[Percutaneous drainage of the gallbladder in acute cholecystitis].

Percutaneous cholecystostomy has replaced surgical treatment for acute cholecystitis in surgical high-risk and critically ill patients. We wished to assess the procedures performed at Lillehammer County Hospital. We report 32 drainages performed in the last three years. Clinical records and radiological reports were reviewed retrospectively. Technical problems and complications during and after the procedure were recorded. In spite of relatively few procedures per radiologist, all were initially successful. No serious complications related to the procedure were encountered. Dislocation of the …

Heterotopic pancreas--an unusual cause of cholecystitis.

A 47-year-old woman presented with signs and symptoms of acute cholecystitis. A routine cholecystectomy was performed. No gallstones were present. On histological examination of the gallbladder, a nodule impinging on the lumen was present in the region of the cystic duct. This nodule was composed of mature, uninflamed exocrine and endocrine pancreatic tissue. In the absence of gallstones, the heterotopic pancreas was the cause of obstruction, with subsequent cholecystitis.

[Cholecystic-choledochal calculi. Endolaparoscopic treatment].

BACKGROUND AND AIM: Common bile duct calculosis is a pathology which has always been treated using traditional laparotomic surgery. The introduction of endoscopic papillosphincterotomy and mini-invasive and videolaparoscopic methods has drawn attention to the fact that these methods may be able to replace conventional surgery. METHODS: The authors examine the epidemiological and clinical aspects of common bile duct calculosis, reporting their experience in ten female patients treated during the period from 1992 to 1997, all of whom presented jaundice, pain and fever. Of these, eight underwent

Is acute cholecystitis a contraindication for laparoscopic cholecystectomy?

Two-hundred and ninety-four patients underwent laparoscopic cholecystectomy between May 1993 and April 1997 in the Clinical and Surgical Gastrointestinal Unit at Santa Casa Hospital of Porto Alegre. Of all, 47 were operated on for acute cholecystitis (AC). The mean age was 47.1 years, 70.2% were females and 29.8% were males. The mean operative time was 142.8 minutes. Routine intraoperative cholangiography was successfully done in 38 (80.8%) and conversion was necessary in 3 (6.4%). All patients received prophylactic antibiotics for 24 hours. No intraoperative or postoperative complications wer…

Surgical risks of acute cholecystitis in elderly.

BACKGROUND/AIMS: For the elderly patient, an emergency biliary procedure carries a higher risk than an elective operation. Recently introduced advances in ultrasonography and critical care medicine have affected the clinical risks of surgery for acute cholecystitis in the elderly. This study evaluated the clinical risks of open cholecystectomy for the elderly with acute cholecystitis. MATERIALS AND METHODS: During a 10 year period (1985-1994), a total of 145 patients were diagnosed with acute cholecystitis and underwent cholecystectomy. According to their age, the patients were divided into 3…

Acalculous cholecystitis in patients with acute leukaemia.

Cases of acalculous cholecystitis in patients with acute leukaemia are rare. Manifestations of the primary disease often mask the acute cholecystitis symptoms. A high index of suspicion and ultrasonographic examination in leukaemic patients with abdominal pain and unexplained fever may allow earlier diagnosis, before the development of complications. We report two cases of acute acalculous cholecystitis during the course of acute leukaemia.

Laparoscopic cholecystectomy for acute cholecystitis in the elderly.

This study reviewed the results of laparoscopic cholecystectomy for acute cholecystitis in the elderly. Among 557 patients who underwent laparoscopic cholecystectomy, 70 (12.6%) had a clinical diagnosis of acute cholecystitis confirmed by ultrasonography. There were 28 men and 42 women with a mean age of 59.9 years (range 20-87 years). Thirty patients >/= 65 years of age were compared to 40 patients < 65 years old. Elderly patients had a higher female predominance (p < 0.05), a higher incidence of intercurrent diseases (p < 0.05), and a higher serum urea level (p < 0.001). The proportions of p…

Laparoscopic cholecystectomy in the treatment of acute cholecystitis.

BACKGROUND: Laparoscopic cholecystectomy has several advantages over traditional cholecystectomy, which make it the treatment of choice for patients with uncomplicated biliary lithiasis. However, in patients with acute cholecystitis, the role of this technique remains controversial and some clinicians regard this condition as a contraindication to laparoscopic cholecystectomy. STUDY DESIGN: Between June, 1991 and July, 1993, a total of 259 patients with cholelithiasis underwent laparoscopic cholecystectomy at the "Virgen de la Arrixaca" University Hospital. Of these patients, 60 underwent lap…

Acute acalculous cholecystitis in the critically ill.

Acute acalculous cholecystitis, inflammation of the gall-bladder without evidence of calculi, accounts for 2 to 15% of all cases of acute cholecystitis. The incidence of acute acalculous cholecystitis in adults undergoing cholecystectomy may be as high as 15 per cent and up to 32 per cent in the pediatric population. During the past 10 years, 22 patients who were being treated in the intensive care unit for other reasons underwent cholecystectomy for acute acalculous cholecystitis. Eighteen (82%) of the patients were male, the average age was 61 years, and patients spent an average of 19 days …

[Acute cholecystitis: elements for diagnosis and therapy].

The Authors report their experience in the surgical management of 439 patients with acute cholecystitis. Symptoms and laboratory findings at onset, preoperative diagnostic tools, associated disorders, surgical procedures as well as age-related postoperative morbidity and mortality and length of postoperative stay were analyzed. The most valuable investigation in the diagnosis of acute cholecystitis was ultrasound carried within the first 48 hrs, with positive results in 81% of cases. Tc-99m-HIDA biliary tract imaging proved to be a rapid, highly sensitive and specific, non invasive method of d…

Laparoscopic cholecystectomy for acute or gangrenous cholecystitis.

Over the past 4 years, laparoscopy has become the preferred approach for cholecystectomy in the majority of patients. Most of the early articles described laparoscopic cholecystectomy for chronic cholecystitis or presented only a limited number of patients with an acute process. This paper reviews our experience with laparoscopic cholecystectomy for acute and gangrenous cholecystitis. There were 460 attempted laparoscopic cholecystectomies, including 44 for acute nongangrenous cholecystitis and 24 for gangrenous cholecystitis. The conversion rate to open cholecystectomy was 2.8 per cent for ch…

Ascaris cholecystitis and cholangitis: an experience in Myanmar.

Twenty-seven cases of ascaris cholecystitis and cholangitis were managed in a surgical unit of a general hospital in Yangon, Myanmar, from January 1989 to March 1990. Nineteen women and eight men with a mean age of 42 years were studied. Main clinical manifestations were right hypochondrial pain, fever, chills, rigors, nausea, vomiting and jaundice. Diagnosis was established by abdominal ultrasonograms in all cases. Laparotomy was performed in all cases because of failure to respond to initial conservative treatment. Live and dead ascarids were found in the gall bladder and biliary ductal syst…
