Updates on Radial Head Arthroplasty in Trauma.

The radial head plays a crucial role in maintaining normal elbow kinematics, and radial head arthroplasty (RHA) can help restore normal elbow kinematics after radial head fracture. Most modern implants are made of polished metal alloy with stems that may be smooth, press-fit, or cemented. There is no consensus as to the superiority of smooth versus press-fit stems, and both can result in good clinical outcomes. Cemented stems are no longer indicated in routine RHA. Correct implant positioning is critical, and numerous methods of assessment have been described.

Three- and four-part fractures have poorer function than one-part proximal humerus fractures.

BACKGROUND: Locking plates have become a commonly used fixation device in the operative treatment of three- and four-part proximal humerus fractures. Examining function in patients treated nonoperatively and operatively should help determine whether and when surgery is appropriate in these difficult-to-treat fractures. QUESTIONS/PURPOSES: We compared functional scores, ROM, and radiographs in patients with one-part proximal humerus fractures treated nonoperatively to those in patients with displaced three- and four-part proximal humerus fractures treated with open reduction and internal fixat

Ipsilateral injuries of the elbow and forearm in children.

Fracture of the forearm with ipsilateral injury of the elbow joint is a rare traumatic entity in children. Twenty-four such cases were treated in our clinic during the last 4 years (1981-1984). The treatment was surgical in six cases and related to the elbow injuries, while the forearm underwent closed treatment. The results of these cases, regardless of treatment and severity of the injury, did not differ appreciably from the results of common forearm or elbow injuries.

The pathoanatomy of lateral ligamentous disruption in complex elbow instability.

We sought to determine the lateral soft-tissue injury pattern in a consecutive series of patients with elbow dislocation (10 cases) or fracture-dislocation (52 cases) that required open operative repair. Patients who were seen more than 3 months after injury or those in whom previous operative intervention had obscured the anatomy were excluded. There were 42 men and 19 women (mean age, 43 years; range, 13-82 years). One patient had bilateral injuries. The mean time to surgery was 15 days after injury, with a range from 1 to 76 days. There were associated fractures in 52 elbows: coronoid (39),

Modified anterolateral approach for internal fixation of Holstein-Lewis humeral shaft fractures.

BACKGROUND: The authors modified the anterolateral approach for Holstein-Lewis humeral shaft fractures using a plating technique to achieve sufficient distal fixation by minimal splitting of the brachioradialis muscle to fix the most distal screws. The purpose of this retrospective study was to evaluate the efficacy of our modified anterolateral approach for Holstein-Lewis humeral fractures and document clinical and functional results. MATERIALS AND METHODS: Between 2008 and 2014, 18 patients (mean age 35.4 years) with a Holstein-Lewis humeral shaft fracture who underwent open reduction and i

Nonunion of distal humeral fractures: Current concepts.

The incidence of complex articular fractures of the distal humeral in adults has increased and will be growing in the future due to the greater incidence of high energy trauma and to the higher percentage of elderly population. Successful treatment is challenging for the needed balance between a stability of often comminuted fracture and early motion. Nonunion is a common complication after distal humerus fractures that is influenced by a variety of factors such as biology, in particular the blood supply of the metaphysis, the non-anatomical reduction of the fracture, the methods of fixation,

Management of lateral humeral condylar fracture in children.

Lateral condylar fractures constitute 12% to 20% of all pediatric distal humerus fractures. These fractures are easily missed and when not managed appropriately can displace. Missed fracture is a common cause of nonunion and deformity; thus, a high index of suspicion and adequate clinical and radiographic evaluation are required. Displaced fractures are associated with a high rate of nonunion. Nondisplaced fractures or those displaced ≤2 mm are managed with cast immobilization and frequent radiographic follow-up. Fractures displaced >2 mm are managed with surgical fixation. Successful outcomes

Compound elbow dislocation and brachial artery rupture.

A case of brachial artery rupture following compound dislocation of the elbow is reported. Arterial injury is a well-known complication of a supracondylar fracture of the humerus, but is very rare in association with elbow dislocation. A reversed saphenous vein graft was used for treatment of the arterial injury and is the recommended method of management.

External fixation about the elbow: Indications and long-term outcomes.

BACKGROUND: Operative management is often required for fractures of the elbow, with treatment goals aiming to restore stability, reduction, and early range of motion. The purpose of this study was to determine risk factors for necessitating the application of an external fixator, and to compare range of motion and functional outcomes between patients who required an elbow external fixator to those who did not. HYPOTHESIS: We hypothesize that patients who require an external fixator will have worse elbow range of motion and functional outcomes when compared to those who did not. PATIENTS AND

Radial head arthroplasty: a historical perspective.

There has been lively debate regarding the rationale behind the use of radial head arthroplasty (RHA) for more than 80 years. Currently, its primary indication is for treatment of non-reconstructible RH fractures. The first RH implant, released in 1941, was a ferrul cap used to prevent heterotopic ossification. Biomechanical studies in the 1980s stimulated a revolution in RHA design by promoting modular implants that replicated the native bony anatomy of the elbow. Subsequent data-driven evolution in design led to the creation of a variety of devices that also accommodated for common ligamento

Long-term outcomes of a cemented bipolar radial head prosthesis: a large retrospective study.

BACKGROUND: In acute radial head fractures (RHFs), a radial head prosthesis can be considered if open reduction and internal fixation are not technically feasible. This study provides the largest long-term clinical and radiologic follow-up study of cemented bipolar Judet prostheses implanted to manage acute RHFs, either isolated and associated with other elbow fractures. We compare their functional and radiographic outcomes to test the hypothesis that the bipolar design can achieve similar results in both groups. METHODS: We reviewed data of 63 consecutive patients implanted with a bipolar pr…

Stress fracture of the olecranon in an adult baseball player.

Stress fractures of the olecranon caused by repetitive stress force have infrequently been reported as a cause of elbow pain in adult athletes, engaged in throwing and pitching sports. We diagnosed as a stress fracture of the olecranon by clinical and radiographic findings and treated surgically. The patient returned to playing baseball at a competitive level and was asymptomatic 4 months after the first operation. However, the patient re-injured the olecranon and a second surgical treatment was performed almost 1 year after the first operation. After the second surgery, the patient returned t

Isolated and combined fractures of the diaphysis of the radius and ulna.

In summary, treatment of diaphyseal fractures requires accurate assessment of the injury to rule out concomitant ligamentous injury at the wrist or the elbow. Minimally displaced ulnar fractures can be managed with bracing. Displaced diaphyseal fractures of the radius and ulna should be plated, usually with 3.5-mm compression plates using AO technique. Complications may still occur but can be minimized if strict attention to technique is followed.

Intercondylar fractures of the distal humerus treated with the triceps-reflecting anconeus pedicle approach.

INTRODUCTION: Treatment choice for displaced, intercondylar fractures of the distal humerus is open reduction and internal fixation (ORIF) through a posterior approach. The triceps-reflecting anconeus pedicle (TRAP) approach, combination of modified Kocher and Bryan-Morrey has been described as a conservative surgical exposure for fixation of the complex intercondylar fractures. MATERIALS AND METHODS: Eleven patients with intercondylar fractures of the humerus operated with this approach were reviewed. The mean follow-up was 26 (14-40) months. The aetiology of injuries was mostly fall on the

Outcomes of suture tension band technique for fixation of olecranon fractures: a retrospective case series.

BACKGROUND: Tension band wiring is one of the earliest and most common techniques for the fixation of olecranon fractures with good functional outcomes. However, it is fraught with hardware-related complications, with reported reoperation rates of 33.3%-79.2%. Despite the aim of reducing hardware-related complications, low-profile precontoured plates continue to have hardware removal rates of 15%-56%. Suture tension band fixation is a novel technique for the management of select olecranon fractures with equivalent or superior biomechanical performance to tension band wiring and avoids hardware
