BACKGROUND: The image diagnosis of idiopathic normal-pressure hydrocephalus (iNPH) is based on the ventriculomegaly, whose criterion is an Evans' Index (EI) >0.3. Recently, disproportionately enlarged subarachnoid space hydrocephalus (DESH) has been proposed as a morphological characteristic to iNPH. Several studies cast doubt on the reliability of these criteria in the diagnosis of iNPH. Furthermore, interobserver differences of these criteria have not yet been investigated. The objective of this study was to assess the diagnostic reliability and interobserver variability of EI and DESH.

OBJECTIVE: Hydrocephalus is a severe disorder characterized by pathological enlargement of the brain ventricles, leading to compression and deformation of brain tissue. The pathophysiological mechanisms underlying some subtypes of hydrocephalus remain poorly understood. Normal pressure hydrocephalus (NPH) continues to be a clinically significant and unresolved issue in elderly care. This study proposes a novel approach to investigate this pathology using mathematical modeling techniques.
METHODS: Using stationary multicomponent poroelasticity equations with physiological boundary conditions,

BACKGROUND: Idiopathic Normal Pressure Hydrocephalus (iNPH) remains a challenging clinical diagnosis with variable treatment response.
OBJECTIVES: This study aimed to identify clinical, imaging, and CSF biomarkers associated with favorable outcomes following shunt placement.
METHODS: All patients evaluated for iNPH at the Tel-Aviv Medical Center (TLVMC) between 2020 and 2022 were included. Participants underwent clinical, cognitive, and imaging assessments, high-volume lumbar puncture (LP). LP responders were referred for shunt placement, and outcomes assessed at one year.
RESULTS: 183 pati

Idiopathic normal pressure hydrocephalus (iNPH), characterized by ventriculomegaly and Hakim's triad of gait disturbance, cognitive impairment, and urinary dysfunction, is common in older adults. iNPH also remains one of the few potentially reversible neurological conditions, typically treated with cerebrospinal fluid (CSF) shunting. Although many patients improve, shunt responsiveness varies widely (≈60%-90%), and a subset of patients show only transient benefit. A major contributor to this variability is the high prevalence of comorbid neurodegenerative disease with symptom overlap, particul

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is suspected by clinical symptoms and enlarged ventricles on imaging (Evans index [EI] >3.0). However, it remains unclear how to deal with patients who present with clinical symptoms but have no ventricular enlargement.
CASE DESCRIPTION: A 77-year-old man, whose symptoms were diagnosed as age-related (because of no ventriculomegaly and negative tap test) at a hospital visited 3 months prior, came to our clinic presenting with gait disturbance, cognitive decline, and urinary incontinence (iNPH triad). His responses to questions were v

Normal pressure hydrocephalus (NPH) is an uncommon neurological disorder that is most often seen in adults over 60 years of age. Its pathogenesis remains uncertain but is believed to involve impaired cerebrospinal fluid (CSF) absorption, increased resistance to CSF outflow, or a mismatch between CSF production and resorption. Reports of NPH in patients younger than 60 are rare, as advanced age is closely associated with the condition. We describe a 55-year-old woman who presented to the emergency department with progressive ataxia, cognitive slowing, and urinary incontinence. Brain imaging dem

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is a reversible cause of dementia, typically treated with shunt surgery, although outcomes vary. Artificial intelligence (AI) advancements could improve predictions of shunt response (SR) by analyzing extensive datasets.
METHODS: We conducted a systematic review to assess AI's effectiveness in predicting SR in iNPH. Studies using AI or machine learning algorithms for SR prediction were identified through searches in MEDLINE, Embase, and Web of Science up to September 2023, adhering to Synthesis Without Meta-Analysis reporting guideli

OBJECTIVE: The ventriculoperitoneal shunt (VPS) is an established approach in treating normal pressure hydrocephalus (NPH). This study aims to examine the long-term effects of VPS regarding clinical and radiological outcomes, to explore interdependencies with comorbidities and medication, and to determine a suitable opening pressure of the programmable valve.
METHODS: 127 patients with VPS were retrospectively evaluated. The Hakim triad along with Evans index (EI) and callosal angle (CA) were examined preoperatively and postoperatively at various time points up to over thirty-six months. Pree

BACKGROUND: Cerebrospinal fluid tap test is a common procedure to predict the efficacy of ventriculoperitoneal shunt for idiopathic normal pressure hydrocephalus. Objective tests after cerebrospinal fluid tap test are used to establish the surgical indication, but subjective improvements may also be important in selection of surgical candidates. The aim of this study was to evaluate surgical outcomes of patients with ventriculoperitoneal shunt for idiopathic normal pressure hydrocephalus, comparing patients showing objective improvement with patients improving only on subjective assessments.

OBJECTIVE: We previously investigated the preclinical state of idiopathic normal pressure hydrocephalus (iNPH): asymptomatic ventriculomegaly with features of iNPH on magnetic resonance imaging (AVIM) found in community inhabitants. The aim of the study was to determine how iNPH develops longitudinally.
MATERIALS AND METHODS: A previous longitudinal prospective community-based cohort study was initiated in 2000. The 271 70 year-old participants were followed up in 2016 at the age of 86 years. At this time, 104 participants could be reached for clinical examinations and brain magnetic resonanc

BACKGROUND: Lumboperitoneal shunt (LPS) is now an effective surgical modality for idiopathic normal pressure hydrocephalus (iNPH), but there is still a lack of clinical data on LPS in older adult iNPH patients in China. We aim to report the shunt effect and the complications of older adult iNPH patients treated with LPS at a single center in Shanghai, China.
METHODS: We conducted a retrospective study among adults over 60 years old who were diagnosed as iNPH and treated with LPS from September 2016 to December 2020. The shunt effect was evaluated from two dimensions of functional and symptoma

OBJECTIVE: Normal pressure hydrocephalus (NPH) and degenerative cervical myelopathy (DCM) can each lead to gait dysfunction and urinary incontinence and may occur concurrently in some patients. In patients presenting with NPH and DCM, there is a paucity of literature describing the medical and surgical complications of treatment and the potential consequences of the sequence of surgical procedures. The aim of this study is to evaluate patients with DCM, NPH, and dual pathology to determine epidemiology and how the order of surgical intervention for both conditions may impact complications and

Introduction Ventricular shunting remains the standard of care for patients with idiopathic normal pressure hydrocephalus (iNPH); however, not all patients benefit from the shunting. Prediction of response in advance can result in improved patient selection for ventricular shunting. This study aims to develop a machine learning predictive model for treatment response after shunt placement using the clinical and radiomics features. Methods In this retrospective pilot study, the medical records of iNPH patients who underwent ventricular shunting were evaluated. In each patient, the "idiopathic n

OBJECTIVE: This study aimed to compare lumboperitoneal shunt (LPS) and ventriculoperitoneal shunt (VPS) in the treatment of idiopathic normal pressure hydrocephalus (iNPH).
METHODS: From September 2016 to November 2019, 76 iNPH patients who underwent shunt operation were recruited and assigned to a lumboperitoneal shunt group (LPS group, n=40) and a ventriculoperitoneal shunt (VPS group, n=36) according to different treatment methods. The right first time (RFT) and improvement in triad of the two groups were observed. Keifer's hydrocephalus score (KHS) was used to evaluate the improvement of
