Metabolic systems approaches update molecular insights of clinical phenotypes and cardiovascular risk in patients with homozygous familial hypercholesterolemia.

BACKGROUND: Homozygous familial hypercholesterolemia (HoFH) is an orphan metabolic disease characterized by extremely elevated low-density lipoprotein cholesterol (LDL-C), xanthomas, aortic stenosis, and premature atherosclerotic cardiovascular disease (ASCVD). In addition to LDL-C, studies in experimental models and small clinical populations have suggested that other types of metabolic molecules might also be risk factors responsible for cardiovascular complications in HoFH, but definitive evidence from large-scale human studies is still lacking. Herein, we aimed to comprehensively character…

Assessment of Statin Treatment for Pulmonary Alveolar Proteinosis without Hypercholesterolemia: A 12-Month Prospective, Longitudinal, and Observational Study.

BACKGROUND: Pulmonary alveolar proteinosis (PAP) is a rare disorder which is characterized by the accumulation of excessive surfactant lipids and proteins in alveolar macrophages and alveoli. Oral statin therapy has been reported to be a novel therapy for PAP with hypercholesterolemia. We aimed to evaluate the safety and efficacy of oral statin therapy for PAP without hypercholesterolemia. METHODS: In a prospective real-world observational study, 47 PAP patients without hypercholesterolemia were screened. Oral statin was initiated as therapy for these PAP patients with 12 months of follow-up.…

Hemoglobin concentration, hematocrit and red blood cell count predict major adverse cardiovascular events in patients with familial hypercholesterolemia.

BACKGROUND AND AIMS: Familial hypercholesterolemia (FH) is a genetic disease associated with an important risk of premature and recurrent atherosclerotic cardiovascular disease (ASCVD). Red blood cell (RBC) parameters such as cell count and hematocrit (HCT) have previously been associated with ASCVD risk in the general population. However, little is known concerning their effect in FH. The aim of the present study is to investigate the effect of the different RBC parameters on the incidence of major adverse cardiovascular events (MACE) in FH patients. METHODS: In this prospective cohort study…

Hypercholesterolemia affects cardiac function, infarct size and inflammation in APOE*3-Leiden mice following myocardial ischemia-reperfusion injury.

BACKGROUND: Hypercholesterolemia is a major risk factor for ischemic heart disease including acute myocardial infarction. However, long-term effects of hypercholesterolemia in a rodent myocardial ischemia-reperfusion injury model are unknown. Therefore, the effects of diet-induced hypercholesterolemia on cardiac function and remodeling were investigated up to eight weeks after myocardial ischemia-reperfusion (MI-R) injury which was induced in either normocholesterolemic (NC-MI) or hypercholesterolemic (HC-MI) APOE*3-Leiden mice. METHODS: Left ventricular (LV) dimensions were serially assessed…

Maternal Hypercholesterolemia Associated with Nicotine Exposure in Adulthood May Induce Kidney Injury in Male Rats if Hypomagnesemia Occurs.

BACKGROUND/AIMS: Maternal hypercholesterolemia is a risk factor to renal injury in rat pups at adulthood, especially if they feed a cholesterol-enriched diet after weaning. However, the renal function of male pups of dams with hypercholesterolemia (PH) that were fed a regular chow from weaning to adulthood needs investigation, particularly those exposed to an adverse risk such as nicotine. METHODS: We evaluated the renal function of PH animals and we compared the data with those found in male pups of control dams (PC) at 3- and 6-month-old by inulin clearance. Moreover, we investigated the ef…

Gonadal steroids, gonadotropins and DHEAS in young adults with familial hypercholesterolemia who had initiated statin therapy in childhood.

OBJECTIVE: Statins are currently the preferred pharmacological therapy in children with familial hypercholesterolemia (FH) with the aim to prevent premature cardiovascular disease (CVD). However, concerns have been raised that lowering cholesterol levels with statins could interfere with hormone production. In this study hormone concentrations were assessed in young adult FH subjects before and 10 years after the initiation of statins, and compared with their unaffected siblings. METHODS: All 214 FH children (8-18 years) who were previously randomized into a placebo-controlled trial evaluatin…

Translational lessons from a case of combined heart and liver transplantation for familial hypercholesterolemia 20 years post-operatively.

We review the lessons from a case of combined heart and liver transplantation (CHLT) 20 years post-operatively from the molecular to clinical levels. CHLT replaces cardiac function and provides a new source of Low density lipoprotein-receptors (LDL-R) known to be deficient in Familial Hypercholesterolaemia. Little is known of the long-term outcomes of this strategy. We review the lessons from a case of CHLT 20 years post-operatively, which illustrate the successful transition from the molecular understanding of the pathophysiology to the clinical therapy. Most importantly, there is evidence th

Plasma adiponectin levels in primary biliary cirrhosis: a novel perspective for link between hypercholesterolemia and protection against atherosclerosis.

INTRODUCTION: Hypercholesterolemia is a common finding in primary biliary cirrhosis (PBC), but the risk of cardiovascular events in PBC patients is not increased in respect to the general population. High serum adiponectin levels appear to play a protective role in the development of either metabolic syndrome or cardiovascular disease. AIM: To investigate factors potentially preventing atherosclerosis in PBC patients. METHODS: Circulating levels of adiponectin, resistin, leptin, and tumor necrosis factor-alpha (TNF-alpha) were measured in 137 consecutive PBC patients (125 women, 12 men; mean…

Ginsenoside Rb3 Combined with 20(S)-Protopanaxadiol Ameliorates Hypercholesterolemia via the Regulation of Lipid Metabolism and Gut Microbiota.

Hypercholesterolemia is a major contributor to cardiovascular disease and imposes a serious global disease burden. The stems and leaves of Panax notoginseng, as edible parts of the plant, are rich in ginsenoside Rb3. However, the lipid-lowering effects and mechanism of ginsenoside Rb3 remain unclear, which limits the utilization of Panax notoginseng stems and leaves. Our results showed that ginsenoside Rb3, particularly with PPD, inhibited cholesterol absorption, enhanced LDLR-mediated LDL uptake, reduced body weight, alleviated dyslipidemia, and attenuated liver injury and lipid accumulation.…

Unveiling perceptions: a small-scale qualitative case study related to patient experience of inpatient screening for familial hypercholesterolemia.

BACKGROUND: Familial hypercholesterolaemia (FH) is a hereditary, genetic disorder disrupting the metabolism of low-density cholesterol from birth. Untreated, 50% of men develop atherosclerotic cardiovascular disease (ASCVD) by age 50 and 30% of women by age 60. Evidence supports screening individuals with premature ASCVD; however, screening is limited, particularly in inpatient settings. AIM: Our aim was to explore the experiences of individuals presenting with premature ASCVD who were screened for FH during hospitalisation. METHOD: A small-scale qualitative case study followed COREQ (COnsol

Assessment of LDL receptor-dependent lipid lowering therapies in patients with homozygous familial hypercholesterolemia according to functional genotype.

BACKGROUND AND AIMS: Patients with homozygous familial hypercholesterolemia (HoFH) respond to standard lipid lowering therapy (LLT) poorly due to biallelic abnormalities affecting low-density lipoprotein receptor (LDLR) function. Relatively little information stratified by functional genotyping is available. METHODS: We evaluated lowering of low-density lipoprotein cholesterol (LDL-C) with statins, statins plus ezetimibe or add-on proprotein convertase subtilisin-kexin type 9 (PCSK9) inhibitors in 175 patients with HoFH according to LDLR functional genotyping (defective/defective = 119, defec…

CONCLUSIONS: We found a high prevalence of LDL hypercholesterolemia. The use of LLM was low, despite treatment indication, and treatment targets were mostly not achieved either due to underdosing or nonadherence to LLM. These findings confirm that dyslipidemia remains an underestimated cardiovascular risk factor in …

Bridging the gap in familial hypercholesterolemia: An adult care perspective on transitioning emerging adults toward lifelong cardiovascular prevention.

BACKGROUND: Familial hypercholesterolemia (FH) confers lifelong elevation in low-density lipoprotein-cholesterol (LDL-C) and markedly increases the risk for premature atherosclerotic cardiovascular disease. Because atherosclerosis begins early in FH, timely initiation of statin therapy reduces lifetime LDL-C exposure, slows disease progression, and improves cardiovascular event-free survival. SOURCES OF MATERIAL: Relevant studies were identified through PubMed and Embase searches (2010-2025) using the terms FH, transition of care, and lipid management, supplemented by guidelines and consensus…

Simulation model to estimate pretreatment (baseline) low-density lipoprotein cholesterol levels in people living with homozygous familial hypercholesterolemia.

INTRODUCTION: Pretreatment levels of low-density lipoprotein cholesterol (LDL-C) are a primary consideration for the clinical diagnosis of homozygous familial hypercholesterolemia (HoFH). However, determination of pretreatment LDL-C can be challenging in this population because many individuals begin lipid-lowering therapy (LLT) early in life before diagnosis of HoFH. METHODS: A previously developed simulation model was adapted to estimate pretreatment LDL-C in people living with HoFH, based on their treated LDL-C value and current LLT (statin, ezetimibe, alirocumab, evolocumab, and/or lipopr…
