A 42-year-old male was referred to our hospital with the main complaint of gross hematuria. A submucosal tumor was found on top of the bladder dome. A cystoscopy and magnetic resonance image also revealed a urachal cyst at the position of the urachus. We performed a transurethral biopsy and the pathological examination was negative for malignancy. We executed laparoscopic urachal resection and partial cystectomy as well. A 22-year-old African American man with recurrent episodes of gross hematuria for 6 months presented to the clinic for evaluation. A thorough history revealed that the patient emigrated from Mozambique to the United States 12 years ago. Urine culture was negative for a urinary tract infection. Cystoscopy revealed 4 lesions in the bladder. Biopsy of the bladder lesion revealed severe cystitis and Schistosoma haematobium. The patient later confirmed that he used to swim in rivers and streams back in Africa. He completed a course of praziquantel and his gross hematuria resolved.

Leiomyomas of the bladder constitute <0.5% of all bladder tumors, with about 250 cases reported. Most patients present with urinary frequency or obstructive urinary symptoms. Primary malignant melanoma of the urinary bladder is a rare lesion. We report the case of a 78-year-old male with no previous history of cutaneous melanoma who presented with hematuria.
