Tandem Gianturco expandable metallic stents (GEMSs) with barbs were inserted to dilate the left main bronchus of a 38-year-old man who had developed left bronchostenosis due to bronchial tuberculosis. Through the use of these GEMSs, ventilation in the left lung remarkably improved; however, the central airway resistance was found to be greater than that exhibited by the normal side and the mucociliary transport function was reduced.

Congenital polycystic dilatation of the intrahepatic biliary ways (Caroli's disease) is a rare disease. The "pure" type and the one associated with congenital hepatic fibrosis are accepted. Although the current exploration methods, its diagnosis is rare before the surgical procedure. Treatment of the diffuse pure types and the ones associated with hepatic fibrosis is difficult and the outcome is bad. A case of the less frequent type is communicated, in which, as a novelty, intrasurgical echography was used in its treatment.

The management of cystic diseases of the liver requires an understanding of their pathophysiology and natural history. Surgery for congenital solitary cysts and polycystic disease should be reserved for patients with significant symptoms. Caroli's disease requires careful preoperative evaluation and planning and long-term follow-up. Surgery for echinococcal liver cysts should be performed before complications of rupture and superinfection develop.

Two patients with Caroli's disease are reported who did not have renal abnormalities or hepatic fibrosis. Extra hepatic complications of Caroli's disease in these two patients were best evaluated by computed tomography (CT). Although ultrasound evaluates the ductal abnormalities of Caroli's disease well, it does not delineate subphrenic or subhepatic abscesses, pancreatitis, pancreatic phlegmon, or pseudocysts as well CT.

We have seen 21 patients with unilateral congenital giant megaureter. Age ranged from 2 months to 8 years. We studied 80 children who had severe chronic constipation that was refractory to dietary, medicinal, and psychiatric therapy by means of barium enemas, rectal biopsies, and manometry. Four patients had Hirschsprung's disease and three patients had segmental dilatation of the colon. The remaining 73 patients had radiologically dilated anorectums with inpaction.
