated and complicated appendicitis have different pathophysiologies and should be treated differently. Nonoperative management for uncomplicated appendicitis was not inferior to appendectomy in terms of complications and lengt…

First case report of spigelian hernia containing the appendix after liver transplantation: Another cause for chronic abdominal pain.

INTRODUCTION: Abdominal ventral hernias are common in chronic liver disease due to increased abdominal pressure and sarcopenia. Following liver transplantation, diagnosis of chronic abdominal pain is challenging because it may relate to immunosuppression, scaring or opportunistic infections. PRESENTATION OF CASE: A 62 years-old male presented with chronic abdominal pain one year following liver transplantation due to hepatocellular carcinoma. After work-up he was diagnosed with a Spigelian hernia containing the appendix. We did hernia repair with mesh but appendectomy was not performed since …

Diagnostic Utilization and Accuracy of Pediatric Appendicitis Imaging at Adult and Pediatric Centers.

BACKGROUND: Diagnostic imaging in pediatric appendicitis may decrease rates of negative appendectomy and identify alternate pathologies. We compared imaging practices for children transferred from nonpediatric facilities versus directly admitted to our tertiary children's hospital for laparoscopic appendectomy, and assessed the diagnostic accuracy in each population based on final pathologic diagnosis. MATERIALS AND METHODS: After institutional review board approval, all cases of laparoscopic appendectomy at our children's hospital during 2015 were reviewed. Demographic and clinical data were…

Appendicular abscess presenting as neoplastic ileocecal obstruction.

Most complications after appendectomy occur within weeks of the operation. The authors present a case in which an appendicular abscess presented more than 18 years after appendectomy for acute appendicitis. The abscess simulated a neoplastic obstructive process, and a communication was demonstrated between the cecum and the abscess cavity through the appendiceal stump.

Appendix muscle bands, a forgotten entity.

INTRODUCTION: Appendix muscle bands (AMB) develop from the appendix muscle layer into the mesoappendix. There are few recent publications on this forgotten entity. The objectives of this study were to assess the morphological features of AMB as detected at microscopy examination of appendectomy specimens. METHODS: Sixty-six cases of AMB as detected on appendectomy specimens were assessed for microscopy features on routine hematoxilin and eosin stained tissue slides. The morphological features were analyzed with regard to the main clinico-pathological parameters. RESULTS: AMBs were multiple i…

Does surgical removal of mucosal associated lymphoid tissue protect against adult coeliac disease? A case controlled study.

BACKGROUND AND AIMS: Surgical resection of mucosa associated lymphoid tissue has been suggested as a protective mechanism against the development of inflammatory bowel disease. Mucosal T-cell activity plays a pivotal role in coeliac disease pathogenesis. We aimed to determine if the development of adult coeliac disease is influenced by appendectomy or tonsillectomy. METHODOLOGY: Three hundred patients over 16 years of age with biopsy proven coeliac disease were identified from two hospital databases in South Yorkshire. From these databases, appendectomy and tonsillectomy status was determined…

A case of endoscopic removal of a giant appendicolith combined with stump appendicitis.

Stump appendicitis is an acute inflammation of the residual appendix and is a rare complication after appendectomy. The physician should be aware of the possibility of stump appendicitis in patients with right lower abdominal pain after appendectomy so that delayed diagnosis and treatment can be prevented. Stump appendicitis is usually treated by surgical resection, and endoscopic treatment has not been reported previously. A 48-year-old man who had undergone appendectomy 35 years earlier presented to the hospital because of right lower quadrant discomfort. A computed tomography scan showed a …

Laparoscopic management of symptomatic residual appendicular tip: A rare case report.

Appendectomy is one of the most common emergency surgical procedures. Stump appendicitis is well-recognised entity has been described in the literature. Still, with recent advance in imaging technique, it remains as a clinical challenge for diagnosis and effective treatment. We present a case of 13-year-old boy who underwent laparoscopic appendectomy 3 months back and presented to us with acute abdomen associated with vomiting and fever. Imaging revealed the presence of a tubular residual inflamed tip of the appendix of size 4 cm laying in paracaecal position with approximately 50cc purulent c…

Anisakis simplex and Kudoa sp.: evaluation of specific antibodies in appendectomized patients.

High prevalence and intensity of infection with anisakid larvae has been reported in commercially important fish in Spain. Likewise, Kudoa-infected fish have lately been detected in both fresh and frozen fish. In the present study the possible relation between appendectomy and specific antibodies to these fish parasites was investigated. One hundred and sixty patients were enrolled in this study. They were divided into two groups of eighty patients each and matched for sex and age: Group 1 (appendectomized) and Group 2 (control group). Total immunoglobulins (Ig's), IgG, IgM, IgA and IgE agains…

Surgery for appendicitis: is it necessary?

BACKGROUND: Appendectomy for acute appendicitis is an effective, universally accepted procedure performed more than 300,000 times annually in the United States. It is generally believed that appendicitis progresses invariably from early inflammation to later gangrene and perforation, and that appendectomy is required for surgical source control. Although non-operative management with antibiotics of uncomplicated diverticulitis, salpingitis, and neonatal enterocolitis is now established, the non-operative management of appendicitis remains largely unexplored. METHODS: Systematic review of publ…

Scrotal abscess originating from appendicitis: a case report.

There are numerous reports concerning the presentation of appendicitis as a urologic problem. However, scrotal abscess originating from appendicitis has rarely been described. We report a right scrotal abscess due to a preceding retroperitoneal abscess originating from retrocecal appendicitis. The scrotal abscess developed progressively following appendectomy. Scrotal exploration was carried out under the presumptive diagnosis of abscessed epididymo-orchitis. However, the patient remained febrile and had productive pus drained from the scrotum after exploration. Abdominal computerized tomograp…
