Late radiation injury of the rectum and sigmoid colon: barium enema findings in 92 patients.

We reviewed the findings on 169 contrast enema examinations in 92 patients with late radiation injury of the rectum and sigmoid colon, encountered over an 11-year period. The diagnosis was made by rectosigmoidoscopy in all patients. The limitations and pitfalls of both examinations were studied. The mean interval between radiotherapy and diagnosis was 1.7 years and the mean follow-up period was 3.5 years. The main radiological features varied from normal findings (15% of the initial examinations) to decreased distensibility of the bowel wall, intestinal fixation, mucosal and contour abnormalit

Mesh Migration Into the Sigmoid Colon Identified After Positive Fecal Occult Blood Test: A Case Report.

Inguinal hernia repair with mesh reconstruction is amongst the most frequently performed elective general surgical procedures and is associated with well-recognized complications such as recurrence, infection, and chronic pain. Rarer complications of mesh repair have also been reported, particularly since the advent of laparoscopic inguinal hernia repair. This report highlights an important, rare complication of left-sided hernia repair, which should be considered by surgeons, particularly in the presence of certain associated risk factors.  A 72-year-old man underwent colonoscopy following a

Concurrent sigmoid colon adenocarcinoma presenting with cutaneous adult T-cell leukemia/lymphoma: a rare case report.

BACKGROUND: Adult T-cell leukemia/lymphoma (ATLL) is a rare peripheral T-cell malignancy induced by Human T-cell lymphotropic virus type 1 (HTLV-1) infection, commonly affecting individuals in their 50s to 60s. The disease manifests through various clinical forms, with skin involvement being a frequent presentation. While HTLV-1 has been linked to several cancers, including non-ATLL lymphomas and liver cancer. This case report discusses the rare coexistence of sigmoid colon adenocarcinoma and cutaneous ATLL in a 75-year-old woman. MATERIALS AND METHODS: A 75-year-old woman presenting with abdo

A Rare Case of Postoperative Pneumatosis Intestinalis Complicated by Sigmoid Colon Perforation Requiring Immediate Surgical Intervention.

Pneumatosis intestinalis (PI) is a rare but potentially serious gastrointestinal condition with diverse clinical presentations. Pneumatosis intestinalis is identified by the presence of gas in the extraluminal intestinal wall and is further subclassified into benign pneumatosis intestinalis (BPI) and life-threatening pneumatosis intestinalis (LTPI). Prompt and accurate diagnosis, often aided by imaging studies, is crucial to guide appropriate management. Treatment strategies vary depending on the severity of symptoms and underlying etiologies. Collaboration between gastroenterology, radiology,

[A Case of Laparoscopic Low Anterior Resection after Chemotherapy for Sigmoid Colon Cancer with Rectal Invasion].

Surgical resection is the standard treatment for resectable colorectal cancer; however, patients with locally advanced colorectal cancer may face challenges, such as difficulty for the surgeon to achieve R0 resection and a high risk of postoperative recurrence. Although, in the Japanese guidelines, neoadjuvant chemotherapy(NAC)is not recommended for patients with resectable colorectal cancer, clinical studies conducted in Western countries have suggested its clinical benefits. In the FOxTROT trial, NAC was reported to promote tumor regression, contributing to a higher R0 resection rate in pati

Reactive lymphoid hyperplasia of the liver after surgery for advanced sigmoid colon cancer: a case report.

We report a case of reactive lymphoid hyperplasia (RLH) mimicking colorectal liver metastases (CRLM) on preoperative workup that was clinically indistinguishable. A 78-year-old woman was found to have locally-advanced sigmoid cancer (T4), and then treated with radical sigmoidectomy. One year after the surgery, plain computed tomography (CT) revealed a low-density area in the right hepatic lobe. Metastatic liver tumors could not be ruled out with CT/ magnetic resonant imaging (MRI) and positron emission tomography-CT . Based on these findings, the patient was diagnosed with CRLM at S7 of the li

Short-term outcomes of vessel-oriented D2 and D3 lymph node dissection for sigmoid colon cancer.

BACKGROUND: Level of lymph nodes dissection (LND) and inferior mesenteric artery (IMA) ligation is still matter of debate of radical resection of colorectal cancer. This study aims to compare the short-term outcome of three different surgical techniques to treat sigmoid cancer: low ligation (LL) of the IMA with D3-LND, low IMA ligation with D2-LND, and high ligation (HL) of the IMA with D3-LND. METHODS: Patients affected by sigmoid colon cancer, who underwent radical resection with three different techniques (LL and D3-LND Group A, HL and D3-LND Group B, and LL with D2 LND- Group C), were inc

Hemorrhagic shock due to ruptured idiopathic intramural hematoma of the sigmoid colon-An autopsy case report.

We describe herein an autopsy case involving fatal hemorrhagic shock due to idiopathic sigmoid colonic intramural hematoma rupturing into the abdominal cavity. Antemortem computed tomography revealed a mass lesion in the sigmoid colon and a large amount of hemoperitoneum. On forensic autopsy, intramural hematoma of the sigmoid colon with ruptured serosa was identified, while the mucosa remained intact. Microscopically, hematomas were apparent, mainly in the muscularis propria. We diagnosed the cause of death as hemorrhagic shock due to idiopathic intramural hematoma of the sigmoid colon ruptur

Sigmoid colon schwannoma difficult to distinguish from peritoneal dissemination 13 years after pancreatic neuroendocrine tumor surgery.

BACKGROUND: Schwannoma, which clinicians sometimes struggle to diagnose, is a tumor arising from Schwann cells of peripheral nerves, often in the soft tissues and rarely in the gastrointestinal tract. Pancreatic neuroendocrine tumor (PNET) is rare among pancreatic tumors, and recurrence can occur long after resection. Here, we were presented with a case where a sigmoid colon schwannoma was difficult to distinguish from a postoperative recurrence of PNET and was diagnosed after laparoscopic resection. CASE PRESENTATION: A 51-year-old man was diagnosed with PNET (NET G2) after a distal pancreat

Nationwide Outcomes After Neoadjuvant Chemotherapy for Locally Advanced Sigmoid Colon Cancer-A Propensity Score-Matched Analysis.

BACKGROUND: The role of neoadjuvant chemotherapy (NAC) in advanced sigmoid colon carcinoma remains to be further characterized. Rationale for NAC includes downstaging on final pathology and optimization of microscopically negative margins (R0 resection). We investigated rates of neoadjuvant chemotherapy use in advanced sigmoid colon cancer at academic cancer centers and assessed factors associated with likelihood of NAC administration. METHODS: The National Cancer Database was queried from 2004 to 2017 for patients with clinical T3 or T4, N0-2, M0 sigmoid colon cancer who underwent surgical r

Is there need for routine CT colonography after CT-verified uncomplicated diverticulitis of the sigmoid colon?

BACKGROUND: Following an episode of acute diverticulitis, surgical guidelines commonly advise routine colonic follow-up to rule out underlying malignancy. However, as a CT of the abdomen is frequently performed during clinical work-up, the routine need for colonic follow-up has become debated. PURPOSE: To evaluate the need for routine CT colonography after an episode of CT-verified uncomplicated sigmoid diverticulitis to rule out underlying colorectal malignancy. MATERIAL AND METHODS: This study retrospectively evaluated 312 patients routinely referred to colonic evaluation by CT colonograph

Prognostic analysis and beneficiary identification of adjuvant external beam radiotherapy for stage pT4b sigmoid colon cancer.

The clinical efficacy of adjuvant radiotherapy in sigmoid colon cancer remains questioned. To evaluate the clinical efficacy of adjuvant external beam radiotherapy (EBRT) for patients with pathologic stage T4b sigmoid colon cancer. Patients with stage pT4b sigmoid colon cancer receiving adjuvant EBRT or not followed by surgery between 2004 and 2016 were extracted from the Surveillance, Epidemiology, and End Results database. Analysis of overall survival (OS) was performed using Kaplan-Meier curves and prognostic factors were identified using Cox proportional hazards regression models with 95%

[A Case of Anastomotic Recurrence 10 Years after Excision of Sigmoid Colon Cancer with Submucosal Invasion].

A 72-year-old woman underwent sigmoid colon resection plus D2 lymph node dissection in 2008, with additional resection after endoscopic mucosal resection(EMR). Histopathological examination revealed only atypical ducts in the EMR scar, with no invasion below the submucosa. No lymphatic, venous, or nerve invasions were confirmed, and oral and anal stumps and lymph node metastases were negative. She was followed up for 5 years after the surgery, and no recurrence was detected. In 2018, she visited our hospital with the chief complaint of diarrhea and constipation. Colonoscopy revealed a circumfe
