BACKGROUND: Heart rate (HR) is a major determinant of cardiac output (CO). During tachycardia, adequate left ventricular (LV) filling becomes important; however, excessive tachycardia reduces CO by shortening filling time. The impact of HR on hemodynamics differs according to the underlying cardiac pathological conditions, making HR management challenging. The atrioventricular (AV) time interaction, which represents the temporal coordination between atrial and ventricular functions for optimal LV filling, is key to effective HR management during tachycardia. This study aimed to elucidate tachy

BACKGROUND: Patient-specific programming of cardiac resynchronization therapy (CRT) is often neglected, despite significant nonresponse rates. The device-based SyncAV CRT algorithm dynamically adjusts atrioventricular delays to the intrinsic AV interval, reduced by a programmable offset, to accommodate each patient's changing needs.
OBJECTIVE: The purpose of this study was to evaluate the acute effect of biventricular (BiV) pacing enhanced by SyncAV on electrical synchrony in a broad patient population.
METHODS: Patients with existing CRT implants were prospectively evaluated at 5 internatio

BACKGROUND: Fetal and neonatal atrioventricular (AV) reentrant tachycardias can be life-threatening but resolve in most cases during the first year of life. The transient presence of accessory AV myocardial connections during annulus fibrosus development may explain this phenomenon.
METHODS AND RESULTS: A total of 45 human embryonic, fetal, and neonatal sectioned hearts (4 to 36 weeks of development) were studied immunohistochemically. Accessory myocardial AV connections were quantified and categorized according to their specific location, and 3D reconstructions were made. Between 4 and 6 wee

Dizziness in the upright posture is a common reason to seek medical advice and is often dismissed as benign orthostatic hypotension. We present an unusual case of dizziness in the upright posture due to possible changes in atrioventricular nodal conduction during atrial tachycardia.

Electrocardiograms were taken from an 84-year-old man with right bundle branch block in whom atypical atrioventricular Wenckebach periodicity was frequently occurred. The electrocardiographic findings as mentioned below suggested that the atypical periodicity was caused by conduction through triple atrioventricular junctional pathways as a probable mechanism. When a P wave was blocked after a markedly prolonged PR interval of 0.64 s, the RP interval containing this blocked P wave ranged between 0.84 s and 0.86 s, and the next P wave was followed by a QRS complex of the same configuration, with

A 69-year-old woman developed acute pulmonary edema and severe mitral regurgitation (MR) 2 days following an uncomplicated AV nodal (AVN) ablation and insertion of VVI pacemaker for chronic atrial fibrillation. There was no history of significant mitral valve disease. Left ventricular function was normal and there was no evidence of an acute cardiac ischemic event. Transthoracic echo and right heart catheterization studies showed reduction in the severity of MR with biventricular pacing as opposed to RV pacing alone. A permanent pacemaker configured for biventricular pacing was implanted with

Pharmacologic treatment has been used for decades for conversion and prevention of recurrent atrial fibrillation (AF). But the use of antiarrhythmic drugs is associated with substantial side effects and mortality in some patients. Accordingly, it is not surprising that nonpharmacologic techniques have been developed for the management of AF, including the use of atrial defibrillators, atrial pacing methods, and several surgical and radiofrequency catheter ablation procedures. The atrial defibrillator has been found to detect and treat atrial and ventricular arrhythmias appropriately, with succ

We present a 57-year-old man with Wolff-Parkinson-White syndrome who exhibited a wide "gap" in retrograde conduction through a concealed atrioventricular accessory pathway. The appearance of the wide "gap" depended on the ventricular pacing sites. While ventricular extrastimuli at a basic cycle length of 600 msec from the right ventricular outflow tract consistently conducted to the atria, retrogradely through the accessory pathway, those from the right ventricular apex repeatedly revealed disappearance of the retrograde conduction at the wide coupling intervals from 550 to 380 msec. The mecha

A case of incessant supraventricular tachycardia continuing despite AV block is reported. Atrial tachycardia and AV nodal reentrant tachycardia were excluded, as was orthodromic tachycardia using a concealed accessory AV pathway. The earliest retrograde atrial activation was at the posterolateral tricuspid annulus, and the tachycardia was eliminated by ablation at this site. The findings in this case are explained only by a concealed atrionodal pathway.

An 18 year old female with Fallot's tetralogy had undergone complete repair at thirteen years of age. Two years later she first presented a wide complex right bundle branch block tachycardia at a rate of 220 beats/min which could not be controlled on intravenous verapamil. Electrical shock successfully converted tachycardia to sinus rhythm, which showed typical Wolff-Parkinson-White syndrome. On electrophysiological study, the ortodromic tachycardia was found to be due to left lateral atrioventricular accessory pathway, which was ablated by radiofrequency catheter ablation. One year later she

Seventy patients with left atrioventricular accessory pathways and paroxysmal supraventricular tachycardia (PSVT) underwent radiofrequency catheter ablation (RFCA). The success rate was 94.3%. Among these patients, 26 had manifest preexcitation syndrome, and 44 had concealed preexcitation. Eighteen patients with concealed preexcitation underwent coronary sinus (CS) pacing, and delta wave appeared in 15. The keys to successful RFCA were correct positioning of the radiofrequency (RF) catheter tip, A/V amplitude ratio, AV interval (in sinus rhythm) and VA interval (during SVT or ventricular pacin

In 19 pentobarbital anesthetized dogs subjected to right thoracotomy, plunge wire and catheter electrodes were positioned to record and pace from the high right atrium, coronary sinus os, aortic root (for His bundle electrograms), and the right ventricular outflow tract and apex. A deflectable-tip catheter electrode was placed under the septal tricuspid leaflet for pacing the right ventricular septal crest. Electrocardiographic leads II and aVR were also recorded during atrial and ventricular pacing in the control state and during low- and high-intensity left vagosympathetic trunk stimulation.

A new technique to block conduction from both atria to the atrio-ventricular node (A-V node) was devised through anatomical considerations of the junctional area. To assess the validity of the technique, a series of experiments were performed in mongrel dogs. Under cardiopulmonary bypass, the interatrial septum was incised along the tendon of Todaro, followed by cryosurgical ablation of the approach to the A-V node. Postoperative electrophysiological studies revealed complete conduction block between both atria and the A-V node. A spontaneous A-V nodal rhythm was produced. Initial activation w

Syncope occurs in up to 20% of patients with supraventricular tachycardias and is suggestive of rapid and dangerous arrhythmias. Incidence, pathomechanism and consequences of syncope in supraventricular tachycardia are reviewed in this presentation. Frequent symptoms in supraventricular tachycardias are palpitations, dizziness or dyspnea. Syncope is more uncommon, however, if a sensation of rapid heart beat precedes a syncope, a causal relationship between arrhythmia and syncope has to be considered. When the surface ECG shows no abnormalities, Holter monitoring or exercise testing usually fai

Escape rhythms characteristics after successful catheter ablation of the atrioventricular junction and intentional complete heart block were compared with those due to others etiologies as well as the advantage of the posterior implantation of a rate responsive pacemaker (VVIR). In 22 patients, 9 men and 13 women, with permanent AV block after fulguration and follow-up periods from 4 to 58 months, escape rhythms data studied during the procedure and at short and long-term periods, showed the following characteristics: 1) Classical parameters of valuation are not as a whole useful to evaluate s

Ten anesthetized dogs were studied in an attempt to provoke partial alterations in atrioventricular (AV) conduction by high-frequency current (HFC) transcatheter ablation. A discharge power (10 to 15 W) was used for less than 5 seconds after reaching complete AV block (CAVB). The catheter was placed within an area having an A/V ratio = 1 with His bundle deflection. If following discharge, no appreciable lengthening an AH, AV nodal block cycle length (Wenckebach point, WP), and/or functional nodal refractory period (FRPAVN) was observed, the procedure was repeated. Four dogs (group I) were kill

In this study we sought to determine whether characteristics of ventricular-induced atrial preexcitation during reciprocating tachycardia could help differentiate atrioventricular (AV) nodal reentry from orthodromic AV reentry using an accessory pathway and to identify the site of accessory pathways in patients with Wolff-Parkinson-White syndrome. Fifty-five patients with orthodromic AV reciprocating tachycardia and 22 patients with AV nodal reentrant tachycardia were studied with standard electrophysiologic techniques. There were 24 left free wall, 23 posterior septal, seven anterior septal,
