No effect of arm-crank exercise on diaphragmatic fatigue or ventilatory constraint in Paralympic athletes with cervical spinal cord injury.

Cervical spinal cord injury (CSCI) results in a decrease in the capacity of the lungs and chest wall for pressure, volume, and airflow generation. We asked whether such impairments might increase the potential for exercise-induced diaphragmatic fatigue and mechanical ventilatory constraint in this population. Seven Paralympic wheelchair rugby players (mean + or - SD peak oxygen uptake = 16.9 + or - 4.9 ml x kg(-1) x min(-1)) with traumatic CSCI (C(5)-C(7)) performed arm-crank exercise to the limit of tolerance at 90% of their predetermined peak work rate. Diaphragm function was assessed before

Anxiety and Fear of Exercise in Cardiopulmonary Rehabilitation: PATIENT AND PRACTITIONER PERSPECTIVES.

PURPOSE: One unexplored mechanism for poor outcomes in cardiovascular and pulmonary rehabilitation (CVPR) is fear about aerobic exercise. Pa … ty of anxiety treatment (r values = 0.30-0.40), and avoidance of exercise was correlated with lower exercise tolerance and poorer physical health quality of life (r values = -0.22 to -0.24). Practitioners estimated that an average of 47 ± 20% of patients experience fear associated with exercise and reported low feelings of preparedness to address patient anxiety and …

Pulmonary vasodilator therapy in tetralogy of Fallot with pulmonary atresia and major aortopulmonary collaterals: case series and review of literature.

We present the use of pulmonary vasodilators in three adult patients with unrepaired tetralogy of Fallot, pulmonary atresia, aortopulmonary collaterals, and segmental pulmonary arterial hypertension. Patients improved by 1-2 NYHA classes with modest exercise-tolerance increase, and remained stable without side effects during 2.5, 10, and 14 years. Literature review revealed five studies with pulmonary vasodilators in heterogeneous, mostly repaired patient populations.

Heparin-induced thrombocytopenia in a patient with lung embolism and left-sided pneumonia: a case report.

A case of a 73-year obese female with arterial hypertension, periodical metrorrhagia, polydipsia, and polyuria is presented. The patient was admitted to the hospital because of increasing dyspnea, cough, subfebrile body temperature, and decreasing exercise tolerance. Physical examination and laboratory tests were suggestive for massive lung embolism. Low molecular heparin was started and after 5 days' therapy, heparin-induced thrombocytopenia was diagnosed.

[Some specific indices for stress tests in patients with ischemic heart disease concurrent with chronic cholecystitis].

The myocardial coronary reserve and functional state were studied in patients with ischemic heart disease concurrent with chronic choletistitis against the background of a stress-test (veloergometry). Particular features are discussed of hemodynamic changes in conducting exercise tolerance tests in coronary patients presenting with diseases that lead to a reduction in the myocardial coronary reserve and decrement in performance capability of the heart.

The CREST syndrome--successful reduction of pulmonary hypertension by captopril.

A patient with severe pulmonary hypertension due to pulmonary arteriolar obliteration complicating a long standing CREST variant of systemic sclerosis showed improvement in cardiac output and exercise tolerance during treatment with captopril, which was discontinued because of persistent systemic hypotension. It is suggested that captopril would have a useful and possible prophylactic effect early in pulmonary hypertension in systemic sclerosis.

Agenesis of the lung.

Agenesis of the lung is rare. The cases of ten patients with this malformation are recorded and illustrated. Most had congenital malformation of other organs, especially the heart, as well. All had hypoplasia of the pulmonary artery or its branches. A pantaloon-like appearance of the trachea and the main bronchi on the bronchograms is described. Exercise tolerance was impaired in all patients.

The Relationship Between Physical Activity Level and Exercise Capacity and Respiratory Parameters in Individuals With Spinal Cord Injury.

Background: Spinal cord injury (SCI) detrimentally impacts individuals' exercise capacity and respiratory parameters depending on sensory, motor, and autonomic dysfunctions. Regular physical activity (PA) positively impacts cardiovascular health and pulmonary function in these individuals. This study determined the relationship between PA and exercise capacity and respiratory parameters in individuals with SCI. Methods: This cross-sectional study included 40 individuals with paraplegia. Assessments included the Physical Activity Scale for Individuals With Physical Disabilities (PASIPD), 6-min

Effect of Aerobic Exercise Training on Ventilatory Efficiency and Respiratory Drive in Obese Subjects.

BACKGROUND: Obese patients show a decline in exercise capacity and diverse degrees of dyspnea in association with mechanical abnormalities, increased ventilatory requirements secondary to the increased metabolic load, and a greater work of breathing. Consequently, obese patients may be particularly predisposed to the development of respiratory muscle fatigue during exercise. The aim of this study was to assess inspiratory muscle performance during incremental exercise in 19 obese male subjects (body mass index 41 ± 6 kg/m2) after aerobic exercise training using the noninvasive, inspiratory mus

Sildenafil and Exercise Capacity in the Elderly at Moderate Altitude.

OBJECTIVE: Hypobaric hypoxia decreases exercise capacity and causes hypoxic pulmonary vasoconstriction and pulmonary hypertension. The phosphodiesterase-5 inhibitor sildenafil is a pulmonary vasodilator that may improve exercise capacity at altitude. We aimed to determine whether sildenafil improves exercise capacity, measured as maximal oxygen consumption (peak V̇o2), at moderate altitude in adults 60 years or older. METHODS: The design was a randomized, double-blind, placebo-controlled, crossover study. After baseline cardiopulmonary exercise testing at 1400 m, 12 healthy participants (4 wo

Association of exercise capacity on treadmill with future cardiac events in patients referred for exercise testing.

BACKGROUND: Little is known about the association between exercise capacity and nonfatal cardiac events in patients referred for exercise treadmill testing (ETT). Our objective was to determine the prognostic importance of exercise capacity for nonfatal cardiac events in a clinical population. METHODS: A cohort study was performed of 9191 patients referred for ETT. Median follow-up was 2.7 years. Exercise capacity was quantified as the proportion of age- and sex-predicted metabolic equivalents achieved and categorized as less than 85%, 85% to 100%, and greater than 100%. Is Heat Intolerance State or Trait?

The existing literature suggests two standpoints in defining heat intolerance, which are heat tolerance as state or trait. The former bases … ciated with one's vulnerability to heat stress. Individuals who seem to exhibit lower than expected exercise tolerance in moderate heat and those who never regain heat tolerance post EHS fall into this category. However, there is a large area of uncertainty in this debate because a true prospective investigation of factors associated with heat intolerance is methodo …

Mild hyperbaric hyperoxia improves aerobic capacity and suppresses cardiopulmonary stress during the maximal cycle-ergometer test.

Aerobic exercise is more recommended than anaerobic exercise for individuals with cardiopulmonary dysfunction to avoid cardiopulmonary stress. However, their oxygen-carrying capacity is already reduced, making it difficult to exercise enough. Therefore, we aimed to investigate whether mild hyperbaric hyperoxia enhances aerobic capacity and decreases cardiopulmonary stress during exercise with a particular focus on the ventilatory threshold (VT). Nineteen healthy men (age 34.9 ± 10.8 years) performed ramp-loading tests on a cycle-ergometer under the three oxygen conditions: hypobaric hypoxia (H

Sex Differences and Correlates of the Utility of the Cardiopulmonary Exercise Test for Prescribing Exercise at Entry to Cardiac Rehabilitation.

BACKGROUND: Despite the importance of objective measures for prescribing aerobic exercise for mitigating cardiovascular risk in people with coronary artery disease (CAD), no study has examined sex differences in the utility of the cardiopulmonary exercise test (CPET) for developing the exercise prescription. METHODS: CPET results from 1352 females and 5875 males with CAD were analysed to determine if there was a sex difference in achieving maximal oxygen uptake (V˙O2max) or an identifiable first ventilatory threshold (VT1). Secondary outcomes were to determine correlates of not achieving V˙O2
