Impact of Preoperative Heel Ulceration on Surgical Outcomes Following Open Reduction and Internal Fixation of Rotational Ankle Fractures.

BACKGROUND: As surgical treatment with open reduction and internal fixation (ORIF) becomes more common, attention to preoperative risk stratification has grown. Although multiple modifiable risk factors and comorbidities have been associated with adverse outcomes, the impact of preexisting decubitus heel ulcers in this context remains yet to be defined. METHODS: Using a nationally representative multi-institutional database, a retrospective analysis was conducted on adults undergoing ORIF for rotational ankle fractures between 2010 and 2022. Patients with a documented decubitus heel ulcer wit

Outcomes of Metacarpal Shaft Fractures Treated by Open Reduction Internal Fixation With Plates and Screws Versus Intramedullary Screws.

BACKGROUND: Metacarpal fractures are among the most common orthopedic injuries. When metacarpal shaft fractures require surgical fixation, common options include closed reduction percutaneous pinning, open reduction internal fixation (ORIF) using plates and screws (PS), and intramedullary screw fixation (IMS). Certain metacarpal shaft fractures are amenable to ORIF with either PS or IMS. The purpose of this study is to compare the functional outcomes and complication rates between these techniques. METHODS: A retrospective review was performed of all patients that underwent ORIF with PS or IM

Comparative efficacy of tunnel minimally invasive technique versus traditional open reduction and internal fixation for rib fractures.

BACKGROUND: Rib fractures are often treated conservatively, but for cases with significant displacement or instability, surgical intervention may be necessary. Traditional open reduction and internal fixation (ORIF) has been the standard surgical approach; however, the tunnel minimally invasive technique is emerging as a less invasive alternative. This study compares the efficacy of these two surgical methods in terms of incision length, postoperative pain, surgical time, and hospital stay. METHODS: A retrospective evaluation was conducted from June 2022 to December 2024 at our hospital, incl

Exploring patient perspectives of younger adults with complications after internal fixation of femoral neck fractures: A qualitative study.

INTRODUCTION: Internal fixation is the recommended treatment for femoral neck fractures in younger adults, as it preserves the native hip joint. However, high failure rates and suboptimal outcomes following internal fixation persist. While there is substantial research on the experiences of older adults with femoral neck fractures, the perspectives of younger adults with complications following internal fixation remain largely underexplored. This study aims to examine the lived experiences of younger adults who underwent secondary surgery after internal fixation of a femoral neck fracture, wit

Definitive management of complex tibial plateau fractures: Combining limited internal fixation with bridge spiral Ilizarov frame.

BACKGROUND: High-energy tibial plateau fractures are commonly observed in concomitant with other injuries, soft tissue lacerations, and ischemic complications. The widely accepted management approach involves the application of a bridging external fixator to allow for soft tissue recovery, followed by delayed ORIF. Nonetheless, delayed internal fixation remains associated with notable risks, including deep infection, non-union, and implant failure. As an alternative, limited internal fixation in conjunction with tensioned-wire external fixators has been introduced. However, the use of metaphys

Internal fixation of comminuted patellar fractures with only non-absorbable suture via nice knot: a retrospective study.

BACKGROUND: Due to the patella's thin overlying skin, metal implants frequently cause local discomfort that necessitates removal. This study aimed to evaluate the intraoperative and early postoperative outcomes of treating comminuted patellar fractures using non-absorbable sutures as the sole internal fixation method via the Nice knot technique. METHODS: This retrospective study reviewed 25 patients with unilateral closed comminuted patellar fractures who underwent open reduction and internal fixation using either non-absorbable sutures tied with a Nice knot (NK group, n = 12) or the traditio

Stem revision vs. internal fixation in Vancouver B2/B3 periprosthetic hip fractures: systematic review and metanalysis.

INTRODUCTION: Vancouver B2 and B3 periprosthetic fractures (PFF) are associated with stem instability and often require a demanding stem implant revision (SR) or internal fixation (ORIF). This latter surgery is increasingly performed in the last few years instead of SR, but it is unclear which is the best treatment to manage PFF patients. The aim of this study is the compare the outcomes of B2/B3 PFF managed by either ORIF or SR, by performing a systematic review and meta-analysis of current literature. MATERIALS AND METHODS: Cochrane Database, PubMed, Google Scholar and MEDLINE were examined

Does local fat thickness correlate with post-operative infection in open reduction and internal fixation of acetabulum fractures?

PURPOSE: Obesity is an epidemic which increases risk of many surgical procedures. Previous studies in spine and hip arthroplasty have shown that fat thickness measured on preoperative imaging may be as or more reliable in assessment of risk of post-operative infection and/or wound complications than body mass index (BMI). We hypothesized that, similarly, increased local fat thickness at the surgical site is a predictor of wound complication in acetabulum fracture surgery. METHODS: Patients who underwent open reduction and internal fixation (ORIF) of an acetabulum fracture through a Kocher-Lan

Evaluating three internal fixation techniques for Pauwels III femoral neck fractures via finite element analysis.

The selection of implants for fixing unstable femoral neck fractures (FNF) remains contentious. This study employs finite element analysis to examine the biomechanics of treating Pauwels type III femoral neck fractures using cannulated compression screws (3CS), biplane double-supported screw fixation (BDSF), and the femoral neck system (FNS). A three-dimensional model of the proximal femur was developed using computed tomography scans. Fracture models of the femoral neck were created with 3CS, BDSF, and FNS fixations. Von Mises stress on the proximal femur, fracture ends, internal fixators, an

Functional Outcomes and Complications After Open Reduction and Internal Fixation of Mid-shaft Clavicle Fractures: A Retrospective Study.

INTRODUCTION: Clavicle fractures are routinely encountered in orthopedic clinical practice and have often been the subject of debate when it comes to optimal treatment. Clavicle fracture surgery has come a long way with excellent pre-contoured superior locking plates available for fixation. This study aimed to evaluate a cohort of patients operated for displaced mid-shaft clavicle fractures by open reduction and internal fixation using superior clavicle locking plates. MATERIALS AND METHODS: This is a retrospective cohort study of mid-shaft clavicle fracture patients who were operated on and

Trimalleolar Fractures Treated by Open Reduction Internal Fixation Compared With Arthroscopically Assisted Reduction and Minimally Invasive Surgery.

BACKGROUND: Currently, the standard of treatment for trimalleolar (TM) fracture is osteosynthesis through open reduction and internal fixation (ORIF). This study assessed whether arthroscopically assisted reduction and minimally invasive surgery (AARMIS) can be an alternative surgical method for TM fractures. METHODS: This retrospective cohort study included 49 patients with TM that were surgically treated. 27 patients received ORIF and 22 patients underwent AARMIS . At baseline, we recorded the patient's demographic features, fracture pattern (AO-OTA and Haraguchi classification), and surgic

Preoperative Anemia Is Associated With Worse Postoperative Outcomes After Open Reduction Internal Fixation of Distal Radius Fractures.

BACKGROUND: Preoperative anemia has been shown to be associated with complications after numerous orthopedic procedures. No studies to our knowledge have examined its impact on outcomes after open reduction internal fixation (ORIF) of distal radius fracture (DRF). We hypothesized that patients with preoperative anemia would have increased likelihood of adverse outcomes, and likelihood would increase with severity of anemia. METHODS: A total of 14 136 patients underwent ORIF for DRF over 2012-2021, identified in the National Surgical Quality Improvement Program database. Patients were classifi
