[Intermittent-pulse treatment of onychomycosis with orally administered itraconazole].

Fifty-four patients received an intermittent regimen of itraconazole (26 of them were finger onychomycosis and 28 toe onychomycosis) and were followed-up for nine months. The result showed that the clinical cure rate of fingernails was 88.5% and the mycologic cure rate was 96.4%, while the clinical cure rate of toes was 82.1% and the mycologic cure rate was 96.4%. Few adverse events were observed by 7.4%, just like nausea, gastric discomfort etc. It suggests that the pulse administration system is highly effective and has a few side-effects.

Severe allergy to sorbolene cream.

A 40-year-old male developed a constant hand and foot dermatitis in 1980 while using sorbolene cream as a barrier cream at work. He was forced to leave his trade as a motor mechanic because of his hand dermatitis. It was not until 16 years later that he was diagnosed as having an allergic contact dermatitis to chlorocresol secondary to an endogenous hand and foot dermatitis. Chlorocresol is the preservative used in sorbolene cream as well as many corticosteroid creams. Patch testing carried out in 1983 was negative but had omitted testing with chlorocresol or sorbolene cream.

Acitretin and etretinate in the treatment of palmoplantar pustulosis: a double-blind comparative trial.

Sixty patients with palmoplantar pustulosis were treated in a double-blind trial with either acitretin (etretin, Ro 10-1670) or with etretinate. The study consisted of 4 weeks of therapy with three 10 mg capsules/day followed by 8 weeks of therapy with a varying number of capsules given daily according to therapeutic response. At the end of the 12-week treatment period, the mean number of pustules (+/- SEM) had decreased from 57.8 (+/- 8.6) to 3.9 (+/- 1.6) in the acitretin group and from 57.1 (+/- 14.1) to 5.7 (+/- 2.7) in the etretinate group. With regard to influence on erythema, infiltrati

How much skin protection cream is actually applied in the workplace? Determination of dose per skin surface area in nurses.

BACKGROUND: Skin protection creams (PCs) are used in the occupational setting to prevent irritant hand dermatitis. However, so far, the actual amounts of PC applied and the resulting dose per area unit on hands at work have not been a matter of systematic investigation. The quantities used in experimental studies investigating the efficacy of PCs range between 4 and 25 mg/cm(2). OBJECTIVES: To develop a practical and accurate method to analyse the actual consumed quantities of PCs at workplaces in relation to hand surface area. METHODS: Thirty-one hospital nurses without hand eczema were pro

Irritant contact dermatitis in warehouse employees.

A detailed survey of skin complaints amongst 114 airline employees working in a new warehouse revealed 26 cases of skin problems which originated during the 2 1/2 year operation. A clinical survey of broadly the same population confirmed 14 cases from 98 employees as chronic irritant contact dermatitis of the hands. The work involved the reception, unpackaging, inspection, repackaging and dispatch of aircraft parts. The source of the skin irritation was not to be found in the work itself. Rather, the presence among the employees of two severe cases of non-occupational eczema, combined with the

[Incidence of hand eczema after surgical washing and use of surgical gloves].

Contact dermatitis caused by surgical gloves is well known. Contact urticaria and anaphylactoid reactions among hospital personnel and patients following contact with latex gloves and glove dusting powder have previously been reported. A suspected increased frequency of hand eczema among personnel using detergents and surgical gloves in operating units initiated the present study. A questionnaire survey was performed in order to examine the frequency and degree of hand eczema among 332 surgeons and nurses working at the surgical units of Odense University Hospital during the period 1.6.-1.9.19

Handwashing in Finland.

To prevent skin problems we have recommended in Finland that hospital personnel should avoid soap or other detergents for handwashing and instead use alcoholic preparations containing emollients such as 2% glycerol. Alcohol with emollient disinfection is used frequently in hospitals and it causes fewer complaints of skin dryness than washing with soap. However, there are still members of staff who have hand skin problems. Our studies conducted during winter have shown that when these persons used emulsion for hand cleansing, instead of washing with soap, skin deterioration was much less, allow

Getting a Grip on Occupational Hand Dermatitis: Key Considerations for Evaluation and Management.

Occupational hand dermatitis (HD) is a chronic refractory hand eczema that is caused or exacerbated by workplace exposures. The hands are uniquely vulnerable to dermatitis from exposure to allergens and irritants in the workplace, including metals, preservatives, acrylates, rubber accelerators, food, water, plants, and fragrances. In occupational allergic contact dermatitis (ACD), patch testing plays a critical role in identifying contact allergens to help guide avoidance strategies that are key for clinical improvement. A coordinated return-to-work plan is essential for managing patients with

Herpetic whitlow in a child with AIDS: the importance of Tzanck test in the diagnosis.

Herpetic whitlow is a viral infection of the fingers caused by the herpes simplex virus. The disease has a bimodal age distribution, affecting children under 10 years of age and young adults between 20 and 30 years old. It can be easily mistaken for panaritium or bacterial cellulitis. In patients with AIDS, atypical, chronic and recurrent ulcerated lesions occur. The Tzanck test allows a quick and low-cost diagnosis of herpes simplex virus infection. The authors report the case of a child with AIDS with painful finger ulcers in which the diagnosis was confirmed by the Tzanck test.

A self-report questionnaire to detect hand dermatitis in nurses.

BACKGROUND: Hand dermatitis is highly prevalent among nurses due to their frequent exposure to wet work. Providing cost-effective dermatological health surveillance for this occupational group presents a challenge to health service providers. AIMS: To ascertain the predictive value of nurses' self-assessment of whether they had current hand dermatitis using a screening questionnaire when compared with the assessment made by a dermatologist of the nurses' hand photographs. METHODS: We conducted a cross-sectional study comparing the self-report decision made by student and intensive care nurse
