Stereotactic radiosurgery poses a significant risk when treating brain metastases in close proximity to the brainstem. To address this issue, a novel approach known as "combined anti-vascular therapy" has been devised for these metastases. The topographical relationships between the location of brainstem lesions detected by magnetic resonance imaging and abnormality of brainstem auditory evoked potentials (BAEPs) and short-latency somatosensory evoked potentials (SSEPs) were studied in 57 patients with stroke in the posterior fossa. MRT is a highly sensitive method for the diagnosis of childhood tumours in the posterior fossa. Since it demonstrates tumour extent better than does CT, MRT is the method of choice for radiotherapy planning. The result of treatment can be judged morphologically and by measuring relaxation times. Changes due to treatment can be recognised more easily than by CT. A disadvantage of MRT is lack of specificity, since various processes may lead to an equal increase in T1 and T2 times.

A retrospective survey of 41 patients with unbiopsied thalamic, pontine, and medullary tumors treated with radiation therapy demonstrated 30% to 40% 3-year survival, with seven long-term survivors. The case histories of 124 children irradiated for brain tumors were reviewed to determine survival rate and functional prognosis, with special reference to somatic growth. Five-year survival rates (29% of 97 patients with gliomas and 70% of 27 patients with non-gliomatous tumors) were approximately the same as in our adult series, despite the differences in histology and predilection. Although 70% of 42 long-term survivors had active, useful lives, stunted growth was observed in 15 of them. More attention should be directed to the growth of children irradiated for brain tumors.
