A case of a male newborn infant with spontaneous perforation of Meckel's diverticulum is reported. The clinical course consisted of delayed occlusive disorders and bloody stools following 36 h of normal gastrointestinal transit, without pneumoperitoneum and infection. Pathogenesis of the perforation is discussed and the importance of early diagnosis, especially in maternity hospitals, is emphasized.

The serial scintiphotography following intravenous injection of Tc99m-ertechnetate was used for examination of 50 children aged from 11 months to 14 years. The method is founded on the Tc99m-pertechnetate property of selective accumulation in the gastric mucous membrane and in Meckel's diverticulum when the latter contains the ectopic gastric mucous membrane. In diagnosing a Meckel's diverticulum besides the conventional plain films of the abdomen with and without contrast and the angiographic special attention is called to the scanning with technetium. This examination can be very helpful in cases of children with gastrointestinal bleeding episodes in the diagnosis of a Meckel's diverticulum, if it contains ectopic gastric mucosa.

Solitary diverticulum of the caecum is very rare and assumes clinical interest only when inflamed. Preoperatively the condition is virtually impossible to distinguish from acute appendicitis, and even during operation its differentiation from carcinoma is difficult. A case of Meckel's bleeding diverticulum diagnosed by emergent arteriography is presented. Arteriography revealed an extravasation of contrast medium into the distal ileum and an abnormal ileal artery which passed to the antimesenteric border of the ileum. The diverticulum was demonstrated by double contrast study of the small intestine. The merits of emergent arteriography and other examinations for diagnosing Meckel's diverticulum are discussed.

An infrequent case of puncture of Meckel's diverticulum whose aetiopathogenesis was attributed to a segmentary intestinal dyskinetic phenomenon is reported. The pneumoperitoneum consequent on the puncture was not associated with peritonitis and histology showed that the diverticulum did not show either inflammatory phenomena or areas of ectopic mucosa, namely none of the causes which are usually at the origin of Meckel's diverticulum puncture.

The clinical illness observed in a 5-month-old infant with Meckel's diverticulum is described. A sodium pertechnetate radioisotope scan was employed to confirm the clinical impression. The technical procedure, evaluation, and importance of the radioisotope study is discussed.
