Comparative metabolic safety of efavirenz-, dolutegravir-, and bictegravir-based antiretroviral regimens in HIV-infected adults: Evidence for weight-dependent lipid deterioration.

BACKGROUND: The second-generation integrase strand transfer inhibitors (INSTIs) are gradually replacing efavirenz (EFV) in antiretroviral therapy due to their superior efficacy and tolerability. However, there are still concerns about their long-term metabolic safety, particularly weight gain and dyslipidemia. In real-world settings, comparative evidence on the longitudinal metabolic effects of EFV, dolutegravir (DTG), and bictegravir (BIC) in the Asian population is still limited. This study evaluated the metabolic trajectories of these regimens and explored the mediating role of weight gain

Incidence of metabolic syndrome in people with HIV who start dolutegravir based-regimen compared with bictegravir based-regimen after 48 weeks.

OBJECTIVE: Evidence suggests that patients initiating a second-generation integrase strand transfer inhibitors (INSTI)-based regimen may have a higher risk of developing metabolic syndrome (MetS) compared to those on other antiretroviral classes. This study aimed to describe the incidence of MetS at 48 weeks, based on ATP III criteria, in people with HIV (PWH) who started antiretroviral therapy (ART) with a DTG/ABC/3TC-based regimen compared to those receiving a BIC/TAF/FTC-based regimen. DESIGN: A randomized, open-label clinical trial was conducted in PWH with no prior exposure to ART. METH

A phase IV, open-label three-arm study investigating the impact of a combination of tenofovir disoproxil fumarate/emtricitabine with raltegravir or dolutegravir or elvitegravir/cobicistat on renal function in HIV-1 antiretroviral naïve patients.

Objectives: Tenofovir DF (TDF) remains one of the preferred backbone agents for naïve HIV patients starting antiretroviral treatment (ART). The impact of TDF on renal function and metabolic parameters may vary by anchor agent. We investigated the impact of TDF in combination with 3 different integrase inhibitors on tubular and glomerular function, and metabolic parameters in ART-naïve patients. Methods: Sixty patients with normal renal function were randomised (20 per arm) to TDF/emtricitabine (FTC) plus either raltegravir (RAL) (400 mg b.d.), dolutegravir (DTG) or elvitegravir/cobicistat (EVG/

A systematic review of evidence on weight change following initiation of long-acting cabotegravir for the prevention or treatment of HIV.

BACKGROUND: Weight gain is an increasingly recognized concern among people receiving antiretroviral therapy (ART) and may increase the risk of non-communicable diseases. Integrase inhibitors (INSTIs) such as dolutegravir (DTG) have been associated with weight gain, but the metabolic effects of long-acting cabotegravir (CAB-LA), another INSTI, for HIV treatment and prevention remain incompletely characterized. We conducted a systematic review to synthesize evidence on weight and body mass index (BMI) changes associated with CAB-LA-based regimens across ART and pre-exposure prophylaxis (PrEP) tr

Assessment of the pharmacokinetics of co-administered maraviroc and raltegravir.

WHAT IS ALREADY KNOWN ABOUT THIS SUBJECT: * Maraviroc is a CCR5 receptor antagonist, while raltegravir is a HIV-1 integrase inhibitor. * Based on the known metabolic pathways (CYP3A4 for maraviroc and UGT1A1 for raltegravir), interaction between the two drugs is unlikely. However, unexpected interactions have been reported for other antiretroviral drugs. * As both these drugs are likely to be used in combination, this study evaluated the pharmacokinetic interaction between them. WHAT THIS STUDY ADDS: * Relative to individual monotherapy, co-administration resulted in a 20% and 33% decrease in

Safety and effectiveness of raltegravir in patients with haemophilia and anti-HIV multidrug resistance.

Highly active antiretroviral therapy (HAART) of HIV+ patients with haemophilia poses specific questions on safety and effectiveness because of long-lasting HIV infection, multidrug resistance, concomitant chronic liver disease and bleeding risk. Raltegravir belongs to a new class of drugs that inhibits HIV integrase and is known to have a good effectiveness and safety profile. The aim of this study was to evaluate safety and effectiveness of HAART with raltegravir in patients with haemophilia. HIV+ patients with haemophilia treated with raltegravir for ≥ 6 months were included in this retrospe

Dolutegravir Is Associated With Lower Odds of Gestational Diabetes Compared to Other Antiretroviral Therapy Regimens Among Pregnant Women in India: A Cross-Sectional Analysis.

INTRODUCTION: In non-pregnant adults living with HIV, dolutegravir is associated with weight gain and increased type 2 diabetes risk. In pregnancy, data on the effect of dolutegravir on gestational diabetes (GDM) risk is inconclusive. Data are particularly limited in Asian populations, where diabetes risk is especially high and not always associated with weight gain. We examined the association of dolutegravir with GDM among women living with HIV in India. METHODS: We combined data from two cohorts of pregnant women living with HIV conducted at BJ Government Medical College in Pune, India-the
