A case of relapsing polychondritis initially presenting with bronchial chondritis.

A 51-year-old woman presented with one-month history of fever, productive cough, dyspnea, hoarseness and polyarthritis. Computed tomography (CT) depicted diffuse bronchotracheal stenosis, which deteriorated in exhalation. Three-dimensional CT revealed airway stenosis from the trachea to the lobar bronchi. We made a diagnosis of relapsing polychondritis (RP). Administration of high-dose glucocorticoid and oral cyclophosphamide resolved the symptoms and the bronchial stenosis. It was noteworthy that all features presented were bronchial chondritis and polyarthritis. RP should be considered when …

Arterial cerebral air embolism at the site of a spontaneous pontine hemorrhage in a patient receiving erroneous continuous positive pressure ventilation.

We report the case of a 48-year-old male with iatrogenic arterial cerebral air embolism at the site of a spontaneous pontine hemorrhage. The patient inadvertently received continuous positive pressure ventilation without exhalation for a few minutes, resulting in pneumothorax, interstitial emphysema, pneumoperitoneum, and arterial cerebral air embolism at the site of the intracerebral hemorrhage. This is the first report of pneumocephalus without head trauma or previous surgery in which the air embolism occurs at the site of a spontaneous intracerebral hemorrhage. We hypothesize that air prefe…

Pulsed taVNS-Elicited Pupil Dilation: Effects of Intermixed Stimulation, Sham Location, and Respiratory Phase.

Transcutaneous auricular vagus nerve stimulation (taVNS) may offer a powerful, noninvasive way to stimulate activity of brainstem arousal sy … ha. Furthermore, taVNS-evoked pupil dilation was not enhanced when stimulation was delivered during exhalation, as would be expected if pupil effects were mediated by the vagus nerve-nucleus tractus solitarius-LC pathway. Together, these findings show that the effect of pulsed taVNS on pupil dilation can be preserved when taVNS and (earlobe) sham are deliver …

Computational modeling of airway instability and collapse in tracheomalacia.

BACKGROUND: Tracheomalacia (TM) is a condition of excessive tracheal collapse during exhalation. Both acquired and congenital forms of TM are believed to result from morphological changes in cartilaginous, fibrous and/or smooth muscle tissues reducing airway mechanical properties to a degree that precipitates collapse. However, neither the specific amount of mechanical property reduction nor the malacic segment lengths leading to life threatening airway collapse in TM are known. Furthermore, the specific mechanism of collapse is still debated. METHODS: Computational nonlinear finite element m…

An assessment of branching asymmetry of the tracheobronchial tree.

The tracheobronchial tree is commonly seen to have a systematic branching symmetry, despite being known to have an asymmetrical design. Branching asymmetry allows for uniform airflow and provides robustness against the morphogenesis-related size variability. Here, a constructal approach is used to tracheobronchial tree analysis, and a general model based on entropy generation during breathing process is provided, which holds with asymmetric characteristics of the tree, and the change for inhaling and exhaling air. In contrast to traditional models available in the literature, the entropy gener

Measurement and estimation of human forced expiratory noise parameters using a microphone with a stethoscope head and a lapel microphone.

OBJECTIVE: The time signature of tracheal noise in the 200-2000 Hz frequency band recorded during a forced expiratory manoeuvre (FETa) is a promising tool for diagnosis of bronchial obstructions. FETa is typically registered using a microphone with a stethoscope head placed on the neck over the trachea (stethoscope sensor). We studied basic time and spectral parameters of forced expiratory tracheal noises with a microphone placed near the mouth (lapel microphone, outside the flow of exhaled air) and compared these with measurements via stethoscope sensor, with the aim of providing patient home

Before we get started: what is a cough?

Cough is an airway defensive reflex consisting of an inspiratory phase followed by a forced expiratory effort initially against a closed glottis, followed by active glottal opening and rapid expiratory flow. The expiration reflex (ER) differentiates from cough for the lack of a preparatory inspiration. The reflexes subserve different functions: cough will clear the lower airways from debris and mucus, while the expiration reflex will prevent aspiration. Clinically, a cough epoch is a sequence of motor acts resulting from a combination of true coughs and ERs that need to be accurately identifie

Noninvasive SARS-CoV-2 detection using a low-cost electronic nose.

The COVID-19 pandemic highlighted the urgent need for rapid and accurate SARS-CoV-2 detection. Current diagnostic methods often suffer from discomfort, slow results, and limited accuracy in early infection stages. This study proposes a solution: a noninvasive, rapid, and accurate detection approach for point-of-care settings using a metal-oxide-sensor-based electronic nose. This innovative electronic nose uses an array of off-the-shelf gas sensors. These sensors detect and analyze the volatile organic compounds present in saliva and exhaled breath, which change based on the presence of the SAR

How does the pelvic floor respond to modulations in trunk pressure induced by a variety of voicing tasks? A cross-sectional, observational study.

The pelvic floor responds to changes in trunk pressure, elevating during low-pressure exhale and descending during high-pressure exhale. Voicing occurs during exhalation, spanning low-to-high trunk-pressure, yet it is unknown how voicing affects the pelvic floor. The aim of this study was to quantify pelvic floor response to voicing and identify if there are differences for women with stress urinary incontinence. We hypothesized that shouting would cause pelvic floor descent, with greater magnitude for incontinent women. Sixty women (38 incontinent, 22 continent) performed four voicing tasks (…

Role of breath phase and breath boundaries for the classification between asthmatic and healthy subjects.

Asthma is an inflammatory disease of the airways which causes cough, chest tightness, wheezing and other distinct sounds during breathing. Spirometry is a golden standard lung function test, is used to monitor and diagnose asthma. Spirometry is very time-consuming and requires a lot of patient's efforts. Therefore, an alternate method which can overcome spirometry limitations is required. Sound based method can be one such alternative as it is less tedious, less time consuming and suitable for patients of all ages. It has been shown in the past that breath, among other vocal sounds, performs t

Non-Invasive Assessment of Metabolic Adaptation in Paediatric Patients Suffering from Type 1 Diabetes Mellitus.

An analysis of exhaled volatile organic compounds (VOC) may deliver systemic information quicker than available invasive techniques. Metabolic aberrations in pediatric type 1 diabetes (T1DM) are of high clinical importance and could be addressed via breathomics. Real-time breath analysis was combined with continuous glucose monitoring (CGM) and blood tests in children suffering from T1DM and age-matched healthy controls in a highly standardized setting. CGM and breath-resolved VOC analysis were performed every 5 minutes for 9 hours and blood was sampled at pre-defined time points. Per particip

Novel Self-Assembly Route Assisted Ultra-Fast Trace Volatile Organic Compounds Gas Sensing Based on Three-Dimensional Opal Microspheres Composites for Diabetes Diagnosis.

The development of ultra-fast response semiconductor gas sensors for high-accuracy detection of trace volatile organic compounds in human exhaled breath still remains a challenge. Herein, we propose a novel self-assembly synthesis concept for preparing intricate three-dimensional (3D) opal porous (OP) SnO2-ZnO hollow microspheres (HM), by employing sulfonated polystyrene (S-PS) spheres template-assisted ultrasonic spray pyrolysis. The high gas accessibility of the unique opal hollow structures resulted in the existence of 3D interconnection and bimodal (mesoscale and macroscale) pores, and the

Effect of fluticasone on markers of inflammation and quality of life in steroid-naive patients with mild asthma.

BACKGROUND AND AIMS: Patients with mild asthma may adapt to symptoms that may be neglected at a medical consultation. Despite active airway inflammation, indicating need for treatment symptoms may be poorly perceived and influence on quality of life. The aim was to find out if markers of asthma activity and quality of life are influenced by inhaled steroids in patients who regard themselves as free of symptoms. METHODS: Seventy steroid-free patients with mild asthma were treated with inhaled fluticasone (250 microg twice daily) or placebo for 3 months in a randomised, double-blind, study. Spi

Measurement of IL-13-induced iNOS-derived gas phase nitric oxide in human bronchial epithelial cells.

Exhaled nitric oxide (NO) is altered in numerous diseases including asthma, and is thought broadly to be a noninvasive marker of inflammation. However, the precise source of exhaled NO has yet to be identified, and the interpretation is further hampered by significant inter-subject variation. Using fully differentiated normal human bronchial epithelial (NHBE) cells, we sought to determine (1) the rate of NO release (flux, pl.s(-1.)cm(-2)) into the gas; (2) the effect of IL-13, a prominent mediator of allergic inflammation, on NO release; and (3) inter-subject/donor variability in NO release.
