Possible Risk Factors Contributing to Atrial Fibrillation Occurrence in Heart Failure With Mildly Reduced Ejection Fraction.

BACKGROUND: Heart failure (HF) is often accompanied by atrial fibrillation (AF), which significantly worsens the outcome of both diseases. Half of individuals with HF has AF, and HF occurs in more than one-third of individuals with AF. Thus, HF and AF are commonly encountered together and are closely interrelated with similar risk factors. The aim of this study was to investigate the impact of potential risk factors on the occurrence of paroxysmal/persistent AF in patients with heart failure with moderately reduced ejection fraction (HFmrEF). METHODS: The study included 193 patients with HFmr

Efficacy of an Adjunctive Electrophysiological Test-Guided Left Atrial Posterior Wall Isolation in Persistent Atrial Fibrillation Without a Left Atrial Low-Voltage Area.

BACKGROUND: Electrical remodeling precedes structural remodeling. In adjunctive left atrial (LA) low-voltage area (LVA) ablation to pulmonary vein isolation of atrial fibrillation (AF), LA areas without LVA have not been targeted for ablation. We studied the effect of adjunctive LA posterior wall isolation (PWI) on persistent AF without LA-LVA according to electrophysiological testing (EP test). METHODS: We examined consecutive patients with persistent AF with (n=33) and without (n=111) LA-LVA. Patients without LA-LVA were randomly assigned to EP test-guided (n=57) and control (n=54) groups.

Patient-Specific Heart Rate Modulates Developmental Electrophysiology in Transcriptomic-Guided In Silico Models of Pediatric Human Atrial Cardiomyocytes.

Cardiac electrophysiology adapts throughout pediatric development, driven by factors including age-associated ion channel expression changes and decreasing heart rate. Our prior transcriptomic-guided simulations of human atrial cardiomyocytes predicted developmental-associated changes in electrophysiology biomarkers at a fixed pacing rate, leaving the contribution of age- and patient-specific heart rate unresolved. In this study, we incorporated intrinsic heart rate into gene expression-guided computational models to predict the interaction between developmental maturation and pacing rate to s

Bilateral coronary artery fistulas with progressive aortic regurgitation presenting as persistent atrial fibrillation, acute heart failure and pulmonary hypertension: a case report.

Coronary artery fistula (CAF) represents a rare cardiovascular anomaly, defined by an abnormal connection between one or more coronary arteries and the cardiac chambers or other intrathoracic vessels. Most CAFs are congenital, single, small, and asymptomatic, often discovered incidentally. Nonetheless, some cases possess potential clinical implications, including arrhythmia, congestive heart failure, cardiomyopathy and pulmonary hypertension. Furthermore, CAFs may coexist with other congenital cardiac anomalies, posing significant diagnostic and management challenges. This case report describe

Galectin-3 levels and the prediction of atrial high-rate episodes in patients with cardiac resynchronization therapy.

Galectin-3 is an inflammation biomarker associated with atrial remodeling which plays a role in the development of atrial fibrillation (AF). Atrial high-rate episode (AHRE) is related to development of clinically documented AF and stroke. The present study aimed to determine the relationship between the presence of AHRE and the coronary sinus (CS) serum sampling of galectin-3 levels in the long-term follow-up of cardiac resynchronization therapy (CRT) patients. A total of 108 consecutive CRT patients were included prospectively in the study. AHREs were defined as atrial tachyarrhythmia episode…

Chronic atrial fibrillation causes left ventricular dysfunction in dogs but not goats: experience with dogs, goats, and pigs.

Structural remodeling in chronic atrial fibrillation (AF) occurs over weeks to months. To study the electrophysiological, structural, and functional changes that occur in chronic AF, the selection of the best animal model is critical. AF was induced by rapid atrial pacing (50-Hz stimulation every other second) in pigs (n = 4), dogs (n = 8), and goats (n = 9). Animals underwent MRIs at baseline and 6 mo to evaluate left ventricular (LV) ejection fraction (EF). Dogs were given metoprolol (50-100 mg po bid) and digoxin (0.0625-0.125 mg po bid) to limit the ventricular response rate to <180 beats/

Altered patterns of gene expression in response to chronic atrial fibrillation.

To obtain greater insight into atrial remodeling at the molecular level we analyzed the changes in gene expression in human atrial tissue between patients with chronic atrial fibrillation (AF) and those with normal sinus rhythm (NSR). cDNA microarray analysis was used to identify genes differentially expressed during sustained AF of more than 6 months (n = 9, mean age, 45 +/- 12, 6 males and 3 females) as compared to those with NSR (n = 9, mean age, 47 +/- 13, 6 males and 3 females). Western blot analysis was performed to confirm the altered gene expression and to establish the changes in prot…

Intense Endurance Exercise: A Potential Risk Factor in the Development of Heart Disease.

Exercise is known to improve cardiac health; however, the upper limit of exercise is not as clear. High-intensity endurance exercise and its association with cardiac dysfunction are becoming more important as more people in the USA participate in endurance activity. Intensive endurance exercise is associated with heart disease. Peer-reviewed primary literature related to endurance exercise, cardiac remodelling, myocardial fibrosis, and arrhythmia was searched and selected from the PubMed Database to be reviewed. Increased cardiac remodelling, myocardial fibrosis, and arrhythmias were associate

Relationship between hypertensive disorders of pregnancy (HDP) and cardiac remodeling during pregnancy: Systematic review and meta-analysis.

OBJECTIVES: Hypertensive disorders of pregnancy (HDP) are among the leading causes of maternal morbidity and mortality. The primary objective of this study was to ascertain whether maternal cardiac remodeling is more prevalent in HDP than normotensive pregnancy and if significant change in aortic root size is involved. The secondary objective was to determine the types of cardiac remodeling often associated with HDP. METHODS: A systematic search was conducted across four electronic databases, including Medline, PubMed, Cochrane and EMBASE. The reference lists of selected articles were also se

Genetic ablation and pharmacological inhibition of immunosubunit β5i attenuates cardiac remodeling in deoxycorticosterone-acetate (DOCA)-salt hypertensive mice.

Hypertensive cardiac remodeling is a major cause of heart failure. The immunoproteasome is an inducible form of the proteasome and its catalytic subunit β5i (also named LMP7) is involved in angiotensin II-induced atrial fibrillation; however, its role in deoxycorticosterone-acetate (DOCA)-salt-induced cardiac remodeling remains unclear. C57BL/6 J wild-type (WT) and β5i knockout (β5i KO) mice were subjected to uninephrectomy (sham) and DOCA-salt treatment for three weeks. Cardiac function, fibrosis, and inflammation were evaluated by echocardiography and histological analysis. Protein and gene

Effect of Obesity on Left Atrial Strain in Persons Aged 35-55 Years (The Asklepios Study).

Obesity increases the risk of heart failure and atrial fibrillation. Left atrial (LA) dysfunction is increasingly recognized as a mediator of cardiovascular disease. Early effects of obesity on LA function have not been examined in large population samples. We quantified LA strain and strain rate (SR) through speckle tracking echocardiography in 1,531 middle-aged community-based participants enrolled in the Asklepios study. We compared LA function between individuals with body mass index (BMI) < 25 kg/m2 (n = 779), 25 to 29.9 kg/m2 (n = 618) and ≥ 30 kg/m2 (n = 134). Significant differences in

The impact of diabetes mellitus on cardiac function assessed by magnetic resonance imaging in patients with hypertrophic cardiomyopathy.

BACKGROUND: The adverse prognostic impact of diabetes on hypertrophic cardiomyopathy (HCM) is poorly understood. We sought to explore the underlying mechanisms in terms of structural and functional remodelling in HCM patients with coexisting diabetes (HCM-DM). METHODS: A total of 45 HCM-DM patients were retrospectively included. Isolated HCM controls (HCM patients without diabetes) were matched to HCM-DM patients in terms of maximal wall thickness, age, and gender distribution. Left ventricular (LV) and atrial (LA) performance were evaluated using cardiac magnetic resonance feature tracking s

Multimodal Atrial Cardiomyopathy Phenotypes for Risk Stratification in Severe Aortic Stenosis Undergoing TAVI.

BACKGROUND: Atrial cardiomyopathy (AtCM) is highly prevalent in patients with severe aortic stenosis (AS), even in the presence of preserved left ventricular ejection fraction (LVEF). Structural, functional, and electrical atrial remodeling may differentially influence outcomes after transcatheter aortic valve implantation (TAVI), although their integrated prognostic role remains unclear. OBJECTIVES: To assess whether multimodal markers of AtCM identify distinct risk phenotypes associated with adverse outcomes after TAVI in patients without prior atrial fibrillation (AF). METHODS: We retrosp…

LAAEV-centric atrial failure phenotype and a novel integrative score for predicting early recurrence after AF ablation.

BACKGROUND: Early recurrence of atrial tachyarrhythmia (ERAF) after radiofrequency catheter ablation (RFCA) for atrial fibrillation (AF) remains a major clinical challenge. While left atrial diameter (LAD) is a known predictor, the role of atrial mechanical function, particularly left atrial appendage emptying velocity (LAAEV), and its integration with other markers needs clarification. METHODS: This single-center retrospective cohort study included 157 drug-refractory non-valvular AF patients undergoing first-time RFCA. Among them, 53 (33.8%) developed ERAF within the 90-day blanking period.

Exergy destruction in atrial fibrillation and a new 'Exergy Age Index'.

The concept of exergy in living organisms has been widely used to explore correlations between exergy and different physiological conditions. Atrial fibrillation (AF) is an abnormal physiological condition that takes place inside the heart and is recognised as a common supraventricular arrhythmia. AF can significantly undermine heart function and subsequently circulatory system. Thus, exergy analysis of cardiac flow during AF is a procedure to quantify the long-term impact of persistent AF. The present study adopts the lumped modelling approach for considering cardiovascular circulation and th

Peroxisome proliferator-activated receptor-α activation attenuates susceptibility to postoperative atrial fibrillation.

BACKGROUND: Postoperative atrial fibrillation (POAF) is a frequent complication after cardiac surgery and is associated with adverse outcomes. Peroxisome proliferator-activated receptor-α (PPAR-α) is a key regulator of cardiac energy metabolism, but its role in POAF remains unclear. This study aimed to determine whether downregulation of the PPAR-α pathway contributes to POAF-related metabolic remodeling and whether pharmacologic activation of PPAR-α attenuates susceptibility to POAF. METHODS: Thirty Sprague-Dawley rats were randomized into Sham, atrial fibrillation (AF; sterile pericarditis)
