Exploring community pharmacists' clinical decision-making using think aloud and protocol analysis.

BACKGROUND: Clinical decision-making (CDM) is the dynamic process used to gather, interpret and evaluate data to select an evidence-based choice of action. As the role of the pharmacist becomes more patient-focused, effective CDM skills are increasingly vital to achieve patient outcomes centred on quality, safety and efficacy. OBJECTIVE: To examine how community pharmacists reason through a clinical problem and what factors assist or hinder decision-making. METHOD: Fifteen New Zealand registered community pharmacists in central Auckland were presented with a bacterial conjunctivitis case. Th

Protective Factors Associated With Fewer Generalized Anxiety Disorder Symptoms Among Women Exposed to Intimate Partner Violence.

Generalized anxiety disorder (GAD) has received minimal empirical attention in the context of intimate partner violence (IPV). Furthermore, factors related to lower levels of GAD symptoms in this population have received limited focus. This study evaluated the protective role of four forms of support, spiritual, family, friend, and community, in predicting levels of generalized anxiety among women who have experienced recent IPV. Participants included 116 women who were recruited from local agencies serving IPV-exposed individuals. Participants completed measures of IPV, GAD, stressful life ev

[Residency: in the hospital or in the community?].

Over the last two decades larger proportions of medical activity have been transferred from the hospital to the community. Although the reason has been economic and not professional, it has affected medical education and the residency programs. In this issue of Harefuah, Nirel and colleagues, suggest transferring a part of the residency program to the community. They review the necessary setup, the difficulties and offer some solutions. Their message is clear: since the current residency program does not prepare the specialist for "real life", a switch to the community may fill the gap. This e

Programmatic and Communication Issues in Relation to Serious Adverse Events Following Ivermectin Treatment in areas Co-endemic for Onchocerciasis and Loiasis.

In areas co-endemic for loiasis and onchocerciasis, the classic Community-Directed Treatment using ivermectin (Mectizan(R)) must be adapted as additional program activities, better communication and tighter control of ivermectin stocks are required to minimize risk and manage serious adverse events following ivermectin treatment in patients co-infected with Loa loa. The importance of these serious adverse events on community participation in onchocerciasis control efforts has not been adequately studied. Program implementers do not as of yet fully understand the psychological impact of serious

Lymphatic filariasis in the Caribbean region: the opportunity for its elimination and certification.

In order to support the case for a certification of elimination of lymphatic filariasis (LF) in some Caribbean countries, we compared the prevalence of circulating Wuchereria bancrofti antigen in communities in Guyana, Suriname, and Trinidad. For the study, we assayed school children in six communities in Guyana, five communities in Suriname, and three communities in Trinidad for the prevalence of circulating W. bancrofti antigen, using a new immunochromatographic test for LF. We also assayed adults in these three countries, with a special focus on Blanchisseuse, Trinidad, where mass treatment

Barriers to care: Caregivers' accounts of raising a child with a communication disorder.

BACKGROUND: There exists a dearth of research on the psychological experiences of childhood communication disorders. Caregivers of these children are one source who can provide us with this information since the child exists within a system. Literature on the experiences of caregivers of children with communication disorders, specifically in the South African city context, is lacking. OBJECTIVES: The article presents the reported experiences of six parental caregivers raising a child with a communication disorder within Johannesburg. METHOD: Data were collected via semi-structured interviews

Comparing different forms of structured follow-up support in the treatment of multiple health risks for chronic disease: a randomized controlled trial protocol.

BACKGROUND: Primary care clinics need effective, low-cost tools to screen for and address multiple health risks - and technology can provide important efficiencies. My Own Health Report (MOHR) is a web-based intervention with 3 core functions: (1) risk flagging; (2) goal-setting for patient-prioritized risks; and (3) providing resources. A prior randomized trial in primary care settings showed that MOHR was feasible and effective. However, we identified two key gaps: a need for a flexible menu of delivery options to adopt and sustain MOHR and a pragmatic way to provide structured follow-up. M

Naturalistic Decision-Making Patterns in Cardiometabolic Multimorbidity Self-Care: A Qualitative Study.

PURPOSE: Self-care is central to managing cardiometabolic multimorbidity, yet primary care clinicians have limited insight into how patients navigate self-care decisions in daily life. This study explored self-care decision-making contexts, processes, and patterns among patients with cardiometabolic multimorbidity to inform family medicine practice. METHODS: We conducted a qualitative cohort study guided by naturalistic decision-making theory among 27 adults with cardiometabolic multimorbidity (mean age = 62.7 years; 16 men). Participants were purposively sampled from a tertiary hospital and

Personal sense of coherence and posttraumatic stress symptoms among internally displaced women exposed to the Anglophone Crisis in Cameroon.

Women and girls affected by armed conflict and forced displacement are exposed to a high burden of potentially traumatic events, gendered insecurity, disrupted social networks, and barriers to mental health care. Evidence on protective psychological resources among internally displaced women in Central Africa remains limited. This study examined trauma load, personal sense of coherence (SoC), and sense of community coherence (SoCC) as predictors of posttraumatic stress disorder (PTSD) symptom severity among women internally displaced by the Anglophone Crisis in Cameroon. A sample of 133 female

American Heart Association's Strategic Approach to Promoting Blood Pressure Control: A Science Advisory From the American Heart Association.

Hypertension, which affects ≈122.4 million American adults and contributes substantially to preventable deaths and economic burden, represents a major public health challenge. Despite the availability of effective therapeutic interventions, only 20.7% of individuals diagnosed with hypertension achieve optimal blood pressure control, defined as readings <130/80 mm Hg. Disparities persist across racial, ethnic, socioeconomic, and geographic groups, prompting urgent calls to improve hypertension management strategies. In response to these challenges, the American Heart Association has established

Preferences of health care workers using tongue swabs for tuberculosis diagnosis during COVID-19.

Healthcare workers (HCW) who come into contact with tuberculosis (TB) patients are at elevated risk of TB infection and disease. The collection and handling of sputum samples for TB diagnosis poses exposure risks to HCW, particularly in settings where aerosol containment is limited. An alternative sample collection method, tongue swabbing, was designed to help mitigate this risk, and is under evaluation in multiple settings. This study assessed risk perceptions among South African HCW who used tongue swabbing in TB diagnostic research during the COVID-19 pandemic. We characterized their contex

Victim voice in reenvisioning responses to sexual and physical violence nationally and internationally.

Internationally and in the United States many victims of sexual assault and domestic violence are unserved, underserved, or ill-served, especially those from the most vulnerable populations. Programs developed in the United States are routinely exported to developing countries but often without success. Notably, the failures seen internationally resemble those in the United States and are related to structural and attitudinal-cultural factors. Many victims do not disclose, and if they do seek services, they often report that available options mismatch their objectives, present accessibility ch

Assessment of animal African trypanosomiasis (AAT) vulnerability in cattle-owning communities of sub-Saharan Africa.

BACKGROUND: Animal African trypanosomiasis (AAT) is one of the biggest constraints to livestock production and a threat to food security in sub-Saharan Africa. In order to optimise the allocation of resources for AAT control, decision makers need to target geographic areas where control programmes are most likely to be successful and sustainable and select control methods that will maximise the benefits obtained from resources invested. METHODS: The overall approach to classifying cattle-owning communities in terms of AAT vulnerability was based on the selection of key variables collected thr
