Long-term hip function in slipped capital femoral epiphysis treated with in situ pinning.

BACKGROUND: In recent years, there has been a trend toward more aggressive management of slipped capital femoral epiphysis (SCFE) with acute anatomical realignment; however, the literature is unclear with regard to the indications for this. QUESTIONS/PURPOSES: To collect long-term patient-reported outcome scores on a group of SCFE patients using modern hip scores. The second aim was to determine whether there is a threshold level of deformity beyond which patients have predictably poor outcomes following in situ pinning. PATIENTS AND METHODS: Patients presenting with SCFE between 2000 and 20

Slipped capital femoral epiphysis: what's new?

Slipped capital femoral epiphysis (SCFE) is a common hip disorder among adolescents, whereby the epiphysis is displaced posteriorly and inferiorly to the metaphysis. Treatment modalities aim to stabilize the epiphysis, prevent further slippage, and avoid complications associated with long-term morbidity, such as osteonecrosis and chondrolysis. Controversy exists with SCFE regarding prophylactic fixation of the contralateral, painless, normal hip, the role of femoroacetabular impingement with SCFE, and whether in situ fixation is the best treatment method for SCFE. This article presents and dis

Range of motion after computed tomography-based simulation of intertrochanteric corrective osteotomy in cases of slipped capital femoral epiphysis: comparison of uniplanar flexion osteotomy and multiplanar flexion, valgisation, and rotational osteotomies.

BACKGROUND: Various osteotomy techniques have been developed to correct the deformity caused by slipped capital femoral epiphysis (SCFE) and compared by their clinical outcomes. The aim of the presented study was to compare an intertrochanteric uniplanar flexion osteotomy with a multiplanar osteotomy by their ability to improve postoperative range of motion as measured by simulation of computed tomographic data in patients with SCFE. METHODS: We examined 19 patients with moderate or severe SCFE as classified based on slippage angle. A computer program for the simulation of movement and osteot

[Significance and results of subcapital osteotomy in severe slipped capital femoral epiphysis].

Today treatment with in situ fixation for mild slips in chronic SCFE is undisputed. However, it is far more difficult to give a definite recommendation in cases where a severe dorsal slip of >70 degrees has already occurred. With this position of the femoral head, a severe functional deficit combined with an early onset of osteoarthritis has to be expected. With these severe slips we prefer the subcapital osteotomy, a technique which results in an almost normal anatomical alignment of femoral head and acetabulum. In the literature, this procedure is subject to controversy because of the relati

[Septic coxitis in childhood. Differential ultrasound diagnoses].

The efficiency of ultrasound was tested in septic arthritis. A total of 259 children with hip pain, septic arthritis (n = 14), transient synovitis (n = 120), juvenile rheumatoid arthritis (n = 12), Legg-Calvé-Perthes disease (n = 92) and slipped capital femoral epiphysis (n = 21) were examined by ultrasound. By using the standard planes described by the DEGUM, it is possible to analyze the joint capsule, the surface of the femoral head and the periarticular structures. In cases with synovitis or joint effusion, capsular distention can be diagnosed by ultrasound. This distention is typical in s

Osteotomies of the hip in the prevention and treatment of osteoarthritis.

Osteoarthritis of the hip is common in our society, even in the relatively young patient. Most of this osteoarthritis is mechanical in etiology and is secondary to residual deformity from developmental hip disease. This type of osteoarthritis can often be predicted and prevented if the causative excessive joint pressures are reduced in a timely fashion by corrective osteotomy. Realigning pelvic or intertrochanteric osteotomy of this preventive type is termed reconstructive; those osteotomies performed after osteoarthritis is established are termed salvage. Corrective osteotomy will be clinical

Hormone status in patients with slipped capital femoral epiphysis.

In 15 consecutive patients with slipped capital femoral epiphysis we recorded height, weight and skeletal maturity. Sexual maturity was assessed clinically and biochemically, and Harris's hypothesis that there is an increased ratio of serum growth hormone to oestrogen was tested in comparison with 15 age and sex matched controls. We found no difference in skeletal or sexual maturity between the groups, or any overt endocrine abnormality in the patients. However almost half the patients with slipped epiphysis were over the 90th weight percentile, suggesting that mechanical factors such as obesi

[Labor trauma-induced epiphysiolysis of the proximal femur. 4 cases].

The authors report on 4 cases of obstetrical traumatic slipping of the upper femoral epiphysis. Firstly, they emphasise the difficulties of diagnosis because trauma is often missed. In two cases the delivery was a cesarean section (in the same clinic!). Wrong diagnoses are: --neonatal septic arthritis with swelling and inflammation of the hip, lateral deviation of the metaphysis on the x-ray. Nevertheless, there are no signs of an infectious disease and symptoms begin at birth. --congenital dislocation of the hip but clinical testing of stability is painful and doubtful. Secondly, when diagnos

[Late results of traumatic separation of the upper femoral epiphysis as an obstetrical lesion (author's transl)].

Publications on birth-traumatic epiphyseolysis of the head of the femur are rare. No long-term reports (the course of the disease extending over 10 years and more) have been published. We must mention the detailed paper published by Mau in 1975 on epiphyseolysis of the head of the femur in children, among which a lesion of the epiphysis in an otherwise complicationfree birth is described. In most of the cases, however, epiphyseolysis occurs with breech presentation (Michail 1958, Mortens 1964), less frequently with Caesarean sections (Baumgartner 1961) and internal versions (Lindseth 1971). Does spinopelvic alignment affect femoral head cartilage and the proximal femoral physis in slipped capital femoral epiphysis? A finite element analysis.

BACKGROUND: Slipped capital femoral epiphysis is a prevalent pediatric hip disorder. Recent studies suggest the spine's sagittal profile may influence the proximal femoral growth plate's slippage, an aspect not extensively explored. This study utilizes finite element analysis to investigate how various spinopelvic alignments affect shear stress and growth plate slip. METHODS: A finite element model was developed from CT scans of a healthy adult male lumbar spine, pelvis, and femurs. The model was subjected to various sagittal alignments through reorientation. Simulations of two-leg stance, on

Early MRI Detection and Closed Bone Graft Epiphysiodesis May Alter the Course of Avascular Necrosis Following Unstable Slipped Capital Femoral Epiphysis.

BACKGROUND: Unstable slipped capital femoral epiphysis (SCFE) has an increased incidence of avascular necrosis (AVN). Early identification and surgical intervention for AVN may help preserve the femoral head. METHODS: We retrospectively reviewed 48 patients (50 hips) with unstable SCFE managed between 2000 and 2014. AVN was diagnosed based on 2 different postoperative protocols. Seventeen patients (17 hips) had a scheduled magnetic resonance imaging (MRI) between 1 and 6 months from initial surgery, and the remaining 31 patients (33 hips) were evaluated by plain radiographs alone. If AVN was

Slipped capital femoral epiphysis: long-term outcome and remodelling after in situ fixation.

PURPOSE: In primary treatment of slipped capital femoral epiphysis (SCFE) in situ fixation reliably prevents slip progression. The aim of this study was to examine the long-term outcome of SCFE-patients treated by transfixation with Kirschner wires with a special focus on the remodelling process of the hips under this fixation method. METHODS: 30 patients with stable SCFE (mean initial slip angle: 29°, mean follow-up: 10 years), who were treated with bilateral transfixation by Kirschner wires, were recruited for this study. At most recent follow-up patients were clinically and radiologically

Drehmann sign and femoro-acetabular impingement in SCFE.

BACKGROUND: Drehmann sign is a characteristic clinical feature in slipped capital femoral epiphysis (SCFE). The presence of SCFE indicates an anatomic change of the proximal femur, which induces obligatory hip external rotation with hip flexion. In contrast, a cam-type femoro-acetabular impingement (FAI) is well known as sequelae of SCFE. The purpose of this study was to clarify the relationship between Drehmann sign and radiologic FAI. METHODS: We studied 92 hips of 80 SCFE patients who had been treated with in situ fixation. The occurrence rate of Drehmann sign was analyzed according to the

Outcome of in situ pinning in patients with unstable slipped capital femoral epiphysis: assessment of risk factors associated with avascular necrosis.

INTRODUCTION: Avascular necrosis (AVN) is a devastating complication following treatment of unstable slipped capital femoral epiphysis (SCFE). The advent of newer methods such as open reduction and surgical dislocation of the hip has increased the debate on the optimal method of treatment. However, the risk or predisposing factors for AVN remain unclear. We aimed to assess the outcome of in situ fixation and the risk factors associated with AVN. METHODS: We retrospectively reviewed the records of 27 patients (27 hips) with unstable SCFE out of the 280 children managed for SCFE from 1995 to 20

Significance of clinical and radiographic findings in young adults after slipped capital femoral epiphysis.

The purpose of this study was to assess the clinical and radiographic presentation of young adults in the mid-term follow-up after pinning in situ for mild to moderate slipped capital femoral epiphysis (SCFE). We postulated that there was a correlation between the degree of head-neck-offset decrease and clinical and radiographic signs of hip joint degeneration. Thirty-eight young adults (average age 23.4 ± 3.6 years old) with various grades of femoral head-neck-offset pathologies were assessed clinically via Harris hip score (HHS), Tegner-Lysholm score (TLS) and Short Form 36 (SF-36), and radi

The unstable slipped capital femoral epiphysis: risk factors for osteonecrosis.

BACKGROUND: The incidence of avascular necrosis (AVN) after unstable slipped capital femoral epiphysis (SCFE) varies widely in the literature (10% to 60%), and few studies have examined why certain unstable slips develop osteonecrosis whereas others do not. Our purpose was to determine risk factors for developing AVN after unstable SCFE. METHODS: We reviewed all unstable SCFEs treated primarily at our center. Medical records were reviewed to determine weight-percentile, age, length of prodromal symptoms, and time to treatment. Operative notes were used to classify treatment as either: (group
