Distal iliopsoas muscle pedicle bone graft was investigated in dogs to determine whether a collateral blood supply to the femoral head can develop after displaced femoral neck fractures and dislocation of the hip joint. Microangiography performed in cadavers showed good vascularity of the muscle pedicle graft and similar studies in dogs showed that when transferred to the anterior femoral neck, the iliopsoas muscle pedicle graft healed with an ingrowth of vessels across the graft site. Microangiographic and histologic studies in dogs showed that the procedure could maintain the vascularity and

Although considerable remodeling can occur in slipped capital femoral epiphysis, it is generally conceded that the severely-slipped epiphysis requires corrective osteotomy to restore adequate range of motion, and to prevent early degenerative changes. A study of artifically-created severe slips in cadaver specimens demonstrated that removal of the cervical prominence, as well as valgus-derotational osteotomy is necessary to restore good range of motion and provide a satisfactory weight-bearing surface. Clinical trial in 6 patients also demonstrated that the cervical prominence is a barrier to

This is a report on a University of Illinois Hospital series of cases of extracapsular base of the neck osteotomy for slipped capital femoral epiphyses. The osteotomy was performed on 20 hips in 16 patients, with a 71/2 year average follow-up. The results of this procedure are encouraging and illustrate the advantages of technical simplicity and minimal risk of avascular necrosis. The method does not create any additional deformity. The only disadvantage is that the correction could be limited. The maximum correction of posterior tilt was 55 degrees and the maximum correction of varus deformit

Multiple myeloma is the most common primary malignancy of the bone marrow and may present as bone pain and/or pathologic fracture(s) in affected patients. Treatment of bone lesions typically consists of chemotherapy and radiation and may include prophylactic fixation in patients meeting specific criteria. This report reviews a case of a 74-year-old female with a history of multiple myeloma and breast cancer, previously treated with chemotherapy and radiation, who sustained a pathologic femoral neck fracture with associated ipsilateral lesions of the femoral shaft and peritrochanteric region. Proximal femoral focal deficiency (PFFD) is a developmental defect of the proximal femur and acetabulum. PFFD includes a spectrum of findings ranging from mild femoral shortening with varus deformity to complete absence of both the acetabulum and the proximal femur. Most attempts to classify the severity of the abnormality are based on the radiographic findings. The most commonly used system is that of Aitken. This classification scheme is based upon the presence and location of the femoral head and neck on conventional radiography and provides an assessment of future limb function and plan tr

In the relatively recent past, the treatment of congenital dislocation of the hip was carried out according to Lorenz or to some other similar method. Many poor late results and too many deformities of the proximal part of the femur were observed, following a modification of the orientation of the femoral neck or avascular necrosis of the head and its growth plate. The full abduction position was responsible for these complications. Since 1960, a method has been developed based on the observation of the normal physiologic fetal position of the thighs close to the trunk, and on the fact that in

BACKGROUND: An association between arterial stiffness and osteoporosis has previously been reported. Therefore, we investigated the relation … ships between pulse wave velocity and bone mineral density of the lumbar spine (r=-0.335, P<0.001), femur neck (r=-0.335, P<0.001), and total femur (r=-0.181, P=0.04). Pulse wave velocity showed the strongest association with age (r=0.586, P<0.001). Multiple regression analysis identified an independent relationship between pulse wave velocity and lumbar spi …

AIM: To evaluate the accuracy of distal radial pQCT in discriminating between osteoporotic fracture and non fracture cases and its efficiency to predict fractures.
METHODS: Densitometric data determined with an XCT 900 Stratec pQCT device at the distal radius of women who had suffered distal radius (57), spinal (87) or femoral neck fractures (21) before or after measurement were compared with densitometric data of age-matched healthy women (265) and were analyzed in regression tests. Women whose fractures occurred before measuring were selectively analyzed.
RESULTS: More significant differen

PURPOSE: Optimal cephalomedullary nail (CMN) length for unstable pertrochanteric femur fractures is controversial. Long CMNs (L-CMNs) are currently recommended; however, intermediate-length CMNs (I-CMNs) may provide stable fixation without the additional surgical steps required by L-CMNs. We analyzed outcomes after unstable pertrochanteric femur fractures treated with L-CMNs or I-CMNs to determine whether functional outcomes, perioperative measures, complications, and mortality and reoperation rates differ by CMN length.
METHODS: We retrospectively reviewed medical records at our institution

INTRODUCTION: Femur head necrosis (FHN) is a challenging clinical disease with unclear underlying mechanism, which pathologically is associated with disordered metabolism. However, the disordered metabolism in cancellous bone of FHN was never analyzed by gas chromatography-mass spectrometry (GC-MS).
OBJECTIVES: To elucidate altered metabolism pathways in FHN and identify putative biomarkers for the detection of FHN.
METHODS: We recruited 26 patients with femur head necrosis and 22 patients with femur neck fracture in this study. Cancellous bone tissues from the femoral heads were collected a…

Predicting pathologic fractures in femora with metastatic lesions remains a clinical challenge. Currently used guidelines are inaccurate, especially to predict non-impeding fractures. This study evaluated the ability of a nonlinear quantitative computed tomography (QCT)-based homogenized voxel finite element (hvFE) model to predict patient-specific pathologic fractures. The hvFE model was generated highly automated from QCT images of human femora. The femora were previously loaded in a one-legged stance setup in order to assess stiffness, failure load, and fracture location. One femur of each

PURPOSE: Fibrous dysplasia (FD) of the proximal femur can result in severe deformity and disability. The results of surgical management in such situations have been reported to be poor. We present a novel, five-step surgical treatment to correct proximal femoral deformity in FD.
MATERIAL AND METHODS: This study is a review of prospectively collected data of cases of proximal femur polyostotic FD managed at our institute from 2012 onward. We managed three patients with FD involving four femora (one patient had bilateral disease). Mean age at presentation was 16 years with an average follow-up

BACKGROUND: Cementless hip prostheses with different offsets are frequently used to restore the rotation center of the hip. However, a rising offset is theoretically associated with a potential risk for increased interface stresses and early loosening.
METHODS: To assess this potential risk for cementless stems, the primary stability of the CLS Spotorno stem was examined with respect to three different femoral neck versions (125°, 135° and 145°) measuring 3-dimensional micromotions. For this purpose 18 stems were implanted in composite femurs and tested dynamically using physiological loading

OBJECTIVE: To determine the prevalence and correlates of low bone mineral density (BMD) in ambulatory outpatients with end-stage heart failure who were awaiting cardiac transplantation.
METHODS: Fifty-five cardiac transplant candidates with end-stage heart failure were enrolled in this study. Bone mineral density at the lumbar spine and proximal femur was determined by dual-energy x-ray absorptiometry. Laboratory studies included serum alkaline phosphatase, calcium, intact parathyroid hormone, and 25-hydroxyvitamin D.
RESULTS: The mean proximal femur and lumbar spine Z scores were 0.3 +/- 1.

This study was conducted to determine if bone and muscle loss is reversible in patients who had undergone total hip arthroplasty (THA) owing to necrosis of the femoral head after osteosynthesis for a displaced femoral neck fracture. The cortical bone mineral density (BMD), bone volume, bone mass, and muscle volume of the thigh and the BMD of the distal femur and proximal tibia were measured by quantitative computed tomography in 16 patients. Both extremities were measured just before reoperation. The measurements were repeated 3 and 6 months later. At reoperation, there was a mean 12% relative

OBJECTIVE: To evaluate whether femoral head diameter and head-to-neck ratio (HNR) influence prosthetic impingement-free motion (IFM) in canine total hip replacement (THR) using a press-fit cementless hip system in silico.
METHODS: This was a descriptive study conducted from January through June 2025. Twenty-eight head-neck combinations were assembled using 3-D stereolithographic files of cementless THR implants (Universal Hip System; BioMedtrix). For each implant combination, IFM was evaluated using a custom-built computer simulation software program around 3 orthogonal axes: abduction-adduct

Femoral neck fractures (FNFs) affect the young adult population and are intimately related to high-energy trauma. Despite innovations in osteosynthesis materials, the rate of complications remains at 10%-59% in Pauwels type III (PIII) fractures. The authors thus propose a fixation model with a novel self-compression screw, comparing it to a sliding hip screw plate associated with a derotation screw in the fixation of a PIII fracture with posterior inferior comminution. The finite element method (FEM) was used in this comparison along with a virtual femur model with structural characteristics s

Subchondral insufficiency fractures of the femoral head are generally considered to be osteoporosis-related fragility fractures. There have been reports of microfractures being found in subchondral bone on pathological examination. However, the mechanism of these microfractures is not known. In this report, we describe a patient with osteogenesis imperfecta who developed a subchondral insufficiency fracture of the femoral head after a fall that had resulted in a subcapital femoral neck fracture. Bipolar hemiarthroplasty was performed, and bone at the femoral head and neck was sampled for patho
