Why is there still hepatitis C transmission in Australian prisons? A case report.

BACKGROUND: The ability to cure hepatitis C viral infection, with specific reference to the prisoner population and the prison environment, will be challenged, even if opiate replacement therapy is concurrently offered, and even if bleach is available. The missing elements, widely available in the community, are a regulated injecting equipment exchange and tattooing parlours. CASE PRESENTATION: We report a case of re-infection of hepatitis C in a prisoner treated with a direct-acting antiviral. What makes this case so remarkable is that it was entirely predictable and preventable. CONCLUSION

Who goes first? Understanding hepatitis C risk among injecting networks in the prison setting.

BACKGROUND: Hepatitis C (HCV) is a blood-borne virus that is most commonly transmitted through shared injecting equipment. Due to the crimin … mong people who inject drugs in prison. Risk of HCV exposure rarely influenced the injecting order. Harm reduction strategies should consider the social factors influencing injecting drug use in the prison setting especially to optimise the population benefits of the roll-out of highly effective HCV treatments.

Impact of institutional smoking bans on reducing harms and secondhand smoke exposure.

BACKGROUND: Smoking bans or restrictions can assist in eliminating nonsmokers' exposure to the dangers of secondhand smoke and can reduce tobacco consumption amongst smokers themselves. Evidence exists identifying the impact of tobacco control regulations and interventions implemented in general workplaces and at an individual level. However, it is important that we also review the evidence for smoking bans at a meso- or organisational level, to identify their impact on reducing the burden of exposure to tobacco smoke. Our review assesses evidence for meso- or organisational-level tobacco cont

Development of a text message intervention aimed at reducing alcohol-related harm in patients admitted to hospital as a result of injury.

BACKGROUND: Screening for alcohol misuse and brief interventions (BIs) for harm in trauma care settings are known to reduce alcohol intake and injury recidivism, but are rarely implemented. We created the content for a mobile phone text message BI service to reduce harmful drinking among patients admitted to hospital following an injury who screen positive for hazardous alcohol use. The aim of this study was to pre-test and refine the text message content using a robust contextualisation process ahead of its formal evaluation in a randomised controlled trial. METHODS: Pre-testing was conducte

Alcohol consumption in late adolescence and early adulthood--where is the problem?

Risky single-occasion drinking (RSOD) is more common in late adolescence and early adulthood (approximately between the ages of 16 and 30) than in any other period in life. This is also the age when young people in Switzerland and many other European countries are legally allowed to buy and drink alcohol, but they usually do not yet have adult responsibilities. This paper reviews evidence from the international literature and provides examples of studies conducted in Switzerland demonstrating that (a) RSOD is by far most prevalent on Saturday evenings followed by Friday evenings, usually becau

An approach to heroin use disorder intervention within the South African context: a content analysis study.

BACKGROUND: The field of heroin use disorder intervention has been in transition in South Africa since the outbreak of the heroin epidemic. Yet despite growing evidence of an association between heroin users' use of supplementary intervention services and intervention outcomes, heroin use disorder intervention programmes in South Africa generally fail to meet international research-based intervention standards. METHODS: Semi-structured interviews with ten heroin use disorder specialists were conducted and the interviews were subjected to content analysis. RESULTS AND DISCUSSION: In terms of

Methadone as HIV prevention: high volume methadone sites to decrease HIV incidence rates in resource limited settings.

The link between injection drug use and HIV has been extensively described. Despite worldwide prevention efforts, injection drug use continues to be a risk factor for HIV transmission and both HIV and injection drug use continues to spread across the globe. Although methadone has demonstrated multiple health benefits including the reduction in injection drug use and HIV acquisition, the utilisation of methadone in many areas of the world remains one of secondary, rather than primary, HIV prevention. As a result, many who finally begin methadone enter treatment having accumulated medical and me

Pediatric Clinical Interventions and Adverse Safety Events Among Emergency Medical Services Encounters in Agencies With and Without a Pediatric Emergency Care Coordinator: An Observational Study.

OBJECTIVES: The purpose of this study was to compare the frequency and types of adverse safety events (ASEs) and potential harm in a diverse population of pediatric emergency medical services (EMS) encounters. As a secondary aim we evaluated the association between prehospital pediatric emergency care coordinators (PECCs) in EMS agencies on the frequency, type and harm severity of ASEs. METHODS: This was a cross-sectional study of EMS and hospital records for children <18 years who received care from 15 EMS agencies from 2019 to 2023, using a validated ASE chart review tool. Koori voices: self-harm, suicide attempts, arrests and substance use among Aboriginal and Torres Strait Islander adolescents following residential treatment.

BACKGROUND: Complex interacting social, economic and historical factors influence the availability and uptake of alcohol and drugs, including among Indigenous communities. Self-harm and suicide as well as homelessness and incarceration, can both precede and result from drug and alcohol use. Rates of self-harm, suicide and incarceration among Aboriginal and Torres Strait Islander people in Australia are among the highest in the world and drug and alcohol treatment programs need to address these underlying complexities. This study examines whether an 'holistic' residential drug and alcohol treat

Antituberculosis BCG vaccination: more reasons for varying innate and adaptive immune responses.

Alternative nicotine delivery products (ANDS) have gained popularity in recent years. Their potentially lower harm compared to traditional cigarettes and the possibility of using them as part of a harm-reduction strategy in tobacco addiction treatment have been discussed. Current epidemiological trends show a gradual increase in ANDS use by the general population, especially among adolescents and young adults, but also among non-smokers. The most comprehensive evidence regarding health effects is available for e-cigarettes, which suggests that their use does not increase cardiovascular risk, e

Female genital cutting: a harmless practice?

A recent article in Medical Anthropology Quarterly (Obermeyer 1999) argues that the "facts" about the "harmful effects" of female genital cutting (FGC) are "not sufficiently supported by the evidence" (p. 79). The article suggests three further hypotheses, among others: (1) FGC may be of minimal harm because the more educated continue the practice just as much as the less educated; (2) FGC may be of minimal harm because it is so widespread and persistent; (3) FGC may be of minimal harm because the supposed link between the clitoris and female sexual pleasure is a social construction rather tha

Competency-based approaches to sexualized substance use: Addressing curriculum gaps in nurse practitioner education.

The rise of chemsex and party-and-play (PnP) among gay, bisexual, and other men who have sex with men (MSM) presents complex challenges for … ore the need for NP graduates to acquire competencies in sexualized drug use, trauma-informed care, harm reduction, social determinants of health (SDOH), and culturally responsive communication. This gap leads to fragmented care, missed diagnoses, and diminished opportunities for early intervention in substance use, sexual health, and mental health. Drawing on 2 …

Age of onset of adolescent alcohol use with parental permission and its impact on drinking and alcohol-harms in young adulthood: A longitudinal study.

Parental permission to use alcohol is common in adolescence, and many parents believe it to be an effective harm reduction strategy because it provides an opportunity to supervise drinking. Contrary to this belief, prior research has consistently linked parental provision of alcohol and permission to drink to increases in future alcohol-related harms. Whether the age of onset of parental permission to use alcohol influences these outcomes is poorly understood. This study is the first to investigate the impact of age of onset of parental permission to use alcohol on later drinking outcomes, uti…
