Giant frontal sinus osteoma and its potential consequences: illustrative case.

BACKGROUND: Osteomas of the paranasal sinuses are benign, slow-growing bone tumors that can cause a variety of clinical features depending on their size and location. Most osteomas are asymptomatic and located in the frontal sinus. In rare cases, they may grow to extend into the cranial or orbital cavities, resulting in atypical presentations. The authors presented an aggressive case of a frontoethmoidal sinus osteoma with intracranial extension of an inflammatory sinonasal polyp. OBSERVATIONS: A 30-year-old man with a history of chronic sinusitis presented to the hospital after three episode

Delayed development of mucocele in the fractured frontal sinus.

We present a case of delayed mucocele in the frontal sinus which occurred in 8 years after the initial trauma.A 56-year-old man visited the hospital with a complaint of his left eye protruding for years. Eight years prior, he fell down stairs and sustained an injury on the head and was diagnosed to have a fracture of the anterior wall of frontal sinus. Frontal sinus mucosa was extirpated, and the space was obliterated with bovine artificial bone.Magnetic resonance imaging and computed tomography scans demonstrated an enlargement of the left orbit, measuring 4.0 cm x 2.6 cm, occupied by a huge

Preoperative sagittal CT evaluation of the frontal recess.

Endoscopic surgical approaches for chronic frontal sinusitis require the reestablishment of adequate frontal sinus ventilation and drainage for relief of symptoms. After the resection of anterior ethmoid mucosal disease and cellular structure, the anterior to posterior depth of the nasofrontal beak to the base of skull at the insertion of the ethmoidal bulla (frontal sinus ostium) often represents a critical margin for functional success. However, little information concerning this dimension is available. Depending on intraoperative surgical judgment of this distance, extended endoscopic surgi

Spontaneous rapid regression of a juvenile primary aneurysmal bone cyst of the skull: A case report and literature review.

Aneurysmal bone cyst (ABC) is a benign lesion that often starts off the metaphysis of long bones and which, as it grows, may blow out bone. Only 3%-6% of cases are located in the skull. Spontaneous recovery has been reported. These cases occur more often in adults and in pelvic locations. Spontaneous regression at the skull level remains a very rare entity and few cases were described in the literature. Here, the authors report another case of spontaneous rapid regression of ABC of the skull in a 7-year-old boy revealed by gradually increasing painless hard swelling in the right frontal bone r…

Further observations on the fate of pedicle osteocutaneous grafts.

Recent work has shown that pedicle osteocutaneous grafts retain their viability and participate actively in the restructuring of transferred bone. The present study investigates two aspects of this process: (1) whether or not healthy recipient bone is needed for the survival of these grafts and (2) the degree of periosteal involvement in osteogenesis. Six adult dogs were used. Pedicle osteocutaneous grafts were constructed using the anterior table of the frontal sinus. Rotation of these flaps allowed implantation of the bone transplant into the ipsilateral parietal muscle. Free bone grafts wer

Calvarial Chondroplastic Osteosarcoma With Distant Brain Metastasis Treated With Radiosurgery: A Rare Case Report.

Background: Cerebral metastases from soft tissue and bone sarcoma are uncommon. Metastatic sarcoma of the brain is a highly aggressive disease with a poor prognosis. There is no consensus regarding the management of cerebral metastases from bone sarcomas. Clinical Presentation: The patient is a 60-year-old, right-handed male, who presented with a right frontal scalp swelling that was hard in consistency. On examination, he had pain and tenderness over the swelling. The neurological examination was normal. Investigations: Initial CTH revealed a right frontal skull lesion with characteristic exp

Reduction of comminuted fractures of the anterior wall of the frontal sinus using threaded Kirschner wires and a small eyebrow incision.

BACKGROUND: Frontal sinus fractures are relatively rare. Their surgical management significantly differs depending on whether the posterior wall is invaded and the clinical features vary. A bicoronal incision or endoscopic approach can be used. However, the minimally invasive approach has been attracting attention, leading us to introduce a simple and effective surgical method using multiple-threaded Kirschner wires. METHODS: All patients had isolated anterior wall fractures without nasofrontal duct impairment. The depth from the skin to the posterior wall was measured using computed tomograp

Topographical variations of the incisive canal and nasopalatine duct in human fetuses.

The incisive canal for nerves and vessels is generally thought to run along a suture between the incisive bone (IN) and maxilla. In contrast, there was a report saying the canal passes through the IN or primary palate in human fetuses. Examination of sagittal and frontal sections from 69 fetuses (31 of gestational age [GA] 9-15 weeks and 38 of GA 26-34 weeks) showed that the canal often penetrated the IN at the nasal half of its course and that, in other fetuses, the canal penetrated the IN along its entire course, irrespective of involvement of the nasopalatine duct. Canals developing in and

Evaluation of craniofacial effects during rapid maxillary expansion through combined in vivo/in vitro and finite element studies.

It is well documented in the literature that a contracted maxilla is commonly associated with nasal obstruction. Midpalatal splitting using the rapid maxillary expansion (RME) technique produces separation of the maxillary halves with consequent widening of the nasal cavity. Although clinicians agree about many of the indications for and outcomes of RME, some disagreements persist in relation to the biomechanical effects induced. The present research was based on the parametric analysis of a finite element model (FEM) of a dry human skull with the RME appliance cemented in place in order to ev

[Original three-dimensional reconstruction of a case of metopism associated with a unilateral complete absence of the left frontal sinus: Clinical interest and review of the literature].

Metopism (complete persistence of the metopic suture in adults) is a rare but not exceptional variation of the calvaria. Hypoplasia or aplasia of the frontal sinus may be associated without evident correlation. Nevertheless, a misdiagnosis of these variations may have clinical consequences, especially in a traumatic context. The aim of this paper was to report a case of metopism associated with a unilateral aplasia of the left frontal sinus, originally illustrated with 3D image fusion volume rendering reconstructions.

A simplified approach for the reduction of specific closed anterior table frontal sinus fractures.

We describe a simplified approach to the management of closed anterior table fractures of the frontal sinus. Forty-seven patients were diagnosed with this type of fracture during a 3-year period. Of these, 6 had minimally-invasive periosteal raising of the anterior table through a small perieyebrow skin incision, and access to the fracture was through a small trephine or the edge of the fracture itself. We compared this with the more traditional approach of a coronal flap, which was associated with an increased duration of stay in hospital and the potential for neurological complications. Surg

[Resection tumor of the anterior skull base region with extend external frontal sinus approach].

OBJECTIVE: To study a better surgical approach for the resection of tumor in the anterior skull base and the fronto-orbito-ethmoidal region. METHOD: Extend external frontal sinus approach was made in the lesion side. The incision can be extended outward to the nasal side or superciliary arch according to the tumors extent and size so as to get a full exposure of tumors of anterior skull base, fronto-orbito-ethmoidal region, or exterior margin of arcola. RESULT: From January 1998 to December 2003, 28 patients suffered tumors of anterior skull base and fronto orbito-ethmoidal region were recei

Cholesteatoma of the frontal and ethmoid areas.

Cholesteatomas of the frontoethmoid region must always be considered in the differential diagnosis of a mass in the frontoethmoid region. A painless, slow-growing mass, often accompanied by proptosis and diplopia, but without history of trauma or infection, should make one suspect of this pathologic entity. Although histologically a benign lesion, the keratinizing squamous epithelial lining will continue to desquamate and expand, leading to erosion of surrounding structures which could be life-threatening. In order to prevent recurrence, complete removal of the cyst lining is mandatory. Cosmet

ACR Appropriateness Criteria® Imaging of Facial Trauma Following Primary Survey.

Maxillofacial trauma patients comprise a significant subset of patients presenting to emergency departments. Before evaluating for facial trauma, an emergency or trauma physician must perform a primary survey to ensure patient stabilization. Following this primary survey, this document discusses the following clinical scenarios for facial trauma: tenderness to palpation or contusion or edema over frontal bone (suspected frontal bone injury); pain with upper jaw manipulation or pain overlying zygoma or zygomatic deformity or facial elongation or malocclusion or infraorbital nerve paresthesia (s…

The sinus septi nasi and other minor pneumatizations of the nasal septum.

The osseous nasal septum (NS) consists of the perpendicular plate of the ethmoid bone (PPE) and the vomer bone. Few studies evaluated the possibilities of septal pneumatization of the PPE, or adjacent to it. We aimed to observe the anatomical possibilities of NS pneumatizations. A retrospective lot of cone-beam computed tomography (CBCT) files was used. One hundred seventy-one CBCT files from 51 males and 120 females were documented. There were found 46 files that were null for septal pneumatization. The other cases (73.1%) had different septal pneumatizations extended from neighboring air spa
