Total Lung-sparing Surgery for Tracheobronchial Low-grade Malignancies.

BACKGROUND: Total lung-sparing tracheobronchial sleeve resections are a step forward in the treatment of low-grade bronchial tumors in which minimal resection margins are required to achieve complete control of the disease. METHODS: This study retrospectively collected data on patients who underwent total lung-sparing procedures for low-grade tracheobronchial tumors at 2 thoracic surgical centers from January 1984 to October 2019. RESULTS: The study included 98 patients, 46 -female (47%) and 52 -male (53%), with a median age of 39 years (range, 7 to 70 years). Thirty-four patients underwent …

Fluorescence navigation with indocyanine green for identification of intersegmental planes using a photodynamic eye camera.

BACKGROUND: Pulmonary segmentectomy is an important surgical option for complete resection in patients with poor lung function. However, correctly recognizing the intersegmental plane for accurate segmentectomy is sometimes difficult. We therefore developed a novel method that allows the detection of intersegmental planes using an indocyanine green (ICG) fluorescence imaging device, photodynamic eye (PDE) camera, PDE-neo. METHODS: As a prospective study, we performed bronchial ICG-guided segmentectomy using PDE-neo. The patients were placed in a lateral position under general anesthesia, and …

Downregulation of PGI2 pathway in Pulmonary Hypertension Group-III patients.

Pulmonary hypertension (PH) is a progressive and life-threating lung disorder characterized by elevated pulmonary artery pressure and vascul … nary artery, pulmonary vein, distal lung tissue, pulmonary artery smooth muscle cells (hPASMC), and bronchi in PH Group-III (n = 35) and control patients (n = 32). Our results showed decreased PGIS expression and/or 6-keto-PGF1α levels in human pulmonary artery, hPASMC, and distal lung tissue derived from PH Group-III patients. Moreover, the production of …

Impact of endobronchial coiling on segmental bronchial lumen in treated and untreated lung lobes: Correlation with changes in lung volume, clinical and pulmonary function tests.

OBJECTIVES: To assess the impact of endobronchial coiling on the segment bronchus cross-sectional area and volumes in patients with lung emphysema using quantitative chest-CT measurements. MATERIALS AND METHODS: Thirty patients (female = 15; median age = 65.36 years) received chest-CT before and after endobronchial coiling for lung volume reduction (LVR) between January 2010 and December 2014. Thin-slice (0.6 mm) non-enhanced image data sets were acquired both at end-inspiration and end-expiration using helical technique and 120 kV/100-150 mAs. Clinical response was defined as an increase in …

Obesity induced by neonatal overfeeding worsens airway hyperresponsiveness and inflammation.

BACKGROUND: Obesity is a risk factor for the development of certain respiratory diseases, and neonatal overfeeding results in an early onset … on was evident in SL mice on P150, as reflected by inflammatory cells especially macrophages around bronchi and interstitium. BALF and serum TNF-α levels and lung TNF-α mRNA expression were significantly increased in SL mice on P150. More collagen accumulated surrounding the bronchi on P150; lung mRNA levels of TGF-β1 and CTGF were also increased on P150. …

Quantitative assessment of bronchial wall attenuation with thin-section CT: An indicator of airflow limitation in chronic obstructive pulmonary disease.

OBJECTIVE: The purpose of this study was to evaluate the relation between bronchial wall attenuation on thin-section CT images and airflow limitation in persons with chronic obstructive pulmonary disease. SUBJECTS AND METHODS: One hundred fourteen subjects (65 men, 49 women; age range, 56-74 years) enrolled in the National Lung Screening Trial underwent chest CT and prebronchodilation spirometry at a single institution. At CT, mean peak wall attenuation, wall area percentage, and luminal area were measured in the third, fourth, and fifth generations of the right B(1) and B(10) segmental bronc…

Bronchial artery anatomy: preoperative 3D simulation with multidetector CT.

PURPOSE: To preoperatively evaluate anatomic variations of the bronchial arteries by obtaining three-dimensional (3D) simulations with multidetector computed tomography (CT). MATERIALS AND METHODS: This study was approved by the institutional review board, and written informed consent was obtained from all participants. Seventy-three consecutive patients with esophageal cancer underwent dynamic multidetector CT. The data were used to generate 3D simulations of the thoracic cavity. These images were then used to evaluate anatomic variations of the bilateral bronchial arteries and the mediastin…

Airway dimensions at inspiratory and expiratory multisection CT in chronic obstructive pulmonary disease: correlation with airflow limitation.

PURPOSE: To analyze the relationship between airflow limitation and airway dimensions from the third to the fifth generation of bronchi in patients with chronic obstructive pulmonary disease (COPD) by using inspiratory and expiratory multisection computed tomography (CT). MATERIALS AND METHODS: This retrospective study was approved by the institutional review board, which waived the need for informed consent. The study included 50 patients with COPD who underwent both inspiratory and expiratory 64-detector CT. In each patient, mean values of airway luminal areas from the third to the fifth ge…
