Cryoamputation or physiologic amputation has been used at our institution for more than 30 years. From 1971 through 1989, 891 major lower extremity amputations were performed in 750 patients. With the use of dry ice or mechanical refrigeration, 320 (36%) physiologic amputations were performed in 292 patients. The results of resection of the liver in repeated operations on patients with parasitic disease of the liver are discussed. Operations were conducted on 22 patients with alveococcosis and 7 patients with echinococcosis. By means of the method of atypical liver resection radical operations were performed on 20 patients (69%) and palliative resections of the liver in 9 patients (31%). The palliative resections were combined with cryodestruction of the remaining parasitic tissue, which improved the therapeutic effect significantly and prolonged the patients' survival. Intraocular pressure (IOP) was measured before and after peripheral retinal cryopexy given for lattice degeneration and small retinal breaks. During the period of observation the IOP of treated eyes decreased, suggesting that freezing of areas remote from the ciliary body may lower IOP through mediation and/or transscleral outflow.

We report four patients who had glaucoma in association with the Maroteaux-Lamy syndrome. Two of these patients had acute angle-closure glaucoma, one had chronic angle-closure glaucoma, and one was treated for open-angle glaucoma; however, the angle structures could not be seen by gonioscopy. Standard peripheral iridectomy was inadequate for treatment of this angle-closure glaucoma because of the considerably increased thickness of the peripheral cornea that occurs in these patients. Plantar and common warts can be cured with great efficacy by the intralesional injection of bleomycin using the jet injector. Traditional keratolytic treatment and electro-desiccation are also still in use. Condyloma acuminata respond to podophyllin combined with cryotherapy. It is advisable to use the CO2 laser in more extensive infections. Interferon injections also may be helpful in these cases. Plane warts can be treated by tretinoin and 5-fluorouracil. In case of diffuse verruca sensitization with diphenylcyclopropenone can be considered.

Patients with common hand warts and simple plantar warts attending a general practice wart clinic in Northern Ireland were assigned to one of three treatment groups - liquid nitrogen applied weekly, daily application of wart paint (lactic acid one part, salicylic acid one part, collodion four parts), or a combination of the two. Cryotherapy for the relief of pain is widely used in many conditions. The results of 83 cryotherapy sessions in 29 patients with paroxysmal trigeminal neuralgia are reviewed over a five year period. Sixty three per cent of treated nerves, 41% of patients were pain free over one year and there was no permanent sensory loss.

A case of tracheo-bronchial lipomatosis is reported. Bronchial lipomas are benign fatty tumours which frequently result in bronchial superinfection or dyspnoea. The radiological image is often one of atelectasis, and the diagnosis is made at endoscopy. Treatment, rendered necessary by destruction of the underlying lung, is either endoscopic or surgical. Multiple localizations are exceptional and raise special therapeutic problems.

Fifteen women diagnosed with cervical stenosis were treated by vaporization of scar tissue using the carbon dioxide laser. Ninety-three percent of the cases demonstrated binding constriction of the cervical os to a 2-mm diameter or less and 40% of the cases had either a pinpoint opening or total endocervical obliteration. The sciatic nerve was exposed to cryoinjury at different freezing patterns in albino rats and rabbits and the frozen nerves were serially examined with electron microscopy from the time of cryolitic lesion (--60 degrees C for 3 minutes) for up to 28 days. We created temporary scleral buckles in 100 patients who had retinal detachments by inserting a balloon into the parabulbar space for one week. Cryopexy- or laser-induced adhesions were needed to make the attachments permanent. In 90 patients the balloon closed the retinal break and reattached the retina. In six instances the break reopened because of traction or poor adhesion and required a scleral sponge operation to reattach the retina permanently. The low complication rate may compensate for the increased risk of redetachment.

Severe diabetic retinopathy with neovascular proliferation may produce severe vitreous hemorrhage that prevents laser therapy. Retinal ablation, if indicated, can be partly accomplished by cryopexy. In a small series of cases, cryotherapy to the retina was not apparently harmful and sometimes seemed beneficial. We observed 341 patients who had received cryotherapy for lattice degeneration in order to identify possible late complications. Sequelae such as retinal tears posterior to an operculum or flap tears within treated areas showed that treatment did not necessarily prevent subsequent vitreous traction. Cryotherapy was utilized in 5 eyes of 4 patients with recurrent toxoplasmosis retinochoroiditis. Each patient showed slow gradual improvement in vision over several months following cryotreatment where previous medical management had failed. No recurrences have been noted during the follow-up period of 3 months to 3 years.
