Modified posterior soft tissue release for management of severe knee flexion contracture.

A modified posterior soft tissue release has been used in treating severe flexion contracture of the knee in 30 patients (36 knees). The operative technique has been modified with respect to the skin incision, and lengthening of the medial hamstrings. It is performed without a tourniquet. Postoperative skeletal traction was applied with gradual increasing weight to rapidly correct residual deformity. The deformity was fully corrected in 30 knees. The only complication was superficial skin necrosis over a small area, which occured in two knees. In six knees, 10 degrees of flexion deformity pers…

Characterization of implants from Dupuytren's contracture tissue into the nude (athymic) mouse.

Dupuytren's contracture tissues were obtained from six patients as excess surgical material. Pieces of these tissues (a total of 38 implants) were placed into subcutaneous pockets in the suprascapular area of nude (athymic) mice. The objective was to determine whether the implant tissues would be maintained in the mouse with the characteristics of Dupuytren's tissue. The implants were removed for study at 14-179 days after implantation. Microvascular anastomosis between implant and host skin was established within the first 14 days. Histologic character and electron microscopic structure of th…

Neuroleptic malignant syndrome: caffeine contracture of single muscle fibers and muscle pathology.

The neuroleptic malignant syndrome (NMS) is similar to anesthesia-induced malignant hyperthermia (MH) in three major clinical features: hyperthermia, muscular rigidity, and myoglobinuria. In eight cases of NMS, we studied caffeine contracture of single skinned muscle fibers. Sensitivity of the sarcoplasmic reticulum to caffeine was abnormally increased in six of the eight cases. Morphologic studies showed type 2B fiber atrophy in all six cases examined, and there were necrotic fibers in two cases. Since skeletal muscle is affected in NMS, these patients may be susceptible to MH.

[Detection of peranesthetic malignant hyperthermia by muscle contracture tests and NMR spectroscopy].

To diagnose malignant hyperthermia susceptibility (MHS), caffeine and halothane contracture tests were performed on six patients. One of them, who presented a peroperative crisis, was recognized as MHS; the five others were negative (MHN). By means of 31P-NMR spectroscopy, the muscular energetic metabolism of these patients was studied during and after moderate exercise in normal and moderate ischaemic conditions. Metabolic abnormalities appeared in the MHS patient. It must be concluded therefore that malignant hyperthermia is a latent myopathy. 31P-NMR spectroscopy appeared to be a useful non…

Cultured myofibroblasts: a useful model to study wound contraction and pathological contracture.

From both human and animal tissues, myofibroblasts can be successfully cultured in vitro through multiple passages. As compared with cultured fibroblasts from control tissues, early passage myofibroblasts have slower growth cycles, as is typical of highly differentiated cells. Also in early passage, differences between myofibroblasts and fibroblasts in tissue culture in vivo can be preserved. Late passage animal studies suggest dedifferentiation in both morphology and growth cycle rate. As indicated by our data, which have been discussed in detail in our previous investigations, we believe tha

Treatment of axillary burn scar contracture using an arterialized scapular island flap.

A case of an axillary burn scar contracture treated with a scapular island flap is presented. We believe that this will be a useful addition to the treatment of burn scar contractures of the axilla.

Transcutaneous electrical nerve stimulation and extensor splint in linear scleroderma knee contracture.

A girl with knee flexion contracture secondary to linear scleroderma with hemiatrophy was treated with hot packs, gentle stretching and a serial posterior splint. The knee range of motion gained 35 degree over a period of 16 weeks. Nine months, later knee contracture had regressed. As an alternative method of treatment, continuous stretch was applied by using a front extension splint together with transcutaneous electrical nerve stimulation. The knee range of motion gained 55 degrees over a period of 6 weeks.

[The problem of contracture of small hand muscles after mid-hand replantation].

A longterm follow-up of a mid-palm re-attachment in which intrinsic tightness developed is presented. The initial bone shortening in this case was 7 mm. Because in 5 subsequent cases of mid-palm re-attachment with an initial bone shortening of 12-15 mm problems of intrinsic tightness did not occur, it is concluded that in re-attachment at this level the initial bone shortening should be at least 12 mm.

Focal contracture following injection of succinylcholine in patients with peripheral nerve injury.

Focal muscle contracture in the limb following sytemic administration of depolarizing neuromuscular blocking agents have been demonstrated experimentally with transection or crush injury of the nerve, but has rarely been observed clinically in patients with partial peripheral nerve injury. Three cases of spastic response in the hand and wrist are described in patients with subclinical chronic, subacute, and acute nerve injury, to document the occurrence of this phenomenon under various circumstances.

Treatment of Dupuytren's contracture. Long-term results after fasciotomy and fascial excision.

Of 359 hands treated surgically for Dupuytren's contracture, 135 were available for study after two years or more, often because of recurrence. Of the sixty-five hands treated by excision of the involved fascia (subtotal fasciectomy), 63 per cent had recurrences in the area operated on but only 15 per cent had sufficient deformity to warrant another operation. Of the forty-one hands treated by palmar fasciotomy, 43 per cent had recurrent deformities severe enough to warrant further surgical treatment. After palmar fasciotomy, improvement at the metacarpohalangeal joint persisted; but, as expec…

A Novel Surgical Approach to Induce an Ankle Joint Contracture in Rats.

Our primary aim was to establish a novel surgical immobilization approach to induce a persistent ankle plantar-flexion contracture in the Wistar rat. A stainless steel suture was placed in experimental animals (n = 14) and pulled taut between the calcaneal bone and the knee joint, restricting dorsal-flexion while still allowing for limited ankle plantar-flexion, muscle activity, and loading. The immobilization method was well tolerated; adverse events were limited and manageable. After 4 weeks, immobilization was released by cutting the suture. After 4 weeks of immobilization, ankle angle at 5…

Double Opposing Stiletto Flaps for Pediatric Finger Flexion Contractures.

PURPOSE: Pediatric finger flexion contractures can have wide transverse contracted volar tissue resulting in a pterygium or web of tissue extending across the affected joints. Here, we establish the double opposing stiletto (DOS) flap technique for releasing the skin of the flexion joint contracture transversely and then recruiting flap tissue from both the radial and ulnar aspects of the pterygium to cover the volar defect and with primary closure of flap donor sites. METHODS: The study includes pediatric patients with flexion joint contractures resulting from burn scar or camptodactyly etio…

Management of Complex Postburn Scar Contracture of the Dorsum of Pediatric Hand.

This case report details the challenging management of a 3-year-old girl with a severe left-hand extension contracture resulting from a scald burn. Delayed presentation prompted a 2-stage surgical approach. The first stage involved excising contracted skin and using a groin flap for reconstruction. Two weeks later, donor site closure was performed. The patient later underwent syndactyly release and extensor tenotomy, achieving significant metacarpophalangeal and proximal interphalangeal joint flexion. This case underscores the importance of burn injury intervention and reconstructive surgery f…

Congenital Contractures and Fractures: A Variant of Bruck Syndrome Type 2.

Bruck syndrome, an exceptionally rare autosomal recessive disorder, manifests as bone fragility and congenital joint contractures. This syndrome is recognized as a fusion of arthrogryposis multiplex congenita and osteogenesis imperfecta and is categorized into Types 1 and 2. Bruck syndrome Type 2 stems from a homozygous mutation in the PLOD2 gene and exhibits characteristics such as osteopenia, congenital contractures with pterygia, femoral bowing, club feet, postnatal shorty stature, severe limb deformity, and progressive scoliosis. In this report, we describe the case of an infant presenting…
