This case report identifies a temporal relationship between the diagnosis of localized granuloma annulare and the subsequent development of primary hypothyroidism in a previously healthy 10-year-old girl. We suspect these disorders are associated, but any association between them requires further study.

Actinomycotic mycetoma is frequent in Senegal and the treatment has changed greatly over the last decades. We observed 27 cases due to Actinomadura pelletieri (n = 21), Actinomadura madurae (n = 5) and Streptomyces somaliensis (n = 1). The diagnosis was based on clinical and histological arguments. All the patients were treated with cotrimoxazole for several months. The mycetoma was situated on the foot in 15/27 patients. We conducted a study to determine the prevalence and clinical patterns of footwear dermatitis, patch testing 50 patients with suspected footwear dermatitis and 30 controls with 22 allergens of a shoe series (Chemotechnique Diagnostics AB, Malmö, Sweden). The overall prevalence among contact dermatitis cases was 11.7%. The dorsa of the feet and toes were the commonest sites involved. 70% of patients showed sensitivity to footwear allergen(s), as opposed to 6.67% of controls. An extensive and severe actinomycetoma by Nocardia asteroides, a rare etiologic agent of this infection in Brazil, observed during a 9 year follow-up is reported. Unsuitable social and financial conditions led to amputation as the only possible solution for this case, no signs of infection relapse having been observed in three years after his surgery.

The case report of a 83-year-old woman with lichen ruber ulcerosus is reviewed. Clinically the patient showed extensive ulcerations of the sole of one foot, with synechiae of the toes. In addition, there was subtotal atrophic alopecia of the scalp, vulvar atrophy, and circumscribed cutaneous lesions of lichen planus. The disease started with chronic recurrent paronychiae. The ulcerative defect was successfully treated with mesh grafting, which is the therapy of choice.

Mycetoma is a common disease in the Republic of Niger. In two hospitals 133 cases were observed. The major site of lesions was the foot. Actinomycetomata were seen more often than eumycetomata. Streptomyces somaliensis is prevalent in the north desert zone while Actinomadura pelletieri is common in the southern part of the country. Madurella mycetomatis, the usual etiologic agent of eumycetoma, is seen in both regions. The species incidence and distribution in Niger differs from those of the west and east African endemic areas.

A 10% random sample of all bathers at a public swimming bath were examined for tinea pedis and verruca.The overall incidence of tinea pedis was 8.5% and of verruca 4.8%. The incidence of tinea pedis in 205 male adults was 21.5%, in 288 boys 6.3%, in 60 adult females 3.3%, and in 220 girls 0.9%. The incidence of verruca in juveniles ranged from 4.2% in boys to 10.5% in girls.It was clear that both infections spread within the baths, and since a relatively small proportion of users admitted to taking precautions to avoid contracting or developing infections it seems advisable that more publicity
