[Total endoprosthetic management of the hip joint after failed osteosynthesis of para-articular hip fracture].

OBJECTIVE: Treatment of fractures of the proximal femur by open reduction and internal fixation is prone to complications and frequently requiring secondary joint replacement. The aim of the present study was to examine the results of total hip arthroplasty as a salvage procedure for failed internal fixation of femoral or acetabular fractures. PATIENTS AND METHODS: We retrospectively studied 145 patients who had undergone 146 total hip arthroplasties for failed internal fixation of femoral (n = 135) or acetabular fractures (n = 11). Mean follow-up time after insertion of the hip endoprosthesi…

Poor bone quality or hip structure as risk factors affecting survival of total-hip arthroplasty.

BACKGROUND: The principal long-term complication after total hip arthroplasty (THA) has been aseptic fixation failure. Many hip prostheses and operative techniques have been developed to improve outcomes, but few measures have been taken to cope with poor bone quality or hip structure. We assessed risk factors for aseptic fixation failure after THA. METHODS: We assessed, by multivariate analysis, survival of 405 primary Charnley THAs to identify risk factors for aseptic fixation failures. We also investigated risk factors for development of rapid polyethylene wear (penetration depth of the fe…

Total hip arthroplasty after childhood septic hip in patients younger than 25 years of age.

Childhood septic hip should usually be treated immediately by arthrotomy and antibiotic. Even if treated correctly, the affected hip may become osteoarthritic and functionally disabling. Usually the literature is not in favor of total hip arthroplasty in young patients, and the reports are on patients older than 32 years of age. We present here a unique group of very young patients with early coxarthrosis caused by septic hip in childhood, with an average age of 19.14 years (range, 14-25) at the time of the arthroplasty. The Harris hip score improved from a preoperative mean of 58.43 to a post…

[Ultrasound hip joint screening in newborn infants. Correlation of anamnestic risk factors and hip dysplasia].

BACKGROUND: Congenital dysplasia of the hip (CDH) is the most frequent inborn deformity of the locomotor apparatus. Hereditary, pelvic respectively breech presentation or abdominal delivery, premature as well as post-term birth and twin pregnancy are considered to be anamnestic risk factors for congenital dysplasia of the hip. The results of ultrasound hip screening from July 87 until December 94 are presented with special regard to the correlation of these risk factors and the occurrence of pathologic hips. PATIENTS: 19 different orthopaedic surgeons examined the hips of 3739 newborns (femal…

Current practice in primary total hip replacement: results from the National Hip Replacement Outcome Project.

As part of the National Study of Primary Hip Replacement Outcome, 402 consultant orthopaedic surgeons from three regions were contacted by postal questionnaire which covered all aspects of total hip replacement (THR). There was a 70% response rate of which 71 did not perform hip surgery, a further 33 refused to take part, leaving 181 valid responses. Preoperative assessment clinics were used by 89% of surgeons, but anaesthetists and rehabilitation services were rarely involved at this stage. Of respondents, 99% used routine thromboprophylaxis, with 79% using a combination of mechanical and che…

Perthes' disease after "irritable hip": delayed bone age shows the hip is a "marked man".

We report 13 children, aged 4 through 8 years, who were seen with a prolonged or recurrent irritable hip syndrome. Only those with > 2 years' delay in their bone age eventually declared themselves to have been in the early phase of Perthes' disease at initial examination when the original hip radiographs had been normal. A plain wrist radiograph was found to be a useful screening tool to differentiate those who should have early repeat investigation such as radiographs, or preferably a magnetic resonance image scan if available, to detect the disease in its initial phase. This facilitated earl…

Bipolar hip arthroplasty for recurrent dislocation after total hip arthroplasty. A report of three cases.

Recurrent dislocation after total hip arthroplasty is often a difficult complication to manage. Bipolar prostheses may be useful in these cases because motion can occur at two bearing surfaces and thus permit the greater range of motion necessary to dislodge the head from the acetabulum. The bipolar head is also larger than a conventional total hip femoral component, so a greater volume must be displaced from the acetabulum for dislocation to occur. Three patients with recurrent dislocation of a total hip prosthesis were successfully treated by conversion to bipolar devices after failure of mu…

Cementless total hip arthroplasty for patients with Crowe type III or IV developmental dysplasia of the hip: two-stage total hip arthroplasty following skeletal traction after soft tissue release for irreducible hips.

BACKGROUND: Total hip arthroplasty (THA) for severe developmental dysplasia of the hip (DDH) is a technically demanding procedure for arthroplasty surgeons, and it is often difficult to reduce the hip joint without soft tissue release due to severe flexion contracture. We performed two-stage THAs in irreducible hips with expected lengthening of the affected limb after THA of over 2.5 cm or with flexion contractures of greater than 30 degrees in order to place the acetabular cup in the true acetabulum and to prevent neurologic deficits associated with acute elongation of the limb. The purpose o…

Total Hip Arthroplasty for Post-Traumatic Hip Arthritis in Chronic Pubic Diastasis: A Case Report.

INTRODUCTION: Total hip arthroplasty (THA) in the presence of chronic pubic diastasis is uncommon and technically demanding due to hemipelvic malrotation and abnormal acetabular orientation. CASE PRESENTATION: A 41-year-old woman developed advanced post-traumatic hip osteoarthritis following fixation of an acetabular fracture and sacroiliac joint injury, complicated by a 90-mm chronic pubic diastasis. Preoperative imaging showed a vertically oriented, retroverted acetabulum with external hemipelvic rotation. THA was performed using a modified posterolateral approach with normalized imaging an…

Complication Rates of Hip-Spine Syndrome Patients With Total Hip Arthroplasty and Lumbar Spine Surgery.

INTRODUCTION: Hip-spine syndrome, defined as the coexistence of degeneration in the hip and lumbar spine, is a common disorder in the elderly patient population. There is much debate on whether the hip or lumbar spine should be prioritized for surgical intervention. Although lumbar spine surgery (LSS) and total hip arthroplasty (THA) are well-investigated with proven success, there is limited knowledge on whether the order of surgical intervention influences outcomes. This study investigates dislocation rates and complications in hip-spine syndrome patients with a history of LSS and the impact…

Temporospatial parameters and gait symmetry in hip pathology patients: Pre and post hip preservation surgery.

BACKGROUND: Clinicians specializing in hip preservation surgery perform procedures to increase longevity of the diseased hip. To assess patient ambulation, clinicians use temporospatial parameters (TP), to identify underlying deficits, along with asymmetry. The purpose of this study was to investigate the differences in TP and gait symmetry in a group of adolescent and young adult patients undergoing HPS. METHODS: A retrospective query was conducted for these diagnoses: acetabular hip dysplasia (AHD), femoroacetabular impingement (FAI) and Legg-Calvé-Perthes disease (LCP). Pre-surgery and pos…

From destruction to restoration: total hip arthroplasty in advanced hip joint tuberculosis. A case report.

Hip Tuberculosis (HTB) is a severe form of extrapulmonary Tuberculosis (TB) that can lead to progressive joint destruction, functional impairment, and reduced Quality Of Life (QoL). Optimal management requires timely Antitubercular Therapy (ATT), appropriate surgical intervention, and structured rehabilitation. This case report aims to present a 57-year-old Bulgarian female with advanced HTB, persisting chronic pain, severe functional limitation, and marked restriction in Range Of Motion (ROM) and Muscle Strength (MS). Baseline assessment revealed a Harris Hip Score (HHS) of 56 and pain intens…

Gait changes after intramedullary nailing versus total hip arthroplasty for hip fractures in older adults.

This study aimed to investigate potential differences in spatiotemporal gait parameters and clinical outcomes between older adults undergoing intramedullary nailing (IN) and those undergoing total hip arthroplasty (THA) for unilateral hip fractures. A secondary objective was to identify predictors of postoperative falls in older adults following surgical treatment for hip fractures. We conducted a prospective study involving 42 community-dwelling older adults, assessed 6 months post-surgery. Of these participants, 21 (14 females, 7 males; mean age 76.0 ± 8.6 years) underwent IN, while the rema…

Chronic Kidney Disease and Outcomes of Total Hip Arthroplasty Patients Who Have Periprosthetic Hip Fractures.

BACKGROUND: Patients who have end-stage renal disease (ESRD) and chronic kidney disease (CKD) have an increased risk of mortality and perioperative complications after primary total hip arthroplasty. The association between increasing severity of renal disease with clinical and financial end points in patients treated for hip periprosthetic fracture (PPF) is underexplored in the literature. METHODS: Hip PPF hospitalizations were identified using a large national database and grouped into non-CKD, early-stage CKD (CKD 1 to 3), advanced-stage CKD (CKD 4 to 5), and ESRD. The primary outcome of t…

Hip axis length and non-hip cortical fragility fractures in young postmenopausal nonobese Caucasian women.

INTRODUCTION: Although measuring bone mineral density (BMD) with dual X-ray absorptiometry (DXA) represents the standard of diagnosis and management of osteoporosis, there is a significant number of fragility fractures occurring in young patients without low BMD. Recently, clinical risk tools included hip axis length (HAL), a geometric parameter derived from the hip DXA scan, as a predictor of hip fractures in older postmenopausal women. This study aims to evaluate the relationship between HAL and other cortical bone fractures in young postmenopausal, clinically healthy women. MATERIALS AND M…

Morphological differences between residual childhood hip dysplasia with previous osteotomy and adolescent-onset hip dysplasia.

BACKGROUND: Hip dysplasia (HD) at skeletal maturity can result from residual developmental dysplasia of the hip (DDH) treated in childhood or from primary adolescent-onset HD (AOHD). This study aims to compare the pathomorphology of these two HD subtypes with that of a normal control group. METHODS: This retrospective study reviewed patients who underwent periacetabular osteotomy for symptomatic HD between 2013 and 2020. The study included 27 residual HD patients (32 hips) following a previous pelvic osteotomy and 39 AOHD patients (68 hips), compared to 29 age- and sex-matched healthy individ…
