Innovative grid positioning system (GPS) guidance for minimally invasive spinal surgery.

Symptomatic degenerated spinal discs and spinal stenosis are common problems that can often be treated conservatively, but some require decompressive spinal surgery for relief. Traditional open spinal discectomy is associated with significant tissue trauma, higher morbidity and complication rates, a longer convalescence, and even destabilization of the spine. The trend of spinal surgery is rapidly moving toward less traumatic minimally invasive spine surgery (MISS).1,2 The problem that faces the surgeon performing endoscopic MISS is that it is done with limited surgical exposure and visualizat

Return to play and outcomes of surgically treated upper limb nerve entrapment in athletes: a systematic review.

PURPOSE: Athletes face a higher risk of upper limb nerve entrapment due to repetitive stress, trauma, and biomechanics. Diagnosis is challenging, and delayed treatment can impair performance. When conservative care fails, surgery may be needed to restore function and enable return to play (RTP). METHODS: This systematic review adhered to PRISMA guidelines and evaluated surgical outcomes, RTP rates, and complications in athletes with upper limb nerve entrapment. A comprehensive search was conducted using MeSH terms and keywords for surgical interventions, nerve entrapment syndromes, and sports

Cervical Radiculopathy and Vertebrobasilar Insufficiency Secondary to Intraneural Foramen Vertebral Artery Loop: A Case Report.

This case report describes a 40-year-old male who presented with chronic neck pain radiating to the left upper limb, associated with weakness and numbness. He also had symptoms of vertebrobasilar insufficiency. Imaging revealed an intraneural foramen vertebral artery (VA) loop compressing the C3 nerve root. Conservative management was ineffective, prompting surgical decompression via a left C2-C3 facetectomy and foraminotomy. The patient experienced immediate pain relief and gradual improvement in weakness, with complete resolution of symptoms at the six-month follow-up. This case highlights t

Anterior Inferior Iliac Spine Deformity as a Cause for Extra-articular Hip Impingement in Young Athletes After an Avulsion Fracture: A Case Report.

Extra-articular hip impingement from prior traumatic injury to the anterior inferior iliac spine (AIIS) is an uncommon cause of groin pain in young athletes. Currently, the most common treatment for this injury is arthroscopic decompression. However, hip arthroscopy is not universally available and requires advanced skills. We report 2 cases of the development of extra-articular hip impingement from unusual bony exostoses off the AIIS after traumatic injury in 2 young athletes who underwent open surgical resection. The multidisciplinary sports medicine team should be aware of the development o

Patient with congenital optic nerve meningocele presenting with left orbital cyst.

INTRODUCTION: Meningocele of the optic nerve is a rare condition defined as dilatation of the optic nerve by spinal fluid not due to other pathology, classically presenting with headaches or progressive visual decline and usually follows a rapidly progressive course. DISCUSSION: Surgical decompression is the standard treatment with improvement or arrest of progression in most cases. We describe a case of a child with multiple congenital anomalies including a left optic nerve meningocele that progressively expanded and caused displacement of the orbit laterally resulting in severe cosmetic def

Acute respiratory arrest following partial suboccipital cranioplasty for cerebellar ptosis from Chiari malformation decompression.

Cerebellar ptosis is a rare complication following Chiari malformation decompression, and generally is the result of a very large suboccipital craniectomy. This can lead to the descent of the cerebellum through the craniectomy defect, which in turn may result in cerebellar herniation through the surgical defect as well as the reestablishment of contact between the cerebellar tonsils and the brainstem. In addition, dorsal adherence of the herniated cerebellum to the dura mater or dural patch and an associated obstruction of cerebrospinal fluid flow at the cervicomedullary junction may ensue. Su

Minimally invasive operative management for lumbar spinal stenosis: overview of early and long-term outcomes.

Standard open posterior decompression is well established and familiar to virtually all spine surgeons. However, this traditional surgical treatment of lumbar spinal stenosis (LSS) is often associated with significant postoperative pain, disability, and dysfunction. This article reviews the use of a minimally invasive microendoscopic approach for bilateral decompression of lumbar stenosis by way of a unilateral approach. This technique has been shown to provide symptomatic relief equivalent to that of open discectomy, with significant reductions in operative blood loss, postoperative pain, hos

[Research progress of minimally invasive decompression techniques for the treatment of lumbar spinal stenosis].

The minimally invasive decompression technique represents a novel approach for the treatment of lumbar spinal stenosis(LSS). This method minimizes peripheral tissue damage, decreases the incidence of postoperative complications, optimizes the alleviation of nerve root compression, and expedites patient recovery. Therefore, there are various opinions on the minimally invasive decompression surgery technique for the spine both domestically and internationally today. To enable patients to select an appropriate surgical plan with optimal therapeutic outcomes, this article offers a comprehensive an

The Blocking Flap for Ulnar Nerve Instability After In Situ Release: Technique and a Grading System of Ulnar Nerve Instability to Guide Treatment.

In situ ulnar nerve release has been gaining popularity as a simple, effective, and low-morbidity procedure for the treatment of cubital tunnel syndrome. One concern with the technique is how to manage the unstable ulnar nerve after release. It is unclear how much nerve subluxation will lead to problems and surprisingly there is no grading system to assess ulnar nerve instability. I propose such a grading system, as well as a new technique to stabilize the unstable ulnar nerve. The blocking flap technique consists of raising a rectangular flap off the flexor/pronator fascia and attaching it to

Minimally Invasive Palmaris Longus Abductorplasty for Severe Carpal Tunnel Syndrome.

Camitz abductorplasty is the most commonly used tendon transfer in patients with severe carpal tunnel syndrome with significant muscle wasting and loss of opposition. This procedure requires a long incision in the palm to harvest a strip of palmar aponeurosis to lengthen the palmaris longus tendon, allowing it to reach the abductor pollicis brevis insertion. Several complications have been attributed to this extensive dissection in the palm. We describe a minimally invasive palmaris longus abductorplasty using a strip of free flexor carpi radialis tendon graft to achieve the necessary length.

[Current situation of surgery for traumatic optic neuropathy].

Although corticosteroid therapy remains the mainstream of various treatment procedures in traumatic optic neuropathy, some patients are still unable to benefit by this procedure. Therefore, optic nerve decompression has become an important choice for treating traumatic optic neuropathy. However, several problems have been raised on optic nerve decompression, such as who is suitable for this operation; as well as when to do and how to do this operation. The answers to these questions are controversial. We systematically review the current clinical evidence, and suggest that surgical decompressi

Minimally invasive lumbar decompression.

With the increasing elderly population, the number of patients presenting with symptoms secondary to lumbar stenosis can be expected to increase accordingly. Therefore, treatment of this disease process should become more prevalent, and the minimally invasive techniques offer another treatment option. As surgeons become more experienced in minimally invasive techniques, the reported advantages of the minimal access surgery, including reduction in soft tissue injury, less blood loss, shorter hospitalization, and faster recovery, should make this an attractive alternative to traditional open sur

Escharotomy for the Face: Facial Aesthetic Subunit Principle-Based Approach.

Escharotomy is the relaxation of an eschar through longitudinal or horizontal incisions in order to protect regional perfusion. In peripheral areas, such as limbs, trunk, and neck, eschar pressure poses significant issues; it causes circulatory disorder in limbs and potential limb loss, inadequate thoracic expansion in the thorax, and perfusion and oxygenation problems in the neck. To prevent complications, a basic rule of burn surgery is to perform escharotomy incisions quickly and without hesitation. However, the face is not an area in which eschar formation is commonly seen due to its robus

Minimally invasive lumbar decompression: a review of indications, techniques, efficacy and safety.

Lumbar spinal stenosis is a common degenerative spine condition. In properly selected patients, minimally invasive lumbar decompression (mild®) may be an option to improve outcomes. This review provides an in-depth description of the mild procedure and a comprehensive examination of safety and efficacy. Two randomized controlled trials, together with 11 other controlled clinical studies, have established the efficacy of mild, which is a minimally invasive procedure that does not involve implants and has demonstrated excellent efficacy and safety. With an established safety profile equivalent t

Closure of a giant anterior sacral meningocele with an omental flap in a patient with Marfan syndrome: case report.

Anterior sacral meningoceles (ASMs) are rare lesions often associated with connective tissue disorders. These lesions are typically treated posteriorly via closure of the dural stalk. However, given their insidious nature, ASMs can be quite large on presentation, and this approach may not provide adequate decompression. In this case report, the authors describe the successful treatment of a large ASM through drainage and watertight closure of the cyst with an omental flap. A 43-year-old woman with a history of Marfan syndrome and a large ASM was referred for neurosurgical intervention. The ASM
