Presynaptic compensation sustains neuromuscular transmission in the mouse diaphragm at a late stage of aging.

The diaphragm is the primary respiratory muscle. Like other skeletal muscles, it is subject to age-related changes, but it may be more resistant to sarcopenia due to compensatory mechanisms operating at the neuromuscular junction (NMJ). Here, we examined key parameters of neuromuscular transmission in the diaphragm of 24- and 3-month-old mice. In aged NMJs, a decrease in the amplitude of evoked postsynaptic responses in combination with a much greater reduction in miniature end-plate amplitude occurred. This was accompanied by an increase in spontaneous exocytosis intensity, presynaptic action…

The contribution of mechanical ventilation to critical illness-associated diaphragm dysfunction: a critical appraisal.

Although diaphragm dysfunction is increasingly recognized as a major contributor to morbidity and mortality in the intensive care unit, the physiological mechanisms leading to its development in mechanically ventilated patients are incompletely understood. Mechanical ventilation is often recognized as the main cause of the problem, delineating a paradigm known as ventilator-induced diaphragm dysfunction (VIDD). Yet, evidence on this causal relationship in ventilated critically ill patients is scarce, with direct experimental data mostly coming from animal models or brain-dead organ donors. In …

Necrosis of the diaphragm pleural empyema and pneumomediastinum as rare complications of appendicitis: A case report.

BACKGROUND: Appendicitis is one of the most common surgical emergencies in general surgery. To our knowledge, this is the first reported case of diaphragmatic necrosis associated with appendicitis. CASE: A 38-year-old male north African patient with no past medical history presented with fever and right lower quadrant pain, for which he started oral self-medication with ibuprofen. Clinical and radiological examinations revealed perforated appendicitis with right-sided pleural empyema and pneumomediastinum. During the initial surgical procedure, we performed an appendicectomy, inserted a chest…

Computed Tomography Assessment of Diaphragm Thickness in Myasthenia Gravis With Clinical and Functional Correlations.

Myasthenia gravis (MG) is an autoimmune disorder of the neuromuscular junction in which respiratory muscle involvement, particularly of the diaphragm, constitutes the principal risk factor for myasthenic crisis and respiratory failure. Objective structural evaluation of the diaphragm in MG remains incompletely standardized. This narrative review synthesizes published evidence on diaphragm thickness measurement in generalized MG, with attention to measurement reproducibility, normative reference values, structural-functional linkages, and potential clinical applications of computed tomography (…

An unexpected finding of cardiac injury penetrating the diaphragm in a suspected abdominal trauma case.

We report a rare case of penetrating abdominal trauma involving right ventricular injury. An 81-year-old man presented with a stab wound in the upper abdomen and hemodynamic instability. Hemorrhagic shock was initially suspected to be due to intra-abdominal organ damage. Emergency laparotomy revealed no evidence of intra-abdominal organ injury; however, subsequent thoracotomy and sternotomy identified cardiac tamponade from right ventricular injury caused by a penetrating object that traversed the diaphragm. Prompt surgical intervention enabled successful resuscitation. This case highlights th…

Intermittent dizziness due to a diaphragm of the common carotid artery successfully treated by stenting.

Carotid diaphragm is considered a form of fibromuscular dysplasia (FMD) that may result in the narrowing of blood vessels, potentially increasing the risk of stroke. More than 50 cases of carotid diaphragm have been reported, most cases of which are located in the internal carotid artery. Patients are usually middle-aged and initially manifest as syncope, tinnitus, and ischemic stroke. In this paper, we report a rare case of septal aneurysm in the common carotid artery (CCA). This is the fourth case reported in the literature.

Descriptive Epidemiology and Prognostic Significance of Diaphragm Thickness in the General Population: The Nagahama Study.

BACKGROUND: Diaphragm thickness is a potential marker of sarcopenia in addition to muscle mass and strength at extremities. We aimed to clarify the descriptive epidemiology and prognostic significance of diaphragm thickness in the general population. METHODS: The study participants were 3324 community residents (mean age: 61.4 ± 12.8 years) who participated in a longitudinal cohort study. Clinical parameters were obtained during the follow-up survey of the study population. Diaphragm thickness was measured from B-mode ultrasound images obtained in a supine position. Clinical and physical fact…

In vivo shear wave elasticity imaging for assessment of diaphragm function in muscular dystrophy.

Duchenne muscular dystrophy (DMD) causes patients to suffer from ambulatory disability and cardiorespiratory failure, the latter of which leads to premature death. Due to its role in respiration, the diaphragm is an important muscle for study. A common method for evaluating diaphragm function is ex vivo force testing, which only allows for an end point measurement. In contrast, ultrasound shear wave elastography imaging (US-SWEI) can assess diaphragm function over time; however, US-SWEI studies in dystrophic patients to date have focused on the limbs without preclinical studies. In this work, …

A soft and ultrasensitive force sensing diaphragm for probing cardiac organoids instantaneously and wirelessly.

Time-lapse mechanical properties of stem cell derived cardiac organoids are important biological cues for understanding contraction dynamics … sensors that are typically rigid, planar and bulky. Here, we present a soft resistive force-sensing diaphragm based on ultrasensitive resistive nanocracked platinum film, which can be integrated into an all-soft culture well via an oxygen plasma-enabled bonding process. We show that a reliable organoid-diaphragm contact can be established by an 'Atomic Forc …

The relationship between diaphragm thickness and the severity of the disease in pregnant patients with Covid-19.

OBJECTIVE: We aimed to investigate whether there is a relationship between diaphragm thickness and disease severity in Covid-19 pregnant subgroups. MATERIAL AND METHODS: In this prospective study 100 pregnant patients were enrolled. Thickness of the diaphragm muscle at end-expiration was measured using B-Mode US. Hemoglobin,WBC, NLR, procalcitonin and LDH levels were measured. RESULTS: There was a statistically significant difference between the groups in terms of diaphragm thickness, and the diaphragm thickness was thinner in the severe disease group (p < 0.001). There was no statistically …

Diaphragm function in acute respiratory failure and the potential role of phrenic nerve stimulation.

PURPOSE OF REVIEW: The aim of this review was to describe the risk factors for developing diaphragm dysfunction, discuss the monitoring techniques for diaphragm activity and function, and introduce potential strategies to incorporate diaphragm protection into conventional lung-protective mechanical ventilation strategies. RECENT FINDINGS: It is increasingly apparent that an approach that addresses diaphragm-protective ventilations goals is needed to optimize ventilator management and improve patient outcomes. Ventilator-induced diaphragm dysfunction (VIDD) is common and is associated with inc…

Surgical Management and Reconstruction of Diaphragm, Pericardium and Chest Wall in Mesothelioma Surgery: A Review.

Mesothelioma is an aggressive disease arising from parietal pleura. Surgery is a valuable option in the frame of a multimodality treatment. Several surgical approaches have been standardized with the aim of a macroscopic complete resection; these often require homolateral diaphragm and pericardial resection and reconstruction. Extrapleural pneumonectomy (EPP) and extended pleurectomy decortication (EPD) have been recognized as radical surgical procedures. Nevertheless, both operations are technically challenging and associated with a significant rate of peri-operative morbidity and non-negligi…

Endoscopic Lung Volume Reduction Results in Improvement of Diaphragm Mobility as Measured by Sonography.

BACKGROUND: Hyperinflation in patients with pulmonary emphysema is an important cause of reduced diaphragm mobility. We investigated whether endoscopic lung volume reduction (ELVR) could improve diaphragm mobility. METHODS: Diaphragm mobility data obtained by sonography from 44 patients were compared before and 3-6 months after ELVR therapy with a Spiration™ valve system. These patients were asked whether they wanted this procedure again after they had learned of their treatment outcome; this was a subjective indicator of outcome. Lung function parameters and blood gases were also measured. …
