Association of the neuronal nicotinic acetylcholine receptor subunit alpha4 polymorphisms with febrile convulsions.

PURPOSE: The alpha4-subunit gene of the neuronal nicotinic acetylcholine receptor (CHRNA4) has been identified as the first gene underlying an idiopathic partial epilepsy syndrome in human autosomal-dominant nocturnal frontal lobe epilepsy. Studies provided evidence that the protein coded by CHRNA4 is one of the most abundant subunits of the neuronal nicotinic acetylcholine receptors in mammalian brains, and mutations of CHRNA4 seem to cause neuronal excitation. The CHRNA4 gene may have a role in the development of febrile convulsions (FCs), the majority of childhood seizures. This study asses

Aetiology of convulsions due to stellate ganglion block: a review and report of two cases.

Stellate ganglion block is a selective sympathetic block that affects the ipsilateral head, neck, upper extremity and upper part of the thorax. Convulsions are a recognized complication of intra-arterial injection during stellate ganglion block. As central nervous system toxicity depends ultimately on the concentration of the local anaesthetics presented to the brain, the likely causative factors are discussed as well as the types of toxic symptoms and their onset times. The paper considers the aetiological factors of such convulsions resulting from stellate ganglion block in two patients.

Acute pancreatic damage associated with convulsive status epilepticus: a report of three cases.

BACKGROUND AND OBJECTIVE: Renzapride (ATL-1251), a novel benzamide, is currently under clinical development for the treatment of irritable bowel syndrome (IBS). Previous in vitro and in vivo experimental studies have characterized renzapride as a full serotonin 5-HT(4) receptor agonist on the gut and a 5-HT(3) receptor antagonist. Clinical studies have confirmed the therapeutic efficacy, tolerability and safety of renzapride in patients with constipation-predominant IBS. This study set out to characterize the pharmacological profile of renzapride and its potential metabolic products at both 5-

Fluconazole-induced convulsions at serum trough concentrations of approximately 80 microg/mL.

On the basis of two case reports it is suggested that serious convulsions may occur in patients treated with flucoazole when serum trough concentrations exceed 80 microg/mL. The authors recommend monitoring fluconazole concentrations during high-dose therapy in patients with poor kidney function.

[Two cases of cranial pachymeningitis presenting only severe continuous headache or convulsion].

In cranial pachymeningitis, headache, cranial nerve palsies, and ataxia were the most common clinical manifestations. We reported two rare cases presenting only headache or convulsion as a main symptom. A 66-year-old man was suffering from severe continuous headache for years with no other clinical symptoms. Laboratory data revealed positive RA test and elevated RAPA. With oral steroid therapy, his headache was subsided. A 39-year-old woman visited us with convulsive attacks as only symptom. Biopsy of the thickened dura revealed granulated epithelioid cells without central necrosis. With antit

Febrile convulsions after 5 years of age: long-term follow-up.

In order to examine the natural history of febrile convulsions occurring after 5 years of age, we studied 44 children in whom febrile convulsions persisted after 5 years of age (group 1) and compared their development of unprovoked seizures with a group of 195 children in whom febrile convulsions occurred before 5 years of age (group 2). All subjects of group 1 were re-evaluated at the mean age of 13.4+/-1.7 years. The children in group 2 were followed up until they reached the same mean age as children in group 1 (13.1+/-2.3 years). In group 1, we found a higher percentage of unprovoked seizu

Acute carbamate and organochlorine toxicity causing convulsions in an agricultural pilot: a case report.

Agricultural pilots are involved in a particularly hazardous occupation with a high occupational mortality rate. They face many risks, not least of which is exposure to the agricultural chemicals that they spray. Many accidents and illnesses have been reported previously in the literature, attributable to the acute and chronic effects of chemical exposure, most notably the organophosphate family of insecticides. There are few reports of post-crash exposure to carbamate and organochlorine insecticides. This report documents the case of a professional agricultural pilot who, following an emergen

Scopolamine-induced convulsions in food given fasted mice: effects of clonidine and tizanidine.

We recently reported that scopolamine pretreated mice fasted for 48 h developed clonic convulsions soon after they were allowed to eat ad libidum. Pretreatment with MK-801, the non-competitive NMDA antagonist, decreased the incidence of these convulsions. We suggested that a possible scopolamine-induced glutamatergic hyperactivity could account for these convulsions. Using alpha2-agonists, clonidine, which has been shown to inhibit glutamate release, and tizanidine, the present study was performed to find some additional data for the role of glutamate in the underlying mechanism of scopolamine

[Magnesium and zinc levels in blood serum and cerebrospinal fluid in children with febrile convulsions].

Magnesium and zinc levels in blood serum and cerebrospinal fluid were estimated in 18 children aged 8 months-5 years with febrile convulsions. Control group consisted of 15 apparently healthy children in the same age. On the ground of ethical reasons no control values for cerebrospinal fluid were evaluated. The mean serum concentration of magnesium and zinc was significantly lower in the sick children (p < 0.001 and p < 0.05 resp.). The magnesium levels in cerebrospinal fluid were evidently lower than those quoted in the literature and the mean concentration of zinc was higher in comparison wi

Risk factors for a first febrile convulsion in children: a population study in southern Taiwan.

PURPOSE: To identify risk factors for a first febrile convulsion among 3-year-old children by a matched case-control population study. METHODS: All 11,714 neonatal survivors born in Tainan City between October 1989 and September 1991 were enrolled. At age 3, 10,460 children were available for telephone survey for febrile convulsions, and were confirmed by home visit interviews. Those without history of seizure were randomly matched to each febrile convulsion case by age, gender, and residence district. RESULTS: Two hundred fifty six children had febrile convulsions, and 218 of them and their

Clinical and immunological significance of neopterin measurement in cerebrospinal fluid in patients with febrile convulsions.

Neopterin is synthesized mainly by monocytes/macrophages and is considered to be a marker for activation of the cellular immune system. It has been reported that cerebrospinal fluid (CSF) neopterin levels are significantly higher in patients with bacterial meningitis than in those with aseptic meningitis or non-pleocytotic CSF. In this study levels of neopterin and interferon-gamma (IFN-gamma) were measured in children with non-pleocytotic CSF. The CSF neopterin levels were significantly higher in patients with typical febrile convulsions (FCs) (15.0 +/- 4.5 nmol/l) than in those with pyrexia

Bilateral tonic-clonic epileptic seizures in non-benign familial neonatal convulsions.

We report an electroclinical pattern considered characteristic of benign familial neonatal convulsions (BFNC) in two neonates without BFNC. Both neonates were products of uncomplicated pregnancies, labor, and deliveries. The cause of the seizures was not established. There was no family history of seizures or epilepsy. Seizures started on the second and third days after birth. Ten seizures were captured by continuous video-EEG telemetry. The electroclinical events began with generalized tonic posturing coinciding with the appearance of diffuse attenuation of the EEG activity. After several sec

Febrile convulsions in selected large families: a single-major-locus mode of inheritance?

As part of a genetic linkage study of febrile convulsions, families with at least four members with the disorder were identified. By applying known principles of human inheritance, possible modes of transmission of febrile convulsions and factors which might impede progress in understanding their inheritance were analyzed. In eight families, 75 of 333 members had febrile convulsions. Inheritance of the disorder occurred in 43, 51, and 46% of children, siblings and all first-degree relatives of probands respectively. Vertical transmission occurred through three to four generations, including fr

Electrophysiological and metabolic effects of a convulsant barbiturate on dissociated mouse primary sensory neurons.

1. The convulsant barbiturate 5-(2-cyclohexylidene-ethyl)-5-ethyl barbituric acid (CHEB) depolarizes dorsal root ganglion (DRG) neurons. We have applied microfluorimetric and whole-cell patch clamp techniques to investigate the mechanisms underlying this response in freshly dissociated mouse DRG cells. 2. Application of CHEB (2-200 microM) raised cytosolic calcium concentration ([Ca2+]i) rapidly and reversibly in 55% of eighty-three neurons tested. This population did not correlate with other classifications of sensory neurons based on either cell size or the expression of membrane currents. 3

Cerebrospinal fluid purine metabolites and pyrimidine bases after brief febrile convulsions.

Adenosine monophosphate, inosine monophosphate, inosine, adenosine, guanosine, adenine, guanine, hypoxanthine, xanthine, uric acid, and pyrimidines bases were determined in cerebrospinal fluid (CSF) of 52 children after simple febrile seizures and in a control group of 63 children. There was no statistically significant difference between the two groups for any of these metabolites, suggesting that simple febrile seizures (SFS) neither significantly disturb the metabolism of nucleotides, nucleosides, or bases nor significantly deplete neuron adenosine ATP levels. Therefore, they do not appear

Efficacy of injectable anticholinergic drugs against soman-induced convulsive/subconvulsive activity.

Six FDA approved, injectable compounds [benztropine (BZT); biperiden (BIP); dicyclomine (DCL); l-hyoscyamine (HYO); orphenadrine (ORP); scopolamine (SCP)] were each compared to diazepam (DZ, the standard) in male guinea pigs against ongoing soman-induced convulsive or sub-CV (CV/sub-CV) activity. Three trained graders concurrently assigned CV/sub-CV scores to each animal based on signs of intoxication at various times post-soman. Animals received (im) pyridostigmine (26 micrograms/kg) 30 min before soman (56 micrograms/kg; 2 x LD50), atropine (2 mg/kg) admixed with 2-PAM (25 mg/kg) at one min

Influence of chronic aminophylline on antielectroshock activity of diazepam and aminophylline-induced convulsions in mice.

The effects of chronic administration of aminophylline (AMPH; 50 mg/kg, twice daily for 14 consecutive days) were studied on both antielectroshock efficacy of diazepam (DZP) and convulsive activity of AMPH in mice. AMPH injected acutely at a dose of 50 mg/kg significantly reduced anticonvulsant action of DZP elevating ED50 from 10.9 (control) to 15.9 mg/kg (p < 0.01). After the administration of AMPH for 3 days, ED50 value was still higher compared with control. Chronic treatment with AMPH resulted in further increase of ED50 of DZP, which was 20.2 mg/kg, and this elevation was significant not

[A case report of hypoplasia of the left internal carotid artery manifested as convulsion attack].

A case of hypoplasia of the left internal carotid artery manifested as convulsion was reported. The features and embryological process were reviewed. The case was that of a 25-year-old female who had been afflicted with repeated convulsion for 16 years. CT scan showed a dilated left posterior communicating artery. Cerebral angiography revealed abrupt narrowing of the left internal carotid artery from a few centimeters distal to the bifurcation of the common carotid artery and terminating at the C4 portion. The left cerebral hemisphere was supplied, through well-developed anterior and posterior
