A 67-year-old female presented with history of recurrent palpitations. During one of the episodes of palpitation, a narrow QRS tachycardia was recorded and it was reported to be terminated with intravenous adenosine. The 12-lead electrocardiogram (ECG) showed no manifest ventricular preexcitation. ECG was within normal limits. The patient underwent an electrophysiology study after informed consent. She was taking calcium channel blockers and that was stopped five half-lives prior to the procedure. As catheters were being placed, a narrow QRS tachycardia got induced. During the study, there was

Diagnosis and timely management of acute myocardial infarction (MI) relies heavily on the ST-segment elevation sum. Presence of concomitant left bundle-branch block (LBBB) in patients presenting with possible acute MI presents a diagnostic dilemma. The LBBB pattern distorts ST-segment changes, delaying or preventing accurate diagnosis. This article reviews the Sgarbossa criteria and ST/S ratio and presents a treatment algorithm that may help improve patient care and reduce morbidity and mortality.

Pulmonary veins isolation (PVI) by radiofrequency (RF) ablation is currently an established treatment for symptomatic, drug-resistant paroxysmal atrial fibrillation. Although the effectiveness of the therapy has been clearly demonstrated, success rate after a single procedure is still sub-optimal. The main reason for recurrences after PVI is electrical pulmonary vein-atrium reconnection. In order to increase the likelihood of permanent PVI, the creation of a transmural, durable lesion is mandatory. The main determinants of lesion size and transmurality are power, stability, duration and contac

Radiofrequency (RF) catheter ablation is a treatment of choice widely used for a variety of supraventricular tachycardia. Transient sinus and atrioventricular (AV) node dysfunctions may occur during RF application to sites remote from the sinus and AV nodes, but they generally resolve quickly after cessation of RF current. We present a case of two episodes of asystole in a 43-year-old man induced by RF catheter ablation of an AV nodal slow pathway. A Bezold-Jarisch-like reflex, direct stimulation of parasympathetic fibers traveling to the sinus and AV nodes, RF-induced myocardial injury or pai

We present a 34-year-old patient with recurrent episodes of drug refractory paroxysmal atrial fibrillation referred for pulmonary vein (PV) isolation at our institution. During isolation of the right superior PV, a dissociated escape rhythm was observed on a portion of the PV that was disconnected, while the rest of the PV showed clear left atrium to PV conduction. This report demonstrates the rare possibility of escape PV rhythm from only partially disconnected PV, highlighting that dissociated PV rhythm does not necessarily reflect complete PV isolation.

A 74-year-old man with idiopathic ventricular tachycardia (VT) with a QRS morphology with a left bundle branch block and right inferior axis underwent electrophysiologic testing. Successful ablation was achieved at the right coronary cusp (RCC) where an excellent pace map with long stimulus-to-QRS interval (St-QRS) (85 ms) was obtained. An excellent pace map was also obtained on the epicardial surface approximately 1 cm away from the successful ablation site and the St-QRS was shorter (40 ms). These findings suggested that a myocardial fibre travelling from the origin in the RCC to the epicard

It is commonly agreed that the electrocardiographic recognition of left ventricular hypertrophy (LVH) is difficult, or even impossible, in patients with bundle branch or fascicular block; the opposite, however, has been demonstrated by several studies. In the presence of intraventricular conduction disturbances, many criteria can reveal LVH, with sensitivity and specificity not inferior than that of electrocardiographic signs used in subjects with normal intraventricular conduction. The following criteria can be helpful in left bundle branch block: QRS voltage increase, left atrial enlargement

AIM: This study aims at developing methods for extracting spatiotemporal information about the electric activity of the atria from electrocardiographic signals, in particular during atrial fibrillation.
METHODS: A biophysical model of the atria and a volume conductor model of the thorax were used to simulate the atrial electrical activity as expressed on the atrial surface as well as on the thorax surface. In all, 22 different types of atrial electric activity were generated, 20 of which related to atrial fibrillation (AF). The spatiotemporal behaviour of the 'true' equivalent dipole expressi

The Fontan circulation has been a major success in the management of patients with functionally single-ventricle cardiac lesions, although arrhythmia remains one of the major complications in the long term and historically has been difficult to treat. The electrophysiologic substrate within the right atrium is complex, with areas of scarring and/or anatomic structures acting as central barriers to conduction in reentrant circuits, which may be mechanistically and anatomically different from typical atrial flutter. Developments in three-dimensional mapping have improved our understanding of dif

The potential for development of 2 to 1 atrioventricular conduction in children with prolongation of the QT interval has been previously reported secondary to electrolytic disturbances. We report here a child who developed 2 to 1 atrioventricular conduction with prolongation of the QT interval following treatment with amiodarone for refractory supraventricular tachycardia. We highlight the importance of electrocardiographic monitoring to assess for those at risk of amiodarone toxicity, which may be manifested by prolongation of the QT interval and the simultaneous loss of atrioventricular cond

Electrolyte abnormalities have become an increasingly important cause of arrhythmias owing to the widespread use of high-potency diuretics. Hypokalemia is one of the common complications of diuretic use. Although some studies of hypokalemia induced by furosemide as well as of potassium-deficient diets in the rat have been reported, the electrocardiographic (ECG) changes during hypokalemia in the rat are poorly understood. This study was designed to examine such changes. For this purpose, hypokalemia was induced by furosemide administration, and the diagnostic criteria for ECG manifestations of

Atrial fibrillation (AF), carries a serious risk of systemic embolic complications, especially cerebral. Antiarrhythmic therapy is the most used method for restoring or maintaining sinus rhythm, and for preventing recurrences or of controlling the ventricular response. A clinical classification was recently suggested to define when to use antiarrhythmic drugs. In a first episode of symptomatic AF (Class I), it is not possible to assess the chances of recurrence and preventive antiarrhythmic therapy would not seem justified. In recurrent paroxysmal AF, the arrhythmias may be asymptomatic (Class

A 4-month-old male Labrador Retriever was presented for recurrent bouts of pulmonary edema associated with tachycardia. Initial physical examination and echocardiography were unremarkable, and the electrocardiogram revealed only an intraventricular conduction disturbance. Subsequent recordings showed paroxysmal supraventricular tachycardia (SVT) (340 beats/min), which consistently produced pulmonary edema. The supraventricular tachycardia was unresponsive to adenosine, esmolol, and propranolol; was variable and transiently responsive to various vagal maneuvers and precordial thumps; and was al

We report a case of a 72-year-old lady with rheumatic heart disease and chronic, long-lasting atrial fibrillation, who reverted spontaneously to sinus rhythm. Doppler study showed evidence of mechanical contraction of the right but not of the left atrium. This may be a sign of marked histologic changes in the atria and may require the insertion of a permanent pacemaker, because of subsequent development of sinus bradycardia.

The authors present a case of trifascicular block: complete right bundle branch block, tachycardia-dependent left anterior hemiblock, and bradycardia-dependent left posterior hemiblock. There is, in addition, a complicating independent AV junctional rhythm that is in most instances not affected by the conducted sinus impulses. Occasionally, however, this focus is discharged by very early sinus impulses that are unexpectedly conducted to the ventricles (a manifestation of "supernormal" conduction). A complex electrocardiographic pattern results from the interplay of the aforementioned mechanism

AQ-A 39 is a new agent with specific bradycardic action on the sinus node. In this study the electrophysiologic and hemodynamic effects of this compound were investigated in 24 patients with various cardiac diseases. The results show: AQ-A 39 causes a significant decrease in enhanced sinus rate. The bradycardic action is more profound at higher sinus rates. In sinus rhythm of normal frequency or, in sinus bradycardia, sinus rate remains unchanged or increases even in the case of sick sinus node syndrome. Conductivity of the AV-node is significantly improved. QT- and QTc-interval increase dose

Amiodarone is an effective antiarrhythmic drug which has been used successfully to treat a variety of cardiac arrhythmias. Early reports emphasized both the striking efficacy of this agent and the relative paucity of side effects necessitating discontinuing treatment with this drug. As amiodarone has been used more widely and in more diverse patient populations, reports of serious thyroid, pulmonary, cardiovascular, and other adverse reactions have appeared in the literature. In this paper, we review the serious adverse effects that have been reported to date. The incidence of these reactions
