A 30-year-old Chinese female presented with progressive periorbital swelling of unknown cause and redness of her left eye. She was given an antibiotic eyedrop and oral cloxacillin to treat periorbital cellulitis. The clinical picture did not improve and she returned with swelling of the fellow eye. We stopped the medication and sent her to a dermatologist because of the abnormal periorbital skin pattern. Her diagnosis was Paederus dermatitis. Intramuscular dexamethasone was administered due to severe periorbital edema. The skin lesion was improved one week later. The ophthalmic manifestations …

Nickel is the most commonly detected cutaneous allergen on patch testing in the United States for children. The ubiquitous nature of nickel as a hardening agent in metal products makes avoidance difficult. Allergy in childhood can be hard to recognize, especially when a strong idiopathic response is noted. Although the standard belt buckle and jewelry trigger still occur, newer sources include technology equipment, such as cases for mobile phones, computers, and tablet devices This contribution reviews the various clinical appearances of nickel contact allergy in childhood, as well as strategi

BACKGROUND: Atopic dermatitis (AD) is a chronic inflammatory skin disease and can be the first step of the atopic march.
OBJECTIVE: In this retrospective study, we analysed the immunological and clinical patterns of AD in a group of children affected by the disease since their first years of life, in order to evaluate if and how these patterns can change over time, and to identify biomarkers that can possibly correlate with the clinical phenotype.
METHODS: We enrolled Caucasian children with diagnosis of AD performed by a specialist on the basis of Hanifin and Rajka's criteria and with a fir…

The best way to be acquainted with the various facets of clinical lesions of allergic contact dermatitis (ACD) is to refer to the concept of the ACD syndrome. Obtaining differential diagnosis between irritant contact dermatitis and ACD based on clinical grounds is difficult. Moreover, hand eczema and photoalleregic contact dermatitis deserve special attention. The patch test remains the "gold standard" diagnostic tool, but many other very useful complementary tests have been developed in recent years.

Allergic contact dermatitis is an inflammatory skin disease (delayed type hypersensitivity reaction) that accounts for up to 20% of all childhood dermatitis. Allergic contact dermatitis represents a clinical manifestation of contact sensitization and usually occurs at skin sites that have come into contact with the allergen. The clinical features of allergic contact dermatitis are itchy eczematous lesions. Prevalence of contact sensitization varies between 27% and 96% of children with suspected contact dermatitis. The relationship between contact sensitization and atopic dermatitis has been wi…

Contact dermatitis--irritant contact dermatitis, allergic contact dermatitis and protein contact dermatitis--are the most common occupational skin diseases, most often localized to the hands. Contact urticaria is rarer The main occupational irritants are wet work, detergents and disinfectants, cutting oils, and solvents. The main occupational allergens are rubber additives, metals (chromium, nickel, cobalt), plastics (epoxy resins, acrylic), biocides and plants. Diagnosis is based on clinical examination, medical history and allergy testing. For a number of irritating or sensitizing agents, ir…

Chlorhexidine is a commonly used antiseptic agent in the health-care setting. Although exposure to chlorhexidine is very common, allergic contact dermatitis (ACD) is rarely reported. We report a case series of ACD to chlorhexidine in health-care workers and discuss our rates of allergy to chlorhexidine, from patch-testing performed at the Skin and Cancer Foundation, Melbourne, Australia. Of 7890 patients patch-tested, 840 patients were tested to 0.5% chlorhexidine diacetate with 28 (3%) positive reactions, 13 (2%) of which relevant to their presenting dermatitis. Altogether 1565 patients were …

The history of contact dermatitis (CD) is inseparable from the history of the patch test, and the patch test is inseparable from the pioneer in the field, Josef Jadassohn (1860-1936). Despite the fact that we have been diagnosing, treating, and investigating the condition for more than 100 years, there are still many unsolved questions and controversies, which show no signs of coming to an end in the foreseeable future. This contribution reviews and highlights some of the disagreements and discrepancies associated with CD. For example: • What is the real sensitizer in balsam of Peru, one of th…

Protein contact dermatitis is a skin condition not well known and underdiagnosed by dermatologists, resulting from an IgE-mediated allergic reaction. Clinically it presents as a chronic hand and/or forearms eczema of occupational origin, especially in professionals who work as food handlers. Epicutaneous tests are negative, and to diagnose this condition it is necessary to perform immediate-type allergy tests. The most sensitive and practical is the prick-by-prick test with food that the patient refers to cause intense itching after immediate skin contact. Treatment is symptomatic, and it is m…

PURPOSE OF REVIEW: Both atopy and diseases from the spectrum of dermatitis and eczema are among the most frequent clinical problems worldwide; nevertheless, they are still poorly defined and too frequently misdiagnosed. In the present review, studies pertinent to this topic were systematized and critically assessed with particular attention to definitions of relevant diseases.
RECENT FINDINGS: The overall message from the research done to date is that various types of dermatitis frequently coexist. Atopy and contact allergy seem independent, while there is insufficient data to state upon the …

Contact dermatitis is a common dermatologic condition that can result from exposure to allergens at home or at work. Cosmetics represent a large diverse group of products that Americans apply to their skin to treat disease or enhance beauty. With increased use of cosmetics, the rate of sensitization to many allergenic components has increased. We review the more common allergens present in cosmetics as well as the types of cosmetics that are known to contain them. With proper education and patch testing, dermatologists will be able to identify contact allergies to cosmetic ingredients and help…

Dermatitis herpetiformis is an autoimmune bullous disease that is associated with gluten sensitivity which typically presents as celiac disease. As both conditions are multifactorial disorders, it is not clear how specific pathogenetic mechanisms may lead to the dysregulation of immune responses in the skin and small bowel, respectively. Recent studies have demonstrated that IgA and antibodies against epidermal transglutaminase 3 play an important role in the pathogenesis of dermatitis herpetiformis. Here, we review recent immunopathological progress in understanding the pathogenesis of dermat…

Contact dermatitis is defined as "airborne" when the causative factor is present in the environment and may determine irritative or allergic skin reactions. It is often work-related. In this review of the literature, we focus our attention on airborne contact dermatitis due to pharmaceutical compounds. Contact reactions to medications, often occupation-related, occur mainly in two exposed groups: employees of pharmaceutical industries involved in the production of the drugs and healthcare workers who use the drugs for therapeutic aims.

Prevention of occupational contact dermatitis is of utmost significance for both insurers and legislators because it can preserve the individual's ability to work and result in decreased costs for public health. In the last 20 years, many concepts for educational interventions and a multitude of teaching aids have been developed by insurance associations and public institutions from different countries (eg, the United States, Sweden, Denmark, Switzerland, and Germany). For didactic purposes, terms of different levels of prevention have been inaugurated in northern Europe in the context of occu…

Allergic contact dermatitis is the consequence of an immune reaction mediated by T cells against low molecular weight chemicals known as haptens. It is a common condition that occurs in all races and age groups and affects the quality of life of those who present it. The immunological mechanism of this disease has been reviewed in recent decades with significant advance in its understanding. The metabolism and pathway of the haptens as well as the activation and mechanism of action of the cells responsible for both the immune reaction and its completion are discussed in this article.
