This is a report of long-term results and complications in 62 patients. Closed tibial osteotomy, direct tibial distraction, and simultaneous indirect proximal fibular epiphyseal distraction are the essential features of this new technique. Fibular osteotomy and complications from tibiofibular synostosis are eliminated. Bone graft and plating procedures have been proven to be unnecessary. The patient is ambulated immediately. Hospitalization has been reduced to a few days.

Ninety-seven limbs, in eighty-one patients, with a diagnosis of congenital deficiency of the fibula have been reviewed. A classification was devised to distinguish the minimal hypoplasia of the fibula (Type I) from the well-known complete absence (Type II). Congenital anomalies of the femur were present in 76 per cent of patients with Type I deficiency and in 59 per cent with Type II. The need for a screening programme for spinal stenosis in children with achondroplasia undergoing limb lengthening was identified in a tertiary limb reconstruction service. The authors report a new technique for mandibular distraction. Lengthening of the mandible by gradual intraoral distraction was obtained in nine young patients. An intraoral device was used in order to avoid external scars. Seven patients had hemifacial microsomia, one patient had a ramus hypoplasia after TMJ ankylosis and one patient had the Treacher-Collins syndrome. PIP joint necrosis occurs in multioperated patients with a history of a dorsal and palmar approach to their joint. Seven cases are reported. The surgical treatment consisted of articular resection followed by external distraction using the MS3 device. Four of the patients regained a painless range of movement (30 to 50 degrees): two of these patients had a clinodactyly of 20 degrees. The final range of movement was better when the extensor apparatus was not injured. Three patients were reoperated: a fusion was performed which healed, more rapidly than normal.

We have studied 204 feet in 181 children with congenital limb shortening. There were 17 short femurs in 11 children in whom it was impossible to lengthen the leg. Thirteen feet were abnormal. It was necessary to adapt the foot to fit the chosen prosthesis. In 34 children it was possible to correct the length of the short femur. The authors describe the progressive correction of a deformed forearm and an ulnar deviation of the hand as a result of a bullet wound. The injury comprised a fracture of the two bones of the forearm, with deformed callus and after effects of a probable Volkmann syndrome. The correction was obtained after 10 months. This report illustrates the interest and the difficulty presented by osteogenesis by distraction and osteotomy with a minimal cutaneous wound. This allows the reduction of risk to the cutaneum and vessels involved in extemporaneous corrections.

Triple arthrodesis can be used for a three-dimensional correction of all types of hindfoot deformities. Prerequisites are a functional integrity of the ankle joint and an adequate bone stock. The procedure blocks the cardanic mechanism of the hindfoot and therefore reduces the shock-absorbing and mobile-adaptive functions of the foot. This results in increased stresses with the risk of long-term degenerative changes. In addition to triple arthrodesis, procedures to the forefoot are often necessary. Especially in neurogenic deformities, muscle lengthening and balancing procedures may be needed.
