Obesity. Pathogenesis, consequences, and approaches to treatment.

Research has demonstrated that obesity is far more complex than simply eating too much. Although the individual who is only modestly overweight is probably guilty of overindulgence, the grossly obese individual is the victim of a far more complex disorder with genetic, metabolic, psychosocial, and perhaps central nervous system malfunctions all interacting in a poorly understood fashion. The medical consequences of morbid obesity are considerable and demand the patient's and the physician's attention. This article discusses pathogenesis, consequences, and approaches to treatment.

Dietary control and the stomach.

Signals for meal termination are traced to the stomach rather than duodenum or mouth when rats eat familiar food. Two types of signal occur, one measuring amount of nutrient independent of volume or dilution, the other gastric distention. The signals concerning the second ascend the vagus. Nutrient content is measured by some product of the digestive breakdown of the nutrient. The nutrient signals have to be calibrated through a process of learning. The gastric signals generated by food with a novel flavor are disregarded and oropharyngeal cues assume control instead. Finally, evidence against

Haemodynamic effects of oral aminorex and amphetamine in unanaesthetized beagle dogs.

The haemodynamic effects of four daily oral doses of 1·5 mg/kg of aminorex (as fumarate) were measured in unanaesthetized beagle dogs. A similar study was performed in another group of related animals using d-amphetamine sulphate (1 mg/kg). Both agents produced similar sympathomimetic effects consisting of transient rises in systemic and pulmonary arterial blood pressures over an eight-hour period after administration of either drug. Heart rate remained unchanged in the dogs receiving aminorex whereas it decreased in those receiving amphetamine. There was no cumulative or residual haemodynamic

Nucleus of the Solitary Tract Serotonin 5-HT2C Receptors Modulate Food Intake.

To meet the challenge to human health posed by obesity, a better understanding of the regulation of feeding is essential. Medications targeting 5-hydroxytryptamine (5-HT; serotonin) 2C receptors (htr2c; 5-HT2CR) improve obesity. Here we probed the functional significance of 5-HT2CRs specifically within the brainstem nucleus of the solitary tract (5-HT2CRNTS) in feeding behavior. Selective activation of 5-HT2CRNTS decreased feeding and was sufficient to mediate acute food intake reductions elicited by the 5-HT2CR agonist obesity medication lorcaserin. Similar to pro-opiomelanocortin neurons exp

The food intake-suppressive effects of glucagon-like peptide-1 receptor signaling in the ventral tegmental area are mediated by AMPA/kainate receptors.

Glucagon-like peptide-1 receptor (GLP-1R) activation in the ventral tegmental area (VTA) is physiologically relevant for the control of palatable food intake. Here, we tested whether the food intake-suppressive effects of VTA GLP-1R activation are mediated by glutamatergic signaling within the VTA. Intra-VTA injections of the GLP-1R agonist exendin-4 (Ex-4) reduced palatable high-fat food intake in rats primarily by reducing meal size; these effects were mediated in part via glutamatergic AMPA/kainate but not NMDA receptor signaling. Additional behavioral data indicated that GLP-1R expressed s

Fetal size and eating behaviour in childhood: a prospective cohort study.

BACKGROUND: Although studies showed that an adverse intrauterine environment increases the obesity risk in adulthood, little is known about consequences of fetal growth and birth size for eating behaviour. We examined whether fetal and birth size are associated with childhood eating behaviour. METHODS: Participants were 4350 mother-child dyads of the prospective cohort study Generation R. We assessed the relation between fetal and birth size measurements with child eating behaviour at age 4 years by maternal report on the Child Eating Behaviour Questionnaire. Child body mass index (BMI) was m

Treatment of overweight and obesity: lifestyle, pharmacologic, and surgical options.

Recent statistics indicate that overweight and obesity have become an increasingly serious clinical and socioeconomic problem worldwide, and one of the greatest public health challenges of our time. In the United States, 133.6 million (66%) adults are overweight or obese (body mass index [BMI] >/=25 kg/m(2)), with 63.3 million (31.4%) considered to be obese (BMI >/=30 kg/m(2)). The International Obesity Task Force estimates that worldwide at least 1.1 billion adults are overweight, including 312 million who are obese. Overweight and obese patients are at an increased risk for developing numero

The threshold of amylin-induced anorexia is lower in chicks selected for low compared to high juvenile body weight.

Chicks that have undergone long-term selection for low body weight responded to intracerebroventricular amylin injection with reduced food intake at a dose considerably lower and with a greater magnitude suppression than those selected for high body weight. Behaviors unrelated to ingestion were not affected. These data support the thesis of correlated amylin system responses to selection for low or high body weight, with possible implications to other species.

3. Management and prevention of obesity and its complications in children and adolescents.

Obesity in children and adolescents has reached alarming levels--20%-25% of children and adolescents are overweight or obese, and 4.9% of boys and 5.4% of girls are obese. Rates of obesity have increased significantly in Australia from 1985 to 1995, with the prevalence of overweight doubling and obesity trebling. Body mass index (related to reference standards for age and sex) is recommended as a practical measure of overweight and obesity in children, and is used in monitoring individual progress in clinical practice. Obesity in childhood and adolescence may be associated with a range of medi

Behavior therapy and sibutramine for the treatment of adolescent obesity: a randomized controlled trial.

CONTEXT: Adolescent obesity is becoming a national public health problem. Weight-loss medications including sibutramine facilitate weight control in adults and could be used with obese adolescents in combination with behavior therapy (BT). OBJECTIVE: To examine whether increased weight loss in obese adolescents is induced when sibutramine is added to a family-based, behavioral weight control program. DESIGN, SETTING, AND PARTICIPANTS: Randomized, double-blind, placebo-controlled trial consisting of 82 adolescents aged 13 to 17 years with a body mass index (BMI) of 32 to 44 conducted from Mar

Ovarian Hormones Mediate Changes in Adaptive Choice and Motivation in Female Rats.

In female rodents, sexual receptivity is coordinated with cyclic changes in the release of gonadal hormones. Increases in estradiol (E) and progesterone (P) during proestrus and estrus not only induce ovulation but also modulate behaviors that increase the likelihood that the female will find a mate and reproduce. This includes changes in receptive behaviors, such as lordosis, as well as changes in appetitive or proceptive behaviors, including motivation. Interestingly, the direction of these changes in motivation is dependent on the type of reward that is being pursued. While induction of sex

Understanding the Co-occurrence of Depressive and Anxiety Symptoms Among 2021 Henan Flood Victims Through Panel Network Analysis: A 6-Month, Three-Wave Longitudinal Study.

Previous studies have mostly used cross-sectional network to analyze the co-occurrence of depression and anxiety, but this method cannot capture the temporal influences between symptoms. This study uses longitudinal data to explore the dynamic structure of the cooccurrence of depressive and anxiety symptoms among flood victims in Henan in 2021. Data were collected at 3-month intervals from July 20, 2021, to January 30, 2022, and the final sample included 279 disaster victims reporting on items measuring anxiety and depression. We employ the generalized vector autoregressive model approach to e

Central injection of epidermal growth factor (EGF) induces hypophagia via D₁ dopaminergic and β₂ adrenergic receptors in broiler chickens.

In avian species, the central nervous system orchestrates feeding behavior through intricate interplay among diverse neurotransmitter pathways. Epidermal Growth Factor (EGF), a key peptide in cell growth and repair, has been implicated in appetite control in mammals, but its role in avian species remains unexplored. The present study was conducted to investigate the effect of central infusion of EGF on food consumption in neonatal broilers and to determine the possible involvement of dopaminergic and adrenergic systems. In a series of experiments, 5-day-old male broilers were subjected to intr

Functional Connectivity During Exposure to Favorite-Food, Stress, and Neutral-Relaxing Imagery Differs Between Smokers and Nonsmokers.

INTRODUCTION: Tobacco-use disorder is a complex condition involving multiple brain networks and presenting with multiple behavioral correlates including changes in diet and stress. In a previous functional magnetic resonance imaging (fMRI) study of neural responses to favorite-food, stress, and neutral-relaxing imagery, smokers versus nonsmokers demonstrated blunted corticostriatal-limbic responses to favorite-food cues. Based on other recent reports of alterations in functional brain networks in smokers, the current study examined functional connectivity during exposure to favorite-food, stre

Treatment of Parkinson's disease with magnetic fields reduces the requirement for antiparkinsonian medications.

Recently, I reported that extracranial treatment with picoTesla range magnetic fields (MF) is an effective, safe, and revolutionary modality in the management of Parkinsonism including those patients manifesting levodopa-induced motor complications. This treatment, which has emerged as a potentially more advantageous modality than pharmacologic therapy, also produces improvements in nonmotor aspects of the disease including mood, cognitive functions, sleep, pain, appetite, autonomic functions, and sexual behavior, which are usually minimally, if at all, ameliorated by long term therapy with le

Repeated transcranial direct current stimulation reduces food craving in Wistar rats.

It has been suggested that food craving-an intense desire to consume a specific food (particularly foods high in sugar and fat)-can lead to obesity. This behavior has also been associated with abuse of other substances, such as drugs. Both drugs and food cause dependence by acting on brain circuitry involved in reward, motivation, and decision-making processes. The dorsolateral prefrontal cortex (DLPFC) can be activated following evocation and is implicated in alterations in food behavior and craving. Transcranial direct current stimulation (tDCS), a noninvasive brain stimulation technique cap

Position of the Academy of Nutrition and Dietetics: Interventions for the Treatment of Overweight and Obesity in Adults.

It is the position of the Academy of Nutrition and Dietetics that successful treatment of overweight and obesity in adults requires adoption and maintenance of lifestyle behaviors contributing to both dietary intake and physical activity. These behaviors are influenced by many factors; therefore, interventions incorporating more than one level of the socioecological model and addressing several key factors in each level may be more successful than interventions targeting any one level and factor alone. Registered dietitian nutritionists, as part of a multidisciplinary team, need to be current
